We are at the point in the legislative session when the Senate and House are negotiating on what gets included in the HHS omnibus finance bill. Because the MN Building Families Act has costs associated with it, our bill needs to be included in the final HHS bill. Unfortunately, it was not included in either chamber’s original bill but can still be added as an amendment. Our bill champions are continuing to work behind the scenes to make this happen before the session ends on May 20. Meanwhile, we know that access to IVF remains on the minds of lawmakers, thanks to all the advocates who met with their lawmakers on MN Advocacy Day in February and the wall-to-wall media coverage generated by the press conference that same day. The drumbeat continued with last month’s Infertility Justice Rally and press conference, featuring patient advocate Jamie Heard from Alabama, who helped lawmakers understand the difference between IVF being legal and being accessible and that without insurance coverage, most people cannot access the medical treatments they need to build their families. Today is International Donor Conception Awareness Day. This day was created to educate recipient parents, fertility clinics, and the general community. Compared to the days where parents were told by their health care teams to not disclose to their child that they were donor-conceived, we have come a long way in hearing the voices of donor conceived adults and incorporating them into practice. As the number of patients using donors increases, it becomes even more important to learn about the best practices of donor conception. As the saying goes “once we know better, we need to do better.” Here are some keys points within the best practices: ⭐️People have a right to information about their genetics ⭐️ Tell your child they are donor-conceived from the onset of their lives so it is something they have always known about themselves ⭐️ Secrets always come out eventually. Secrets almost always lead to feelings of shame or resentment. ⭐️A donor conceived child’s interest in knowing the donor does NOT mean they love their parents any less ⭐️A child’s interest in being donor-conceived may change throughout their lives. A parent’s job is to be there for them and take their lead through these transitions. ⭐️ Sharing a donor conceived child’s origin story with them is not a one time conversation. Incorporate simple comments into everyday conversation (“you are great at soccer just like our donor”), buy age-appropriate donor conception books for your child, and make it clear they can always come to you with questions. ⭐️ DNA alone doesn’t make a family but your family may still be interested in their DNA What else would you add to this list? La gestazione per altri è un metodo di riproduzione medicalmente assistita che esiste dal 1985, in cui si ricorre a una donna che non è la futura madre per portare avanti la gestazione del bambino. Perché diversi termini per riferirsi a questa pratica? E perché alcuni sono più corretti di altri? Ci sono diversi termini in circolazione per riferirsi a questa pratica: gravidanza per altri, gestazione per altri, ma anche maternità surrogata, utero in affitto, riproduzione per conto terzi. In generale una buona regola nella comunicazione della scienza è che bisognerebbe sempre cercare il termine più neutro. In inglese la pratica è nota come “riproduzione per conto terzi”. In italiano questo termine non è molto diffuso. ‘Surrogata’ è un termine che è ancora in voga in inglese sebbene sia usato sempre meno perché rimanda al fatto che la gravidanza sia una specie di gravidanza ma non proprio, qualcosa di meno dell’originale, ma non è così dal punto di vista biologico: si tratta di una gravidanza a tutti gli effetti. ‘Utero in affitto’ è un termine errato dal punto di vista scientifico perché rimanda a una rappresentazione erronea della gravidanza, in cui il feto è contenuto dentro la gestante (erroneamente chiamata “madre”). Si parla qui di modello del contenitore fetale. Questo modello ampiamente diffuso nella cultura della gravidanza non rispecchia la biologia della gravidanza dove il feto non è semplicemente contenuto, ma il feto è parte di un tutto, il corpo della gestante, con cui è collegato. Il feto non è un’entità separata dalla gestante che si può affittare. Questa rappresentazione riduce la persona che gesta la gravidanza a un semplice incubatore. Quindi i termini più corretti perché “neutri” sono gravidanza per altri, gestazione per altri, oppure riproduzione per conto terzi. Donor 👏🏼 conception 👏🏼 is 👏🏼 an 👏🏼 ethical 👏🏼 minefield. I talk to clients and friends about this...a lot. Like maybe daily. The fact is, most people considering growing their family through donor conception start their learning about it is at a fertility clinic or online at a sperm/egg bank (because, hey, that's what pops up on Google first!) Some never get beyond there until after they've made or transferred embryos/done their IUI (it sounds like you're getting a ton of information from reputable sources, so why keep looking?). Some dive a bit further into the internet and discover the big, wide world of donor-conceived advocates and their allies fighting for regulations, more known donors, verified medical histories, family/sibling caps, fertility fraud legislation, etc. And then they (understandably) get very scared. Some will decide to try and forget about it, hoping their child just won't feel this way. Others will pause and think about the ramifications and look at what they can do to possibly resolve and mitigate certain risks for their future child's health, happiness, and overall well-being...which is not easy with many of the options available. Discussing it online can be difficult - understandably big, conflicting opinions and big, conflicting emotions.
