This sweet little nugget came into the world sunny side up and took their sweet time doing so! Sources vary, but only about 2-10% of babies are occiput posterior aka OP aka sunny side up at birth. It is more common for babies to come out facing your rear side than your front side. It is simply easier for them to navigate through the pelvis that way, but some kiddos decide to stand out from the crowd. Persistent OP can be a cause for prolonged and obstructed labors, but sometimes labor progress is perfectly average or even fast and baby surprises us by coming out looking right up towards mom’s face! Shannon got the pleasure of helping this family meet their new babe, along with OMG doula Bridget. Did you know that our affiliated hospital, UChicago Medicine AdventHealth Hinsdale has this amazing support program called Still Missed? For families who have experienced miscarriage, ectopic pregnancy, stillbirth or newborn death, the Still Missed program offers parent support groups, grandparent support groups, Peer Parent Support, Subsequent Pregnancy After Loss Support Group, and so much more. Every Spring, a special garden planting and memorial service is held and the Memorial Garden on the 2nd floor of the hospital is available year round. Each Fall, another memorial service is held at a local Hinsdale church near the hospital. Lucille Virginia Pollard (Lucy, Lulu, Ginny Lou, thus far) born 1/17/2024 at 10:48 am. 7 lbs 5 oz 19 inches. I labored at home until dawn, and as we arrived at the birthing center with @omgmidwives there was a brilliant sunrise rich with reds and purples. @markryanpollard was steady, strong and loving, my doula Christa was a dream, and I felt I was standing on the shoulders of generations of laboring women and the supporters of the truly awesome experience of birth. It is so pedestrian and yet powerful. It is humbling. This little segment of cord has a big story! This active baby got their cord tied in a true knot at some point during pregnancy. Estimates vary, but most data seems to agree that this phenomenon is only found in about 0.3-1.5% of births. In some cases, it is suspected prenatally on an ultrasound and other times it is discovered at birth. A true knot like this sometimes causes no problem at all and other times can decrease blood flow to the baby in utero causing heart rate abnormalities or even still birth. The owner of this cord showed us several signs of distress during labor, including meconium stained amniotic fluid and decelerations in their heart rate. By themselves, these two findings sometimes happen and don’t get us too up in arms, just a little extra alert for a baby that may or may not be feeling stressed. The owner of this cord was born pretty quickly, but did get stuck with a shoulder dystocia on the way out too. While our midwives favor delayed cord clamping until drained, this babe needed a little help after the extra stressors surrounding their birth, so after only a 30 second delay, the cord was clamped and cut. This kiddo had good reserves and quickly perked up and was back with mom in just a couple short minutes! But due to the initial concern when they came out, this segment of cord was clamped and set aside to collect cord gases, which can give an indication of baby’s oxygenation leading up to birth and help guide the treatment team in cases where baby continues to show signs of distress after birth. The parents and birth team were amazed to find this knot and guessed that this may have contributed to the stress baby displayed on their way out. Thank goodness for all that Wharton’s jelly cushioning this cord and helping to balance that compression in the knot out to keep the blood flowing! Swipe for the surprise 😍 Yesterday I was back up for a friend who was out of town while a Mama had to be unexpectedly induced for medical reasons. This Mom’s strength and stamina was absolutely AMAZING and left us all speechless. Talk about a bad ass birther! 😍😍 She birthed her baby in the way that felt most supportive for her. She birthed her baby standing up next to the bed + her birth team went right along with it! She listened to her baby and her body. Her husband was such a sweet + calm support. Our midwives got to let their hair down, take the scrubs off and get a little fancy tonight for the UChicago Medicine AdventHealth Hospitals medical staff holiday party! Some of our back OBs were there too (in addition to physicians, APNs and PAs from lots of other departments across our hospital system. The band was awesome with crowd participation… Megan got caught checking her work calendar on her phone by one of the talented vocalists, and quickly got put on the spot for a little “duet” with her! 