Sometimes skinreconstructions can get frustrating. Things can go sideways even if you do everything by the books… This dog was presented after it had suffered a dog bite to the back and the skin necroses over a large surface. The rDVM can performed the initial wound care and sent the dog for reconstruction. After excision of the granulationbed I reconstructed the wound with an inguinal flank fold flap. Even after performing cultures pre- and intra-op, it became infected. Luckily the flap had a 100% survival. We managed the infection by local rinsing and bandaging and antibiotics based on susceptibility and sensitivity testing. So this tumor was removed with 3cm margins and in the depth this included the antebrachial fascia. The wound was closed with subdermal plexus flap, the axillary fold flap. These flaps do not have a direct cutaneous blood supply and rely on the subdermal plexus for it’s circulation. This dog had difficulties eating and was presented to her vet for a dental clean-up. The evt noticed a small lesion in the back of het mouth and biopsies this. It was a poorly differentiated sarcoma and so she was referred. On the CT the damage of nearly her whole left maxilla could be seen with extension of the tumor into the nasal cavity, periorbital space and zygomatic arch. A total unilateral maxillectomy with inferior orbitectomy was planned and executed. The dog did well and was eating within 48h after surgery. She exhibited no more pain when opening her mouth, as opposed to before her surgery. Unfortunately the tumor was not completely excised at the palate and there was a metastasis in the sentinel lymph node. Radiation therapy and chemotherapy were advised but the owner only wanted chemotherapy. 6 months post-op she again had signs of pain when opening her mouth and a recurrence of the tumor was noticed. Jack, a lovely viszla, was diagnosed with an intraoral tumor of his maxilla. It was a hi-Lo fibrosarcoma (histologically low-grade, biologically high-grade). A CT was oerformed to asses the extent of the tumor and thus the surgical margins. The thorax screened negative for metastasis. A CT lymphangiogram was done and the sentinel lymph node was the submandibular lymph node. I performed a unilateral maxillectomy. To reconstruct the defect a vestibular mucosal-submucosal flap was harvested from the remaining lip. Because a significant part of the lip mucosa was removed as part of the surgical margins, the face now has a concave appearance. The owner was informed of this prior to surgery. In the video you can see him 1 hour after surgery. He started eating the next morning and was released from the hospital that evening. The wounds healed well without complications and the dog was a very happy dog. Histology of the maxilla showed that it was indeed a hi-Lo fibrosarcoma and that there were no tumor cells left in any of the shaved soft tissue and bony borders. This German Shepherd of 9 years old had been admitted to an emergency service for acute abdominal pain and a fever. They took x-rays of his thorax and he was diagnosed with PPDH, or peritoneopericardial diaphragmatic hernia. This is a congenital anomaly in which there is an abnormal development of the diaphragm and an continuum is present between the peritoneal sac and abdomen. This results in the herniating of the abdominal organs into the pericardial sac. On the first photo is the pre-op X-ray in which the air-filled loops of intestines can be seen in the pericardial sac which causes an enlarged cardiac silhouette. This condition is usually present at birth, but animals can remain undiagnosed and asymptomatic for years. The herniated organs can cause an array of problems like respiratory distress from compression of the lungs; incarceration, strangulation or obstruction of the organs in the hernia; bloated organs may cause cardiac tamponade; right heart failure may even occur. Less common consequences are hepatic cysts, gall bladder torsion, portal hypertension and chylothorax. PPDH may be accompanied by other congenital wall or sternum defects. This dog also had a sternal defect. In the video you can see the retraction of all the intestines trapped in the pericardial space. I had to slightly open the diaphragm because the caecum and part of the colon were enlarged and trapped, and had adhered to the diaphragm. On the 3rd photo is the remaining defect in the diaphragm (you can see his heart in his chest). The diaphragm was easily closed and a temporary drain was placed between the sutures to remove the remaining air. On the last photo is the post-op X-ray in which you can see a normal