One Sunday morning, Jill woke up and realized that the right side of her mouth felt different when trying to put her lipstick on. By the time she had lunch, she was unable to drink from a straw, and the entire side of her face felt numb. Jill Watson is an SLP. She’s all too familiar with what unilateral facial weakness and numbness could mean… So she drove herself to the ER and got worked up for a stroke. Thankfully, no stroke! But it turned out she had Bell’s Palsy. Unable to fully close her right eye, smile, or feel any sensation on that side of her face, Jill immediately started a 21-day course of steroids and antivirals. “It definitely scared me,” she told us. The virus and inflammation had cleared up by the end of the 21 days, but she still hadn’t regained full function of that side of her face. So, in classic med SLP form, Jill turned to the evidence. “I had just completed the in-person certification for Ampcare ESP, and I immediately thought I must try the facial protocol.” She went to work early every morning and spent the first 30 minutes with ESP facial placement on the right side of her face in conjunction with oral/facial exercises. She did this five days a week for eight weeks. The outcome? In 2021, Steele, Ennis, & Dobler published an article entitled "Treatment burden associated with the intake of thickened fluids" and they reviewed the evidence for utilizing thickened liquids. Here's what they found: "The implementation of thickened fluids in patients with dysphagia is widely considered an effective strategy for safe and physiologically improved swallow. However, there is limited evidence to suggest that this intervention reduces the risk of dysphagia-related complications, including aspiration pneumonia. In addition, there is growing evidence that this approach is associated with adverse clinical effects including dehydration, malnutrition, and reduced health-related quality of life." A systematic review by Swan et al. (2015) identified eight studies (including two very small randomized controlled trials with 15 and 18 participants, respectively) that examined the effects of altering food (typically cutting, mashing, or puréeing) or fluids (typically thickening) on quality of life. The included trials showed that less modified textures (i.e. normal or nearly unmodified food/drinks) were associated with considerably better HRQoL than more modified textures. So, if you have patients that are on thickened liquids, let us help you help them! In addition to eliminating health risks like dehydration and pulmonary injury due to aspiration of thickener (Nativ-Zeltzer et al., 2021), we want to improve the quality of life for patients with dysphagia! How does Ampcare improve airway protection during swallowing? If you chose B, we hope you were joking as much as we were when we wrote the question. 😂 But if you chose D, then you, my friend, are a WINNER! Or at least a pretty dang sharp SLP! The platysma definitely didn't get a ton of air time in grad school, but when it comes to NMES, it's important to understand. This muscle is like a sheet that covers the other muscles in the neck. It sits superficially in your neck (closest to the skin, farthest from the airway) and runs from your clavicle region to your chin, where it inserts into the lower border of the mandible. Now, why do we care? Does it have anything to do with swallowing? Well, no... BUT! When we're considering using NMES on our patients, it's important to understand what muscles you can reach with the stimulation and be mindful of what muscles (or other structures) you have to go THROUGH to get to the ones you want to stimulate. The electrical current doesn't cherry-pick. While our technology is remarkable, we can't tell the current to skip this muscle or that one and head straight to the hyolaryngeal muscles. It will stimulate everything from point A (the electrode) to point B (ideally the suprahyoid muscles.) Therefore, if you're doing true NMES, you must see a contraction of the platysma if you plan on targeting deeper musculature. So, if you've ever been taught that you can stimulate muscles that are considerably deep in the neck (like maybe in the pharynx,) this is a little lesson to keep in mind. For the cancer population, the use of NMES would be contraindicated if the cancer is in the treatment field. The issue Participants will leave with this population is the mechanical dysfunction created by either the surgical dissection of pertinent muscle groups, the radiation a clear understanding of the tissue, or both. However, as we discussed in our training, the use of Ampcare's Effective Swallowing Protocol may be considered after confirmation from the oncologist's critical role that the patient is "cancer-free." Considerations for this population should include: 🌟 inflamed salivary glands 🌟 change MSV plays in the skin integrity since