🤣 See a new face you don’t recognize in the Happy Holidays group photos? In the beautiful sky blue sweater is Shannon, the newest addition to our team. She joined us in the office about two weeks ago and will be joining us soon in the hospital too! We always love to share a fascinating placenta! This family allowed us to share their placenta and cord with you. This umbilical cord was so long that our team just couldn’t resist measuring it! 📏 This cord measured a total of 36in. or about 92cm! It was also found to be tied in a true knot at delivery. This baby was healthy and showed no signs of distress during labor or at birth. Did you know? ✅ The average umbilical cord is usually about 50-60cm or about 19-24 inches long. ✅ Reports vary, but it is believed that the longest recorded umbilical cord length may be as much at 191cm or 75 inches! ✅ Some studies have shown an association between longer cords and increased cord complications, such as a cord around the baby’s neck, a true knot in the cord or fetal heart rate abnormalities in labor. ✅ Somewhere between 10-30% of babies are born with their cord wrapped around their neck. This may sound scary, but often is not a serious complication at all. ✅ Babies do NOT feel it when their cord is cut. Don’t be afraid to hurt your newborn when the time comes to help free them from their magical cord, luckily it has no nerve endings. ✅ In a normal birth, the cord will typically continue to pulse for about 3-20 minutes after the baby is born. The cord naturally clamps down and stops pulsing when it is ready, though a tie or clamp at the end is recommended when cutting the cord because the tissue in the cord doesn’t fully tighten down when the pulsing stops and we want baby to keep all the valuable blood they just received. ✅ Delayed cord clamping and cord blood banking can both happen at the same birth! It requires a bit of balance, but it very possible. 💧 HYDROTHERAPY UPDATE 💧 We are so happy to announce, that after lots of hard work from our team, the hospital hydrotherapy policy has expanded. What does this mean? 🛀 VBACs are allowed to labor in the tub! 🛀 Prolonged rupture of membranes is not an automatic “disqualification” for tub use, as long as you remain free from signs of infection. 🛀 The BMI limit has been increased from 37-40. (Sorry to our clients of size with a BMI over 40… this has been an uphill battle. The shower is still a great hydrotherapy option which has no BMI limit set by the hospital.) 🛀 Meconium stained amniotic fluid is not an automatic “disqualification” for tub use, as long as the fetal heart rate tracing is considered normal or category 1. 🛀 People who are receiving Pitocin for induction or augmentation of their labor can be in the tub! 🤔 What does this not mean? 🤔 Unfortunately, birth is still not allowed in the tub. Hydrotherapy tubs can only be used for labor and not during the pushing phase of labor. ****Big News**** So many of you have been waiting for this announcement. Beginning April 1, 2023 face masks are no longer required routinely at Hinsdale Hospital (and all other AdventHealth offices and hospitals in the region) for people who are showing no signs of illness. That’s right, we get to see your whole face, if you are ready to share it! 😁 If you have a fever, cough, or other symptoms of illness you will still be asked to wear a mask. 😷 If you feel more comfortable wearing one at any time, we will continue to try and make them available but encourage you to bring a mask with you. Please don’t take offense of many of our staff, nurses, midwives or doctors continue to mask while caring for you. We come in close contact with a high volume of people each day and masking is how many in the healthcare field continue to feel they are caring best for themselves, their families, and the clients who depend on them. 🦠 If you encounter a team member who is not masked and you prefer they wear a mask while participating in your care, please don’t hesitate to say so. #omgmidwives #covid_19

If you were trying to access www.omgwh.com over the past couple months, you may have noticed that sometimes it will not load. We have been having technological issues that were resolved but now over the past few days have returned with a vengeance. We are investigating this to see if it can be resolved, but we are also considering just letting go of the website. It has been a wonderful tool for well over a decade, but in the age of social media, we aren’t sure it’s worth the enormous task of maintaining it. We do not have a professional team who handles that, just us midwives. Yep, you heard that correct, in all our spare time between doing your Pap smears, catching your babies, helping you feed your babies, troubleshooting your bladder infection or your menopause symptoms and everything in between, we have also been building and maintaining a website and our social