Nothing puts the pressure on like treating family and friends; couple that with doing the wax up yourself and you’re in for a treat! The URC composite (in the upper right 3 position) was placed by myself about 5/6 years ago (presumably I was colour blind then), and the UR3 (in the upper right 4 position) had long been instanding and invisible from some angles. We decided to replace the composite on the URC with a lighter shade and improve the form to mirror the contralateral side, and place a new composite on the UR3 to resemble an UR4. Both composites were to be carried out using the injectable technique. I used exocad to mirror the anatomy from the UL3 and UL4 to create a more symmetrical smile. This is not as easy as it looks; wax-ups for injectable composite bonding must adhere to strict rules with regards to distance from gingivae, thickness, shape and more. Furthermore, when planning such cases, careful consideration is necessary to ensure even looking shades. In this case, as the URC was to be much thinner than the UR3, I used shade BW and B1 respectively (after reduction of the old composite). The result is a drastically improved smile from altering only 2 teeth. DIASTEMA CLOSING - TRANSPARENT SILICONE CHECK UP When closing diastemas, there is a slightly higher chance of unwanted uncontrolled excess of composite material in interproximal areas. To prevent this, the double model technique is a great tool as we will be injecting on space of one tooth while adjacent will have no empty space where flowable can flow to. But in such occasions it is also important that transparent silicone index impress all those tiny details, because if there is a small void in the index in this part, it will be filled with composite and cause uncontrolled excess. On the first photo you have a nicely impressed interproximal surface, while on the second one you have a partial void near papilar area. Great care needs to be taken when preparing a transparent silicone index and later on check it in detail to be prepared. SMALL BUBBLES IN TRANSPARENT SILICONE INDEX? If you tried to make a transparent silicone index based on printed models mabe you would notice formation of small bubbles on the surface of the index. It's because of the reaction between printing resin and silicone material. Product is the gas which forms those bubbles like from the foto. You can easily prevent those by placing silicone while setting in a high pressure pot on 2 bars. This won't improve time of polymerization but will squeeze out any air trapped in the silicone and your index will be crystal clear. The material of choice for this purpose is Exaclear from GC. To make things more difficult, some teeth were rotated and the patient declined any orthodontic treatment. This patient was referred to me by her GDP who had attempted multiple adhesive bridges to fill the gap in her smile. On examination we discovered the cause for the lost bridges: the occlusion (bite). After some discussion, we decided to do a fixed bridge off the previously prepared tooth that would be strong enough to withstand the bite issues. After further discussion, the patient didn’t like the fact that her canine was right next to her central, which made her smile asymmetrical. Unfortunately she outright declined any orthodontic work; we decided to disguise the peg shaped lateral and the missing lateral with composite bonding at the same time as the bridge. This lovely lady has been the centre of one of my favourite cases. She was unhappy with her peg shaped lateral incisors and the overall appearance of her smile. She declined whitening and wanted composite bonding that matched the rest of her teeth. Challenge accepted: First, we created a digital mock up based on her wishes and an initial chair-side smile design. We then tried this design onto her teeth to ensure that we were all happy with the end result. Lastly, using stents from @digitalprosthetics we placed 8 direct composite veneers and polished them to high heaven. Not all composite bonding is created equal, and the lovely gloss and soft tissue healing will ensure this will be a beautiful smile for years to come. TRANSPARENT SILICONE INDEX When preparing transparent silicone index for anterior teeth I use non perforated metal tray to impress my wax up with Exaclear, @gc.europe.nv To have sufficient space on all aspects of the wax up I make distal stopers from Exaflex putty to hold it in correct position. From my experience, I prefer to leave slightly thicker vestibular portion in transparent silicone (7mm or more) for stability and thinner incisal part around 3-5mm for simplicity as this part does not playba big role in stabilisation. THIN AND PRECISE MARGINS When planning your next case take a moment and double check your margins. Margin should be placed slightly supragingival and thin so they can nicely polish. That will prevent plaque accumulation, discoloration and biological problems such as inflammation. This will bring stability to your restorations as marginal points are the most crucial over time. TRANSPARENT SILICONE INDEX WITHOUT BUBBLES To avoid any bubbles formation in transparent silicone I advise you to place it in high preasure pot on 2 bars during time of polimerization. It will remove any bubbles (on surface but also from the material so you will end up with nice and clean transparent silicone. To have perfect visibility my favourite is Exaclear from @gc.europe.nv because it is the clearest you can find on the market with perfect precision and dimension stability. Material is polimerizing fast so you need to be fast in this phase. Alsonit is improtant to prepare space holders for ideal thickness of your transparent silicone index later on.
