A contact ulcer or granuloma is primitive granulomatous tissue at the vocal process of the arytenoid cartilage arising from a defect in the mucosal lining. Patients with contact ulcer or contact granuloma may present with vocal fatigue accompanied with pain in the laryngeal area, dysphonia and frequent throat clearing. Contact ulcers and contact granulomas generally arise from the vocal process of the arytenoid cartilage. They may also be above the glottal level when arytenoid apices contact during phonation. They may be on one side or appear on both and be complementary in shape, such as a hammer and anvil. The size may range from a shallow bowl-shaped area to a large exophytic ball lying on the vocal fold and thereby impeding vocal fold vibrations. Voice therapy is helpful in case of habitually hyperfunctional voice use to regain a normal voice. Botulinum toxin injection with our without surgical excision is usullay the standard and most efficient treatment. He studied music at St John’s College, Cambridge, where he was a choral scholar, and went on to study medicine at Imperial College. As a singer himself, he has a particular interest in treating voice disorders in performers. Having worked for eight years in Birmingham as a consultant, he moved to Wexham Park Hospital in Berkshire in 2018. He has published a number of books and has written many chapters, including the chapter on “Larynx” for the 42nd edition of “Gray’s Anatomy”. He is President-Elect of the British Laryngological Association and is on the Presidential Council of the European Laryngological Society. Head and Neck Cancer Head and neck cancer is a definition used to express cancers in the following regions: Nose, paranasal sinuses, oral cavity, pharynx, larynx including the vocal folds, salivary glands . How Common Are Head and Neck Cancers? Head and neck cancers occur with an average frequency of 4% among all cancers. It is more likely to occur in men, those who use cigarettes and alcohol, and over the age of 50. The most common H&N cancer is squamous cell carcinoma. . What Causes Head and Neck Cancer? The use of cigarettes and other tobacco products is the primary risk factor. Another very important risk factor is alcohol consumption. More than 90% of squamous cell carcinomas, which are the most common tumors of the head and neck region, are associated with smoking and alcohol consumption. Excessive consumption of junk, salted foods is associated with cancer of the nasopharynx. Poor oral hygiene, missing teeth and recurrent oral infections can be associated with oral cancers. Today's case is a 60 year old woman who came to us with 4 years of progressive deepening and roughness of her voice. She'd often get mistaken for a man on the phone. She had smoked a pack per week of cigarettes for the past 40 years, but had recently quit and wanted to improve her voice. Voice evaluation revealed a rough vocal quality with a lowered modal register (deeper than normal voice). High resolution videostroboscopy revealed a classic example of Reinke's edema, also known as polypoid corditis or "smoker's polyps" (stay tuned for a future educational post for more info!). In this condition, exposure to inhaled tobacco smoke causes inflammatory changes to the vibratory layer of the vocal cords, making them look like loose water bags. In some cases, the polyps can get so big that they can actually obstruct the airway. She was treated with a combination of laser and surgical trimming of the excess tissue. Healing can take several months due to the extensive work required. Our patient had a great result and was thrilled with the improvement in her voice!
