Physical therapy is physical therapy. I got the same therapist I had 2 pt rounds ago for my shoulders, I like her. Very direct. I've always known I am not graceful, but I had no concept of how uncoordinated I am. We are treating low back pain by working on separating movements between my hips, pelvis, and legs, as well as separating movements between my pelvis and abs. I got 4 steroid injections in my lumbar spine this week and 4 in my shoulders last week to help me work through this (and for bday party this weekend). I am in a lot of pain after moving. There is a constant ache that throbs in my low back, sometimes I feel like I am being sliced, and occasional severe spasm in my right hip muscle area that brings me to the ground more often than I have been paying attention to. Haven't gotten much feedback on that hip other than it's not ovarian cysts bursting (except sometimes they are so that is real fun) and it is a soft tissue or muscle issue. We are doing isometrics for all the above mentioned sections of my body - correcting a leg length discrepancy, correcting a pelvic tilt (wow this is hard to work through) and emphasizing gait training (again). Lots of stretching. Some weights on a pulley for pelvis/ab isolation. Also more seated elliptical. I have been doing my home exercise program as prescribed daily, and doing additional work as recommended long term. I've been relying on meal replacements too much as food, but I am getting nutrients, proteins, collagen and extra l-glutamine. I am taking a Facebook break. 8 steroid shots over two weeks has my thoughts racing. Mechanical Low Back Pain and its Management Mechanic Low Back Pain is described as back pain caused by mechanical factors, such as spinal joints, discs, vertebrae, soft tissues, or even by incorrect mechanics of some other joint. The Lumbar is the commonest region for mechanical low back pain, as lumbar vertebrae absorb and distribute most of the body’s weight during static and active movements. Causes: sprain, strain, lumbar syndromes such as- disc herniation, vertebral compression fracture, lumbar spinal stenosis, spondylosis, and spondylolisthesis. knee deformity, and Also, ankle or foot deformity, etc. Symptoms: Lower back pain that may radiate to the buttocks and thigh, Spasm, and Also, The symptoms may exaggerate with bending forward (flexion of the back) and lifting heavy objects, etc. Physiotherapy Treatment: Therapeutic modalities used to relieve pain are combo therapy, laser therapy, extracorporeal shock wave therapy, TECAR therapy, In addition, thermal therapy (Infrared therapy, microwave therapy, and moist heat (hot packs, infrared therapy) are all examples of thermal therapy.). Chronic Low Back Pain (CLBP) Pain in the lower back that persists for three months or longer is classified as chronic. The exact cause of chronic low back pain is unknown. The lower back comprises five vertebrae (L1-L5). The lower back has an inward curve, commonly referred to as lordosis. It is the L5-S1 and L4-L5 segments that carry the most weight and have the most movement in the lumbar spine, which makes them more vulnerable to injury. Clinical Feature of Chronic Low Back Pain: Pain is usually felt in the lower back (lumbar and sacroiliac region), and patients with chronic low back pain have limited mobility. You may also experience pain in your lower extremities, or generalized pain can be present. In addition, patients with CLBP may experience impaired coordination and movements. Patients may have difficulty controlling their voluntary movements. Patients may have difficulty maintaining neutral posture, which can lead to body misalignment. It might also be painful to carry things with the arms or bend. The Big 3 Finale🎉🎉 Today we're finishing off our Big 3 series with the McGill curl up 💪 Picture 1: Start position 🔷️ Lay on your back with one leg bent and one leg straight. This will keep your pelvis neutral. 🔷️ Place your hands underneath your low back to support the natural curve of your lumbar spine. 🔷️ Brace your abdominal muscles by pulling your belly button towards your spine (or to the floor). Make sure you can breathe consistently while bracing. The Big 3 Continued! 👏👏 This week our physiotherapy team is taking you through "The Big 3" Matt is showing us how to perform the side bridge or plank. He is showing both a modified version (picture 1) and the full version (picture 2) 1️⃣ To begin, lay on your side on a bed or the floor and make a straight line with your body. 2️⃣ Then curl your feet behind you at the knees. 3️⃣ Place your elbow directly under your shoulder to prop up the top half of your torso. 4️⃣ Brace core by pulling belly button towards your spine and push elbow and knees into the table until your hips have lifted off of it. 5️⃣ Place opposite hand on hip or in crook of waist to keep chest up and pointing forward 6️⃣ Squeeze glutes and hold for 10-15 seconds while breathing consistently. 