We love a progress update! Swipe to see how far weâve come in just 2 weeks with this kitty. She initially came to see me on May 2, which is the very last image, with a very deep infected corneal ulcer. We started intensive medical management and saw her back in 5 days, at which time she had already made huge strides (middle photo). Fast forward to today and her eye is all healed! There is some remodeling of the scar that will happen with time but she can be done with medications and go back to normal life. Take a look at these images and tell me what you think is going on with this dog before reading onwards. How many issues can you spot? What if I told you there was a big, invisible systemic issue too? This dog came to see me for vision loss and ocular discharge. The vision loss was acute, but the discharge had been going on for at least a few weeks. On exam, this dog had very low tear production (dry eye), which is the cause for the discharge and the corneal blood vessels you can see growing in from the top of the eye. We also diagnosed complete cataracts, which appeared very swollen (aka intumescent). At this point I was very suspicious of an underlying issue. More questioning revealed that the dog had been acting more lethargic, drinking and urinating more, and had lost weight over the past month. We checked bloodwork because I was concerned we were missing the big picture, and the dogâs blood glucose was nearly 500! This little dog had developed diabetes, but because the signs were fairly vague and insidious, hadnât been diagnosed yet. Diabetes can be life threatening if untreated, so we immediately got them set up with insulin and a follow up plan for the dogâs ongoing care. About 85% of dogs with diabetes develop cataracts within the first year of being diabetic - while itâs uncommon for the ophthalmologist to find the diabetes, this is not the first time! Dry eye is also more common in diabetic dogs. I also started them on eye medication to help decrease inflammation and improve tear production. Whoâs up for some systemic disease discussion? This dog presented for glaucoma in the left eye - at the primary care vet, her intraocular pressure was in the 90s! Normal is 12-25mmHg, so thatâs incredibly abnormal. They started some meds and referred, and some of the corneal edema had died down by the time she came to me. As you can see in the image, she has hyphema (blood in the front of the eye), as well as some bleeding in the back part of the eye. Her pressure was still in the 80s, and the eye was blind. Because this pup was a little older and due to the presence of hyphema, we went looking for causes for the blood and high pressure - primary(inherited) glaucoma rarely causes that much bleeding, and usually crops up in middle aged dogs. We found this pupâs systemic blood pressure was markedly elevated and her kidney values were high! High blood pressure (hypertension) can cause bleeding inside the eye, and subsequently glaucoma from the inflammation and damage, which is what I suspect happened here. Hypertension can also be linked to kidney disease. We started this dog on additional eye medication to help control pressure in the eye, blood pressure medication, and will have her follow up with us for the eyes and her primary care for the systemic concerns. Did you know that fluorescein stain is useful for more than just corneal ulcer diagnosis? This cat is displaying a very nice example of positive (L nostril) and negative (R nostril) Jones test. The Jones test uses fluorescein stain to evaluate the patency of the nasolacrimal ducts. Observing the stain in the nostril is a positive test, and you can be sure that the duct is patent. A negative test (no stain) does not always indicate a duct obstruction but can be supportive with concurrent clinical signs. Some patients have a NL duct that opens into the mouth, or poor tear production that decreases the chance of a positive test. There are several confounding factors that can be at play. Glaucoma causes increased pressure inside the eye (intraocular pressure) and results in pain and blindness. Primary glaucoma means that there is an inherited abnormality in the drainage system in both eyes that eventually leads to issues. Like many dogs with primary glaucoma, Moe already had end-stage damage to his left eye when he first came to our ophthalmology department over a year ago, and his left eye was removed for his long term comfort. We have been monitoring and treating his right eye prophylactically, and when the pressure started to increase his family acted quickly. Dr. Fuchs was able to get the pressure down medically and perform surgery to keep the pressure low and preserve his vision. This involved a combination of a laser surgery to decrease intraocular fluid production, and placement of an anterior chamber shunt to increase fluid outflow. Moe is doing well since surgery and hopefully will continue to maintain. We love a progress series! This series shows the medical management of a very nasty, infected corneal ulcer with uveitis that was the consequence of a dog bite to the face. The first image is today, day 22 of treatment. Scroll through to see day 9, day 2, and day 0 (eeeek)! Thanks to a combination of medical therapies and a dedicated owner, this