Egg-retrieval technique / Ovum pick up (OPU) During this procedure, an ultrasound probe is inserted into the vagina to identify follicles. A needle is guided through the vagina and into the follicles. The eggs are removed from the follicles through the needle, which is connected to a suction device. Once eggs are retrieved from the ovaries, they can be fertilized in a lab. Happy to share my new scientific publication/ article on an interesting case wherein preterm birth due to cervical incompetence was prevented and NICU admission avoided by timely diagnosis & management. 👉Cervical incompetence is defined as the inability of the cervix to retain the fetus or maintain a pregnancy due to functional or structural defects in the cervix in the absence of uterine contractions or labor (painless cervical dilatation). 👉Cervical insufficiency is a part of a large and more complex spontaneous preterm birth syndrome. This generally happens in the second or early third trimester, depending upon the severity of cervical insufficiency. 👉This condition can be congenital or acquired. Defect during embryological development of Mullerian ducts, Ehlers Danlos syndrome or Marfan syndrome may cause deficiency in collagen leading to cervical insufficiency. 👉Amongst the acquired causes are cervical lacerations during previous childbirth, any surgery on the cervix like conization or forced cervical dilatation during the uterine evacuation in the first or second trimester of pregnancy or repeated dilatation and curettage or hysterosocpies. FAMILY means having someone to love you unconditionally in spite of you & your shortcomings. It means loving & supporting one another even when it's not easy to do so. I am so proud to know this beautiful couple and their strong family. This couple conceived after struggling for a while. All of us were so happy awaiting the arrival of our twins after 7 long years! However, God had other plans. He probably wanted to test their resilience and persistence. She had a tough pregnancy & antenatal period. There were repeated episodes of pre-term labour pains, bleeding and the constant fear of losing the precious twins. Unfortunately she went into labour and delivered twins in the early 7th month of pregnancy. We all were scared and worried about the tiny baby boys born at 870gm & 650gm. However this loving family stood strong with the support of each other. With the blessings of God and the hardworking NICU team both boys kept surviving one day at a time. They finally got to go home after spending 60 plus long and exhausting days at the NICU. I am totally mesmerized and touched with this family. The supportive couple & their equally hardworking & loving parents (grandparents of the boys). They spent months of sleepless nights taking care of the premature newborns. I am so happy and grateful to the Lord that the tiny babies have survived all odds and came to meet me today. They all traveled so many miles on such a hot & humid day. I am honoured and humbled. It melts my heart to see both the cuties smile. Success is not final, failure is not fatal It is the courage to continue that counts.... This young couple came to me after suffering 3 consecutive abortions. They conceived again but unfortunately had an Ectopic pregnancy (pregnancy in the fallopian tube). Luckily it was diagnosed in time and she did not need any surgery. The ectopic pregnancy was managed with medical therapy. By the grace of God and determination of this tenacious couple she conceived again. We all prayed for a healthy pregnancy. We are so grateful to the Almighty that He answered our prayers. She is blessed with a healthy baby boy. God bless! Alhamdulillah! Don't lose hope. Sometimes it's that one more effort that needs to be taken ! 👉What is recurrent abortion/ pregnancy loss ? Recurrent pregnancy loss is defined as having two or more miscarriages. 👉Why can I get pregnant but keep miscarrying? Recurrent pregnancy loss can be due to various causes: 📍 Abnormalities in the uterus, such as a uterine septum, fibroids or retained pregnancy tissue. 📍Cervical insufficiency/ incompetent os, which causes losses late in pregnancy eg between 3 to 6 months of pregnancy. 📍Thyroid conditions, diabetes or high levels of the hormone prolactin. 📍Genetic 📍 Unknown 👉What can be done for recurrent miscarriages? These should be offered tests to diagnose the reason for repeated abortions. The goal of emergency obstetric care at the health post level is to diagnose the problem, stabilize the woman, and arrange for transport to the nearest facility capable of managing and treating the complication. Monitoring of Fetal Heart Rate (FHR) during late stages of pregnancy by Non Stress Test (NST) and during labour by Continuous Tocograph (CTG) is an important tool for knowing fetal wellbeing. Normally, the baseline fetal heart rate (FHR) is variable. Variability is classified as follows: 👉 Absence of variability, if an amplitude change is undetectable; 👉 Minimal variability if amplitude range is ≤ 5 beats/min (beats/min); 👉 Moderate variability if amplitude range is 6-25 beats/min; 👉 Marked variability if amplitude range is > 25 beats/min. ♦️Variability may be decreased or lost with fetal hypoxemia or the placental transfer of drugs such as diazepam, magnesium sulfate, and most sedative. ♦️Prematurity, the sleep state, and fetal tachycardia may also diminish beat-to-beat variability. 👉An acceleration is an abrupt increase in FHR of ≥15 beats/min in ≥15 sec. 👉The presence of accelerations or moderate variability reliably predicts the absence of fetal metabolic acidemia and assures fetal wellbeing. A three-tier system has been developed for interpretation of FHR tracings: ❤️ Category I Tracings are normal & are strongly predictive of normal fetal acid-base status. ❤️ Category II Tracings are not predictive of abnormal fetal status, but there is insufficient evidence; further evaluation, surveillance & reevaluation are indicated. ❤️ Category III Tracings are abnormal & predictive of abnormal fetal acid-base status. What is MECONIUM/ MECONIUM STAINED AMNIOTIC FLUID (MSAF)? 👉🏾 Brown or green staining of the amniotic fluid indicates the passage of meconium. Because fetus swallows amniotic fluid in utero, meconium can be present in the infant's oropharynx at delivery. 👉🏾 Some of the factors that increases the risk of MSAF - advanced gestational age at delivery, prolonged rupture of membranes, intra-amniotic infection, pre-eclampsia, oligohydroamnios & diabetes mellitus 👉🏾 Babies who are stressed by low oxygen levels or infections may pass meconium before birth.