7️⃣ Return down to the table and relax your core muscles. Repeat 2-3 times a side. 🌟For the more advanced version, create the straight line with your body and then leave feet stacked ontop of one another OR place top foot infront or behind the bottom foot for added support 🌟 Always ensure you can breathe consistently while bracing your core. This is the first step to all core exercises! Low back issues⁉️⁉️ This week our physiotherapy team wants to educate you on 🎉 "The Big 3" 🎉 Dr Stuart M. McGill, a professor of spine biomechanics at the University of Waterloo, spent over 30 years researching how back pain occurs and how to best rehabilitate and prevent it. He created a grouping of 3 exercises to build endurance in the core and low back while avoiding positions of pain or stress on the spine. These three exercises are the bird dog, the side bridge (or plank), and the McGill curl up. This week Matt will be taking us through one exercise per day! First up, Bird Dog 👏 🔷️ Begin in a quadruped position with hands shoulder width apart and knees hip width apart as shown in the first picture. 🔷️ Tighten core by bracing inward towards the spine (belly button to the ceiling). This will keep your spine neutral throughout the exercise and help you maintain balance 🔷️ When ready, extend one of your legs backwards and the opposite arm forwards as shown in picture 2. Do not lift your leg or arm upwards and do not allow low back to arch or hips to twist. 🔷️ Pause briefly then return to start position. Repeat on the other side. 🔷️ Complete 3-5 repetitions per side for a total of 6-10 repetitions. Rest. Complete a second set. During the exercise, make sure to move slowly and maintain a steady breathing pattern throughout. 🔹TOPIC TUESDAY 🔹 🔹Non-specific Low back pain🔹 🌟Most common cause of back pain🌟 . 🔹Last week we covered what the different types of back pain there are and what non-specific low back pain is. . 🔹This week we look at symptoms of non-specific low back pain . . 🔹sometimes a pain will develop immediately from a heavy lift or after a twist but sometimes it can start for no reason!! Some people just wake up with it 😳. . 🔹it’s sometimes called simple back pain but this does not mean it’s mild! It can be extremely severe causing spasm leading to you walking bent over with your hands on your thighs and your butt in the air! 🙈. . 🔹typically it’s in one area of the back but can spread to one or both buttocks or thighs. . 🔹it’s usually eased by lying flat and often worse with coughing or sneezing. The images above depict how load further away from midline places greater stress (compressive force) on your low back. . While there is no such thing as a bad movement, there are movements that are more efficient than others when handling load (a weighted object) . A common mechanism of injury I see in office is reaching while attempting to pick up or lower an object . Examples: •Reaching and lifting something from the trunk of your car •Placing something into the oven •Bending forward while slowly lowering a child into a crib (I hear this one a lot!) . While these are all relatively safe movements, they may place more stress on the low back than what you are accustomed to- resulting in injury. . *Remember* all movements are good movements, but it’s the movements we are not prepared for (due to strength/mobility/stability demands) that typically result in injury. . An effective way to reduce the amount of compressive force on your low back is to keep the object you are moving closer to midline (base of support). . I see this all the time (and I used to be guilty of this too). A patient says they hurt their low back from lifting improperly, saw their Chiropractor a few times and once the pain was gone they stopped going. 4 months later and the same thing is happening again and the cycle continues. Pain is always the last thing to show up an the first thing to leave when you are trying to recover from an injury. Low back pain is the leading cause of disability world wide! Sacroiliac joint (SIJ) pain accounts for approximately 30% of lower back pain complaints. Pain can arise from mechanical origin or arthritic/inflammatory conditions. Pain is typically located on one side of the lower back and pelvis region that can refer pain into the buttock, groin or posterior thigh. Very rarely do we encounter pain traveling into our lower leg with SIJ related pain. Physical therapy and exercise have been shown to be effective for treating this condition. 