media pages. A study of children born to mothers who took ADHD medications during or surrounding pregnancy was performed at Icahn and the National Centre for Register-Based Research at Aarhus University in Denmark. Researchers followed children born between 1998 and 2015 from birth through 2018 to compile data. The study included over 1 million children including control samples whose gestational parent/mother did not take any ADHD medication, children of those who took medications prior to pregnancy but discontinued them during pregnancy, and those whose gestational parent/mother continued ADHD medications throughout pregnancy. No significant difference was found between groups of children in the rates of neurodevelopmental disorders including autism and ADHD, impaired vision or hearing, epilepsy, febrile seizures or growth retardation/restriction. Previously, animal studies seemed to show a connection and risk. While more research is needed, this is a reassuring finding for pregnant people who rely on ADHD medications to function in their daily life. When you come in to see us for routine prenatal care (or we’ll person care) and we ask if you feel safe at home, if anyone is hurting you, if you have a past history of abuse or related questions, remember that we do this for a reason. Even if “we know you and your partner” and this is your 4th baby with us or 15th annual exam with us. Even if they are with you and they “look like a nice person” we ask these questions for a reason. And you may not always be ready to talk about it that exact moment, but we hope that by opening up the conversation, those who need the help will find a way to let us know. If you or someone you know are in a dangerous situation, please get help. Sometimes that help is to leave immediately. Sometimes that help is to have a safety plan if things escalate. Sometimes that help is to initiate counseling and supportive community services. Just know, there is help. October 15th is Pregnancy and Infant Loss Remembrance Day. Tonight, we light a candle for all the families remembering their missing children. 🕯 Whether from SIDS, neonatal loss, ectopic pregnancy, chemical pregnancy, molar pregnancy, TFMR, miscarriage, stillbirth, vanishing twin, or anything in between and across multiple categories, loss is loss at any stage. Whether it was unplanned, or highly planned and orchestrated using medicine and technology; a first pregnancy or a 10th pregnancy; whether you have had a 🌈 baby since then or not; whether you felt a bit of relief for any reason when you got the news; whether 30 years ago or just yesterday, your pregnancy matters. Your feelings, whatever they may be (past, present, and future) are valid. As your midwives, we celebrate the highest of highs with the families we are blessed to care for. But please know that we feel the lows with you. We remember with you. What was and what could have been. This year’s theme for women’s history month is and the national theme is “Providing Healing, Promoting Hope.” So we’d like to take some time throughout this month to recognize some women who have exactly that. Today’s inspirational woman is Susie King Taylor, who was born into slavery but managed to attend school secretly and learn to read and write anyway. Taylor managed to escape to freedom and volunteered for the Army’s first black regiment. She became the first black army nurse. During the Civil War, when she wasn’t performing her typical nursing duties for wounded or ill soldiers, she was teaching them to read! She is the only black woman who went on to publish an account of her life during the Civil War. Her book, Reminiscences of My Life in Camp with the 33rd United States Colored Troops, Late 1st S.C. Volunteers speaks extensively of her life in the military. She recounts how she worked with no pay for four years and three months and most of the black regiments received no pay for at least 18 months. She recounted blatant racism that she and others faced, despite fighting in the war and making sacrifices for their country, including being denied access to a sleeper berth to bring her dying son home simply due to her skin color. Taylor opened at least 3 schools for black students in Georgia and was the first black woman to teach in a school which catered to the education of former slaves. #omgmidwives #womenshistorymonth