cardiac silhouette again. Georgie ❤️. He sustained a serious bite wound on his back which was initially treated at an emergency clinic and sutured. The wound dehisced with a multiresistent bacteria and he was referred. We treated him with debridements and daily wound care and a tie-over bandage. After three weeks he had a healthy granulation bed and ready for the next step. The wound bed was sharply excised and a transpositieflap was created using ICG fluorescence to guide us to include main blood vessels. A patent ductus arteriosus or PDA is a birth defect of the heart. As a fetus there is an arterial shunt between the aorta and the pulmonary artery allowing the blood to bypass the lungs, during which this shunt (or ductus arteriosus) is ‘patent’. After birth this blood vessel closes within the first few days since blood is being pumped through the lungs to be oxygenated. Sometimes this vessel doesn’t close and stays patent, and thus blood will flow from the high-pressure left side of the heart to the low-pressure right side of the heart. Less blood is pumped through the left side with every heartbeat and the left side will have to work harder and become larger leading to left-sided ventricular hypertrophy. If the defect is large, more blood will go to the right side, which will lead to right ventricular hypertrophy and pulmonary hypertension, which may ultimately even cause the direction of the shunt to reverse. The shunt will cause a continuous murmur which can be heard and you can even feel it when you hold your hand to the chest of these animals. The shunt can be treated with cardiac catheterisation with placement of a ductal occluder device or transarterial coil. If the patient is very small open surgery will be done. Subtotal pericardiectomy in a Shih-Tzu. The pericardial space is filled with fluid (usually bloody). This will cause tamponade of the heart, which causes it to not pump the blood effectively through the body. During an emergency procedure the pericardium is tapped and emptied under ultrasound guidance. If it recurs we will usually remove the pericardium (subtotally): the pericardium is removed below the phrenic nerves that cross the heart. This will relieve the pressure on the heart. This cat was presented with a large ulcerated mass above its right eye which bothered the cat very much. With closer examination we also noticed an ulcerated mass on its nasal planum. Screening (radiographs and CT) was done and biopsies were taken. Both lesions were squamous cell carcinoma (SCC). Exposure to ultraviolet rays/sunlight has been attributed to the development of SCC in cats. White and light-colored cats are more prone to development of these tumors. SCC tends to arise in sparsely-haired and lightly-pigmented areas of the skin, particularly the eyelids, top edge of the nose (nasal planum), lips, ears, and other areas that have greater exposure to ultraviolet rays/sunlight. The mass on its eyelid was excised which included an exenteration of its eye and zygomatic arch. It was reconstructed with an angularis oris axial pattern flap. The lesion on the nose was excised and the nasal planum was reconstructed with a lip-to-nose flap. CT lymphangiogram had shown the right parotid lymph node to be the sentinel, so this was also excised as part of the treatment. Near-infrared fluorescence guided sentinel lymph node excision… a mouth full 🙃. We inject the area immediately around a tumour with a fluorescent dye, indocyanine green. This is then transported through the lymph vessels to the lymph node draining the tumour and it’s surroundings. We call this lymph node the ‘sentinel’. Removal of the lymph node is not only of important diagnostic value but also provides a way of removing potential metastases. Sometimes these nodes are very small and difficult to find or in a surgically challenging place, like this one in this dog: the axillary lymph node. It is buried deep in fatty tissue, under the pectoral muscles adjacent to the vessels and nerves to the front leg. Now this is a weird one to start off with… This little dog was a rescue and was used previously as a breeding dog. She had purulent vaginal discharge and the vet had done a ovariohysterectomy for a suspected pyometra. Unfortunately the discharge didn’t stop and she was referred. On digital examination of her vagina we felt hard and pointy objects. So we examined this with a speculum and removed different foreign bodies. On closer inspection we could see part of a mandible and a tooth. Turns out the head of her last pup had remained in her vagina after giving birth and completely decayed!