little dog is well on his way to normal. Now that the ulcer is healed over and the infection resolved, we are focusing on reducing edema in the cornea and giving time for the scarring to remodel. đđŸđ©”Itâs Finally Fr-Eye-Day! đ©”đđŸ How perfect is Lola? This beautiful girl was referred to our ophthalmology service due to an infected, ruptured corneal ulcer in the left eye. She also had a prolapsed gland of the third eyelid (also known as cherry eye) in both eyes. Due to Lolaâs personality and the severity of her changes to the left eye, her family and our ophthalmologist decided that the best thing to restore her comfort would be eye removal surgery. Fortunately dogs and cats can do just fine with only one eye. Lolaâs left eye was removed, and at the same time Dr. Fuchs was able to repair the cherry eye in the right side. 1 week later. This cat was referred last week for what was suspected to be a deep ulcer. She had a descemetocele, which is the deepest possible corneal ulcer before we end up inside the eye! The ulcerated cornea was infected and very malacic (melty), and there was a high risk of corneal rupture. We discussed the risks and benefits of medical therapy versus surgery, and the clients opted to have surgery to graft the cornea and help support healing. One week later, her graft is incorporating nicely and while it looks super red and ugly, thatâs normal and tells us the graft is healthy! We love getting to see little kittens - I fully own being a crazy cat lady! This perfect little peanut has been getting healthy with her foster family, and had very severe eye issues just a few weeks ago. Thankfully everything has healed and she is left with a condition called symblepharon. This occurs when there are ulcers on the cornea and conjunctiva, allowing them to adhere together as they heal. It can be so severe it results in blindness, or it can be so mild itâs barely noticeable. This is moderate in the right eye, and she has resolving keratitis in both eyes. How perfect is Lola? This beautiful girl was referred to my ophthalmology service due to an infected, ruptured corneal ulcer in the left eye. She also had a prolapsed gland of the third eyelid (also known as cherry eye) in both eyes. Due to Lolaâs personality (unwillingness to participate in her own healthcare) and the severity of her changes to the left eye, we decided that the best thing to restore her comfort would be eye removal surgery. Fortunately dogs and cats can do just fine with only one eye. Lolaâs left eye was removed, and at the same time I was able to repair the cherry eye in the right side. Another wonderful progress update on one of the recent chorioretinitis dogs I am treating! Her initial images are the first two, swipe to see what she looks like just one week later. Her retinas have re-attached and she regained vision. Like some of the other recent chorioretinitis cases, all of this dogâs tests came back normal and she is being presumptively treated for immune mediated disease. I have never had so many immune mediated chorioretinitis cases at one time! The thorn penetrated into the front part of the eye, risking tracking bacteria into the eye and damage to the delicate structures like the iris and lens. Dr. Fuchs was able to remove the thorn and determine that the lens inside the eye had not been damaged. Willow was treated to prevent infection and to help the entry wound in the cornea heal. Just one week later, she was all healed, with no evidence of infection and normal vision. This is a 9yr old cat with progressive iris pigmentation. His intraocular pressure is normal and there is no active inflammation inside the eye. He is visual and comfortable. What is this and what do we do about it? I find this condition challenging because we have so little solid information about how it behaves! This is FDIM, or feline diffuse iris melanosis/melanoma. There is no real way to tell without histopathology whether the change is benign or malignant. There are reports of metastatic disease from iris melanoma in cats, but the reported rates range from 16-63% which is wildly unhelpful. Some people advocate for early enucleation. Personally I offer a range of options depending on the catâs age, rate of progression, the appearance of the lesions, and the ownerâs motivation. We can monitor (watch and wait), take an iris biopsy, perform trans-corneal laser ablation to try to slow down the growth, or remove the eye. I also offer staging (chest x-rays and abdominal ultrasound) prior to surgery, especially in more advanced cases. Progress update! Pay attention to the dates on these images. These are the left and right retinas of the same dog, just a few weeks apart. We posted the initial images for a fundus Friday. This dog presented for decreased vision and elevated third eyelids, as well as some bloodwork changes including low platelets and an increase in lymphocytes (a type of white blood cell). The main concerns were systemic infectious diseases like tick borne and fungal diseases, certain types of cancer, or an immune mediated disease resulting in chorioretinitis and retinal detachment. We performed extensive diagnostic testing including imaging, bloodwork, and testing for most of the infections that cause these signs and everything came back normal. We are treating this dog as immune mediated and fortunately seeing a great response to therapy with oral steroids. đŸđItâs Finally Fr-Eye-Day!