the radiation 🌟 any additional history 🌟 trach presence Knowing the radiated tissue will continue to become increasingly fibrotic, it is likely that the patient will be worse off with swallowing 5-10 years after the curative treatments and rehab therapy if therapy and aggressive home exercise are not maintained. "Can't I just use a cervical collar instead of the Restorative Posture Device?" We've gotten this question a few times, so we've got the answer for you! We tried to validate other standard cervical collars, but they could not sustain or maintain a standard amount of pressure that is consistent with a traditional head lift exercise (Watts et al 2016). Our technology supports multiple exercise principles based on the patient’s needs. 💪The effortful swallowing exercise is an isotonic exercise incorporating NMES to work on timing, initiation, and speed of the contraction. 💪The Chin-to-Chest (CtC) exercise incorporates an isometric exercise incorporating the Restorative Posture Device (RPD) using time under tension (TUT) and total end range time (TERT) principles to facilitate strength. 💪The “S” curve also allows for sufficient recoil to provide the same amount of resistance over multiple repetitions. This is critical if the RPD is used as part of a home exercise program and/or for improving head and neck posture. 🎈We have observed clinicians using towel rolls, inflatable balls, balloons, etc. ask us the same question, but there is no validation that those products provide the same amount of resistance with each pressure over time. 🏋️♀️The RPD has been tested/validated under a robot to withstand up to 5.5 lbs of pressure for 5 seconds aka CtC exercise (enough to elicit twice the amount of force as a traditional head lift – Watts 2016), 10,800 times (e.g. up to 90 repetitions per day, 5 times a week for 24 weeks) to make sure it provided the same amount of resistance over the number of repetitions. MedSLPs! How do you know if your patient is *actually* doing an Effortful Swallow? Or a Mendelsohn? Why don’t you pull the larynx down and have the patient overcome that downward resistance to swallow to achieve greater elevation? This is a common question and a very common misconception about how NMES "should" work! The larynx should never go down to come up. We’re teaching the brain/nerves/muscles what they should do, as we strengthen the process. Let’s teach the correct movement (task specificity) so we don’t create more hurdles to overcome later. Using NMES will strengthen the targeted muscle and improve the neural pathway for that muscle's function. That should be the goal. Term you should know: ⭐️Perturbation: a deviation of a moving object from its normal path, caused by an outside influence. Ampcare's ESP perturbates the laryngeal vestibule by pulling it open and holding it anteriorly/superiorly to strengthen and speed up the closure of the laryngeal vestibule during a regular swallow with a bolus. We see two electrodes under the chin, specifically on the suprahyoid musculature. 🌟If the goal is to move the hyoid forward (anterior) & upward (superior), you would want to SEE that muscle contraction (like we do on the far left!) ❌ If you're not seeing a muscle contraction with your device, it's not magically jumping through all the skin & tissue, contracting something you can't see. 👀Electrical current has to go through the skin to get to the muscles to move it! That's when you can see a visible muscle contraction that moves the muscles in the right direction. 🌟IF you cannot see or feel movement, you're basically doing sensory stimulation (which is okay if that's your goal!) 🚨WARNING: tear-jerking case ahead...💔 Kristen, a med SLP, recently faced a new challenge in assessing & treating dysphagia for a patient with a tracheostomy. 😢The patient was a high school math teacher who was struck by a car, which left him in a coma for 8 months. 🗣️"We took this case because the team thought the trach would be removed by the time he arrived. But his vocal folds were nearly paralyzed." With the trach in place, Kristen mapped out a plan. ❌Could he swallow at all? Barely. ❌Was he managing his secretions? Not so much. ❌With paralyzed vocal folds, was his coveted cough reliable? No. This was all confirmed by an MBSS. 🍦But this man wanted ice cream…and Kristen was determined to make that happen. 😀Imagine waking up to an email from a patient’s spouse that said, “...thank you for changing the face of stroke recovery.” We recently got to experience that rare kind of joy! A man named Rusty reached out to us to share his wife’s journey. 🏥On July 22, she walked through the door with classic stroke symptoms & was rushed to the hospital. The doctors found an infarct in the ventral tract of the pontomedullary junction - a bad sign for swallowing. Sure enough, Rusty shared that dysphagia was his wife’s most significant outcome.
