North American Rescue (NAR) posted this first picture today, showing an example of a counterfeit tourniquet which resembles the ubiquitous CAT-7 model. Sadly, a lot of well-meaning supply techs and purchasing agents have been duped into getting these fakes from internet dealers who entice budget-conscious buyers with lower prices. As the saying goes, the devil is in the details. The other two pictures are from my own CAT-7 and you can clearly see the difference. If you’re in the field of procurement and logistics as I am, then you understand the seriousness of obtaining life-saving gear for those in the field. Obtaining fake tourniquets can literally cost people their lives. Know your vendors. Know the products. This isn’t to throw shade on third-party vendors, but it’s a necessary practice. Trust but verify. I did a bulk order earlier this fiscal year and got them directly from NAR. No doubt that’s the best practice. Keep your eyes open. In the UK we have a bizarre situation where First Aiders are told to leave entrapped injuries if they have been trapped for more than 15 minutes due to the build-up of toxins, yet we are often told to leave tourniquets in place. The build-up of toxins - or rhabdomyolysis - is a complex pathophysiological pathway which does occur....but not in minutes, but hours. Tourniquets 'High & Tight'? This edict has not been promoted in a civilian setting for nearly 10 years where current recommendations are that the tourniquet should be applied a hands width / couple of inches above the injury, with the intention of preserving as much viable tissue as possible. In a military setting, tourniquets may be applied hastily 'high and tight' in the Care Under Fire phase where the situation does not allow for a considered tourniquet application. What is often neglected is that under the Tactical Field Care phase, the tourniquet should be 'converted' or repositioned to just above the point of injury. These two photos from the Ukranian frontline show the consequence of a hastily applied tourniquet 'high & tight' that has been left in situ for 4 days. The soldier allegedly survived but the leg is not salvageable and requires amputation at the groin. Not due to the tourniquet per se, rather the unnecessary length of time it was in place high & tight. 1️⃣ Visualise the whole limb to assess for additional bleeding wounds. 2️⃣ Place the new, distal, tourniquet 5 cm from the highest wound. 3️⃣ Apply the tourniquet tightly. 4️⃣ Release the proximal tourniquet, observing the wound. If bleeding continues, tighten the distal tourniquet until bleeding stops. 5️⃣ Continue to release the proximal tourniquet, observing the wound for bleeding. In a civilian setting, the tourniquet should NOT be converted if: ❗ The transit time to definitive care is less than one hour. ❗ The casualty has other life-threatening injuries. ❗ The casualty has unstable vital signs. Using the same protocols, a tourniquet can be downgraded to a pressure bandage over the injury, removing the tourniquet altogether. We are in full loom band era. Friendship bracelets are coming home every day and I’m finding these little elastics in the wash, in shirt pockets and the bottom of school bags. I’ve even found some snuck into the middle of school readers which also end up in bed! There is so much to love about a humble elastic (and yes I have read and aware of the carcinogenic fear around them- my kids don’t eat them and they wash their hands). But they do promote a sense of play that is relaxing, screams regulation and creates unlikely friendships blossoming. So I won’t be ditching them. But just like hair tourniquets, these little elastics are small enough to create a tight band but large enough to fit over small wrists and fingers. Yesterday Elsie came to me saying “ouchie ouchie”- a phrase I never ever ignore. If I can’t see anything immediately I strip the kids off and check them over. As I was pulling her top off I noticed her very purple and swollen hands. To my dismay she had loom bands wrapped around both wrists. I don’t know where she found them but she did. She is ok but it had the potential to be far worse. A very good lesson. So my top tips for these little elastics: ✔️ know where they are stashed and Create safe play zones - no bed, car or communal play spaces (treat them like LEGO) ✔️ Check fingers before you go to bed (in case they are being played with instead of readers being read 🙃) ✔️ If little ones are in pain, expose their little bodies and check all over- including genitals. Tourniquets are commonly used intraoperatively in orthopaedic surgery to control bleeding and improve visibility in the surgical field. Recent evidence has thrown into question the routine use of tourniquets in the adult population resulting in a British Orthopaedic Association standard for intraoperative use. This systematic review evaluates the evidence on the practice, benefits, and risks of the intraoperative use of tourniquets for trauma and elective orthopaedic surgery in the paediatric population. A prospectively registered systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines (PROSPERO: CRD42022359048). A search of MEDLINE, Embase, the Cochrane Library and a Grey literature search was performed from their earliest record to 23 March 2023. [...] Thirty-nine studies were included. Tourniquet practices and information reporting varied considerably. Tourniquets were used uneventfully in the majority of patients with no specific benefits reported. Several physiological and biochemical changes as well as complications including nerve injury, compartment syndrome, skin burns, thrombosis, post-operative limb swelling, and pain were reported. Tourniquets are routinely used in both trauma and elective paediatric orthopaedic surgery with no high-quality research affirming benefits. Severe complications associated with their use are rare but do occur.