đđŸ This big sweet meathead is Norman. He came to see our ophthalmologist, Dr. Allison Fuchs, for squinting and a corneal ulcer. Norman was diagnosed with a corneal ulcer with dense granulation tissue on the left cornea, as well as entropion in both eyes. Entropion is when the eyelids roll inwards and rub on the ocular surface, causing discomfort, tearing, and potentially corneal damage. Norman underwent eyelid correction surgery the next day and he looks amazing at his follow up two weeks later! He was so painful he wouldnât open his eyes at the first visit. Now his eyes are open and comfortable, the cornea is scarring down, and his tail didnât stop wagging for one single second while he got his sutures removed and lots of belly rubs. Another day, another chorioretinitis dog. This dog came to see me last week for squinting and vision loss. She had very severe uveitis (inflammation of the front of the eye) and chorioretinitis (inflammation under the retina), causing bullous retinal detachments that you can see here as the big billowing areas of tissue coming towards the camera. Uveitis/chorioretinitis can be caused by a wide variety of systemic infections, cancers, metabolic diseases, and immune mediated disorders. We did an extensive systemic workup on this dog to evaluate for any underlying causes. So far all of her tests have come back normal, so we are treating for presumed immune mediated disease with systemic steroids and anti-inflammatory antibiotics while we wait for the results of the last tests to return. Hopefully we can get her ocular signs controlled rapidly and recover her vision. You will not believe the timeline on these images. The first photo was taken on day zero, after this little dog got walloped by a cat - odd, because she and the cat have been friends and it seemed like she startled the cat. She had been losing vision and her eyes were getting progressively cloudier for the past year. Based on photos from one of our ER vets, we started her on aggressive treatment that afternoon for a corneal laceration and she came to see me the next day which is when the next 2 photos are from. We diagnosed her with a (thankfully) sealed corneal laceration and immune mediated keratitis. Treatment for IMMK involves suppressing the corneal immune system, which is bad news if you have a potential source of infection inside your eye. We started treating the other eye with immunosuppressants and continued her therapy for the laceration. The last 2 photos are from just one week later! Look at how amazing she looks already! Monday progress photos! This sweet little dog has been through a lot, and unfortunately last week he acutely developed copious mucopurulent discharge and started squinting. He presented with this nasty little deep ulcer, and zero tear production. We started aggressive treatment and thankfully the ulcer is healed today. His tear production already looks better. Happy Friday! Letâs share another equine case. This horse presented to me due to acute onset of a cloudy spot in his eye that had worsened on a course of NeoPolyDexamethasone ointment. Historically, he had intermittent flare ups of this âspotâ over the last few years that generally resolved after a few days on NeoPolyBac - but this time there was a mixup with medication and he received the steroid instead. The first image is our first examination and you can see the very dense âspotâ of concern in the ventral cornea, with little blood vessels. There is a lot of yellowish haze, indicating white blood cell infiltration and likely infection. There is a band of similar infiltrate laterally, and multifocal punctate cloudy white opacities throughout the cornea. I was very concerned that we were dealing with a stromal fungal infection, but the horse was surprisingly comfortable. I was also worried about an underlying immune mediated corneal disease because of the multifocal white spots. We treated him aggressively, and at 1 week later (second image), you can see some improvement in the yellow infiltrate. We made some tweaks based on his degree of response and at the next exam 2 weeks later, all the yellow is gone! Another day, another weird case! See the left and right retinas of a sweet middle aged large breed dog. A month ago I saw her for ocular surface disease and she sure didnât look like this! Can you spot it? Her left eye has a swollen optic nerve with edema in the retina around it. She is fortunately still visual. We were able to get her right into a neurologist for an MRI to evaluate the optic nerve. It turns out she has very localized inflammation in the retina and the first little section of the optic nerve. We are hopeful that she will respond to systemic steroids to treat what is likely to be immune mediated disease. What is your diagnosis for this horseâs eye? A bit of background: this pony sustained some sort of trauma in the field 6 weeks prior to this photo. He came in with a severely swollen periocular area, some wounds in the skin of the eyelids, and complete corneal edema. We treated him with topical antibiotics, oral antibiotics, and oral non-steroidal antiinflammatories.
