POSTCOVID SPINSELS 🦠 ACTIE! Dat de hele situatie rondom mijn ziekte bij ons gezin er behoorlijk inhakt zal wel duidelijk zijn. Een moeder die altijd of in bed ligt, óf op de bank, is op z'n zachts gezegd niet fijn. Ze zagen hoe longcovid me van een actieve vrouw veranderde in een schim van hoe ik was. Gevangen in mijn lichaam. Manlief en kinderen doen veel voor me, ontzettend veel om ons gezin draaiende te houden. Ze zijn er altijd. Begin dit jaar, in januari, vertelde Karsten ons dat hij een actie had opgezet om geld voor onderzoek in te zamelen voor @stichting_long_covid Hij voelt zich machteloos en wil dit graag omzetten in actie! Hij heeft zich ingeschreven voor een kwart triatlon! Zoiets heeft hij nog nooit eerder gedaan, maar hij houdt van sporten. Een prachtige uitdaging dus! En wat een fantastisch gebaar! Inmiddels heeft hij al meer dan 6200 euro voor onderzoek opgehaald!! Wij zijn ongelofelijk trots op hem! Hoe mooi is het als je je verdriet en machteloosheid om kunt zetten in positiviteit en daadkracht! Volgende week is het dan zover. Het voetbalseizoen is dan net ten einde dus dat is goed gepland. Na maanden trainen en voorbereiding gaat hij het aankomende week wat rustiger houden. G-BA Richtlinie, Teil 4 unserer Serie: Die Richtlinie "LongCOV-RL" des G-BA wurde am 21.12.2023 beschlossen und trat am 09.05.2024 in Kraft. Sie regelt die Versorgung von Patienten mit Post & Long COVID, ME/CFS, Post VAC und Erkrankungen mit ähnlicher Symptomatik. ℹ️Eine medizinische Richtlinie hat einen höheren Verbindlichkeitsgrad als eine medizinische Leitlinie und hat damit einen stärkeren Gesetzes-Charakter. ℹ️Der Richtlinien-Text beinhaltet 9 Paragrafen und ergibt zusammen mit den "Tragenden Gründen" (Begleittext) eine konkrete Hilfestellung für Ärztinnen/Ärzte auf dem Weg zur Diagnose und regelt die Versorgung von Betroffenen. ℹ️Aufgrund der engagierten Mitarbeit des @fatigatio.mecfs als Patientenvertretung im Unterausschuss des G-BA, ist es gelungen zahlreiche wichtige Aspekte für die Versorgung von ME/CFS-Betroffenen in der Richtlinie unterzubringen. Kurzüberblick über den Inhalt der Paragrafen 7-9 ⬇️ 📌 In §7 wird die Versorgung auf der Ebene der universitären Spezialambulanz beschrieben. Hier sind u.a. die Durchführung von Video-Sprechstunden und die Ermöglichung von Teilnahme an Studien als besondere Möglichkeiten zu erwähnen. 📌 Der §8 regelt die Verordnung weiterer Leistungen, z. B. Heil- und Hilfsmittel, Pflegeleistungen, Reha-Maßnahmen u.v.m. Bei PEM ist diese auf den Verordnungen zu erwähnen, außerdem Reiz-Intoleranz oder eingeschränkte Mobilität. Die Spezialisierte Ambulante Palliativversorgung (SAPV) konnte ebenfalls durch unser beharrliches Argumentieren in diesem Paragrafen verankert werden. Dies sollte insbesondere für Schwerstbetroffene eine Erleichterung beim Erreichen komplexer Pflegeleistungen bringen. 📌 §9 regelt abschließend den Umgang mit einer möglichen Evaluation der Richtlinie. Your #baseline is the normal level of functioning and symptoms present with your #chronicIllness such as #MyalgicEncephalomyelitis #MECFS at any given time. When we #crash, experience severe Post Exertional Neuroimmune Exhaustion / Malaise #PENE aka #PEM, or experience a #relase our baseline changes. #Pacing during #baselineChanges is difficult because it disrupts our daily routines which are a staple of #pacing. A sharp increase or decrease in symptoms can be used to identify changes in baseline. This can be done alongside monitoring changing in rhr (increase of 10bpm indicates crash) and HRV (sustained lowered HRV indicates lowered baseline). Baseline decline can lead to #chronicIllnessGrief. It is essential to sharply decrease activity following baseline decline. When baseline improves you can slowly increase activity while continuing to listen to your body. Professional guidance and therapy such as #ACT can help us process the loss of baseline decline. It is essential that denial does not cause a delay in sharply reducing activity following a #crash because this can lead to even further crashing and loss of baseline. It may help to think of the setback as temporary to start. In cases of improvement movement coaches such as Julie from @movement_for_me or guidance from a PT can be helpful for optomizing adding movement to your routine. To avoid added movement becoming #GET: 1.) Guide movement increases by how you feel. 2.) Wait to see how each increase affects you before adding activity. 3.) When you reach a point where increasing movement further causes post exertional symptom stop. Pacing through baseline changes is one of the hardest parts of living with a #relapsingRemitting or #progressiveChronicIllness. It is a skill that takes time to learn. But the faster you are able to master it, the better off you wil be. Nikotin hat eine sehr kurze biologische Halbwertszeit (ca. eine Stunde). Die spezifische Immunantwort benötigt jedoch zur Erkennung und Beseitigung der freigesetzten Virusbestandteile eher mehrere Tage. Eine Unterbrechung oder Beendigung der Nikotinpflaster-Anwendung vor Abschluss der Immunantwort führt dazu, dass sich die frei gewordenen Viruspartikel/Spike-Proteine wieder an die nAChRs binden, statt vom Immunsystem eliminiert zu werden. Da auch Immunzellen sehr viele nAChRs für ihre Funktion benötigen, kann deren Blockade die Immunantwort stark verzögern, weswegen der zeitliche Verlauf nicht vorhersagbar ist. So könnte aus der Kombination der verzögerten Immunantwort und der stundenweise unterbrochenen Anwendung ein Ping-Pong-Effekt zwischen gebundenen und freien Viruspartikeln entstehen, der deren effektive Beseitigung verhindert. Wegen der erwähnten Halbwertszeit von Nikotin ist ein kurzes Entfernen des Pflasters (z.