🧭Low back/Lumbar mobility - These exercises help to mobilize or stretch the joints and segmental muscles in your lower back! It’s critical to move your back in ways that feel good in the presence of pain, because it allows your brain to normalize this movement which can help reduce pain. We’ve put together exercises that range from easy to more advanced, so ensure you feel comfortable with the movement before progressing. It has been said “Don’t take back pain lying down”, so here are a few ways to get yourself moving! Exercise 1: Cat/cows – a classic spinal mobility exercise but doing the exercise on your forearms can help bias your lumbar spine (low back) area. Arch your back up and hold at the top for 1-2 seconds and then slowly bring your back down arching it the opposite way. Keep the movement slow and controlled. Repeat 10 reps x 2-3 times. Exercise 2: Windshield wipers – begin by lying on your back and bring your heels toward your butt so your knees are bent up to a comfortable position. Gently move both knees to one side of your body to create rotation in your low back and then return to the starting position and repeat on the other side. It is important to do the exercise in a controlled manner. Repeat 10x (5 reps on each side) 2-3 times. Exercise 3: Stick assisted lumbar rotation and lumbar rotation/side flexion in half kneel – for this exercise grab anything that looks like a dowel that you can find around your house (i.e. broomstick, hockey stick, golf club). Bring one foot in front of the other in a half kneel position, think about elongating your spine and rotate one way using the stick propped on your shoulders as a way to assist you a bit further into the range. This should never be painful and only go as far as feels comfortable. You can take deep breaths in and out and move further into the rotation on the exhale. Another option is to add a side bend as you move into the rotation, think about bringing your rib cage away from your hip and feel a stretch along the side of your torso. Repeat each way 3x and then switch so your opposite leg is forward.

🧭 Core Exercises (Back Pain) - The McGill Big Three Core exercises are a common approach for physiotherapists to utilize in low back pain rehabilitation. These exercises work to “brace” the core, which acts to add stability to the low back. The core actually extends from the front, and around into the small stabilizing muscles of the back, as well. It’s important to contract throughout the entire core to simulate a corset. Exercise 1: McGill Curl Up - lie on your back on the floor with one leg bent up and the other straight. Place your palms underneath your low back and slowly raise your chest, head and shoulders off the ground using your abdominal muscles. Avoid flexing your back and keep in mind that you don’t have to come very far off the floor, slowly lower back down and return to your starting position. Exercise 2: Side Plank - lie on one side and have your legs stacked and extended. Ensure your head, shoulders and hips are all in line. Engage your core and lift your hips off the ground and hold your body in a straight line. Option: can also do a side plank from your knees to make it easier! Exercise 3: Bird Dog - starting in a 4 point kneel position engage your core and then slowly lift your opposite arm and leg off the floor, straightening both at the same time. You can extend your arm and leg as far as feels comfortable or as far as you are able to maintaining control. Avoid letting your hips sway/drop side to side by keeping your core tight and moving slowly. Return to the start position and repeat with the other arm and leg. Complete 30 seconds of each exercise, 3 times. Ensure that you are not comprising your form to achieve this dosage - it’s okay to work up to it, and it’s important these exercises do not increase your pain. Complete daily!

Hip hinge time ⏰🕰 ⁣ ⁣ While this movement seems simplistic, most people have a difficult time separating movement at the hip from movement at the spine. So let’s go back to the basics. Stability comes from the ability to control hip movement with spinal movement. Mobility comes from the ability to dissociate hip movement from spinal movement. ⁣ ⁣ Picture yourself and how you move throughout your day. Examples like… Getting in and out of a car, up and down from a chair, up and down from the toilet, bending over to brush your teeth, bending over to pick up your children, squatting at the gym, etc. The list goes on and on. Now this is not meant to scare you. It is just meant to open your eyes to the fact that HOW YOU MOVE MATTERS. ⁣ ⁣ I challenge you to think & act proactively now so you don’t have to be reactive later. Dr. Yaniv knows her stuff! DID YOU KNOW...that your lumbar spine (lower back) might have a DIRECTIONAL PREFERENCE??? Some of us have a flexion bias (our spine likes to bend forward, bottom pic) and some have an extension bias (we like to bend backwards, top pic). Your directional preference can manifest as pain with specific activities. For example, if you have a flexion bias, you may notice back pain with prolonged standing (like me!). And if you have an extension bias, you may find sitting to be an issue. Knowing your bias can help guide treatment such as physical therapy and yoga. It can also help you make adjustments and modifications to your daily activities. If you have frequent low back pain, consider an evaluation for your directional preference.