Today’s inspirational woman is Sir Lady Java. As a trans woman, she helped fight against LA’s “Rule No. 9” which criminalized cross dressing. She joined with the ACLU to fight against this rule when she was fired from her job as a waitress and performer at the Redd Foxx Club due to pressure from the LAPD. She argued that Rule No. 9 was unconstitutional and infringed on her ability to make a living. While she didn’t win her fight against this discriminatory rule, she gained national attention. This year’s theme for women’s history month is and the national theme is “Providing Healing, Promoting Hope.” So we’d like to take some time throughout this month to recognize some women who have exactly that. Today’s inspirational woman is Gerty Cori. In 1947 she became the first woman to win the Nobel prize in physiology or medicine (and only the third to receive the Nobel prize in science.) Gerty was born in Russia and emigrated to the US after she received her doctorate in medicine. She and her husband Carl conducted research together at Washington University in St. Louis. She had to fight to work in the male-dominated field and was always paid a fraction of what her husband and other male colleagues were paid. COVID-19 continues to change our daily life and challenge us, but babies are still being born and our clients are still as strong and amazing as ever. COVID-19 has forced pregnant folks to make impossible decisions such as this mother had to make: choosing between her partner or her doula for labor support. She choose her incredible doula to be by her side physically. Despite every effort of strength and perseverance, her baby decided to be born by cesarean. Her partner listened to his child’s first cries with her through speaker phone, as her doula held her hand. He said sweet things for the whole room to hear, while their first child also listened in through the phone. Baby was brought skin to skin in the OR, and this family shared a quiet moment together even though they were miles apart. COVID-19 has changed everything, but it cannot make birth less powerful or less beautiful. Life will continue. Out of all 4 kids, this birth was the only birth where I was able to have immediate skin to skin with baby! When our old obgyn retired, we weren't sure we would find another that we liked ... But we were so wrong! @omgmidwives was the absolute best choice for us!!! This moment right here says it all! It may have been a very tiring, long labor and I, maybe, didn't handle it so well but the midwives never gave up on me! All 3 of the boys births were great but not one of them did I get to have this moment. They were all whisked away to do all the weighing and testing for babies. But this time, Greta was put right on my chest as soon as she came out and we bonded until I was ok with her getting her weight and such done. I cannot thank my midwives enough for this moment right here! It was the one thing I've dreamed about for years now and I feel so very blessed for this moment! My son is here! I cannot believe it. So full of emotions and good feelings. I’m exhausted as hell of course. I’m in awe of him. I’m in awe of Leighton’s reaction to him. She is gentle, so loving and helpful. Josh is of course my support, my partner, the one who is always by my side. He’s the best daddy. Going into this pregnancy I desired to have a vbac. I wanted so badly to experience my baby to my chest after all that hard work of labor/delivery. I wanted to get through the trauma of Leighton’s delivery and heal. I did just that and I’m honestly so proud. I feel so strong. I’ve never felt so good in my own body despite all the craziness this pregnancy has brought. I have birthed two babies two different ways with this body and I am proud of that. I have to say I am so beyond grateful for a supportive care team. I was so thrilled to have found a group of midwives with a supportive OB that really let me be the one in control of my journey. Special shoutout to Kelly & Sarah at @omgmidwives . I don’t know how I could have gotten through laboring and pushing without either of them. There were so many times I wanted to give up because I just couldn’t handle the exhaustion and the pain anymore. But they really helped me push through. I never in a million years thought I could love someone else as much as I love Soleil. On 3.13, I woke up at 2 am with contractions. I napped, walked, and labored at home until being convinced we should probably start making the hour drive to the hospital. I was sure they were going to send me home to labor some more but I was already 6cm dilated! I got right into the water tub and Halo entered our world! 2019 STATS ARE HERE! This was a wonderful year for the OMG midwifery team! Some of our accomplishments include: •Moved into new office space to reduce wait times and allow for practice growth! •Midwife team grew to 5 midwives with the addition of Jaci Noto, CNM! •Facilitated changes to the Hinsdale Hospital hydrotherapy policy to make criteria less restrictive and more accessible to our clients! •The entire midwifery team trained in ultrasound! •Maintained exceptional outcomes for our clients despite the loss of providers and gaining new physician coverage at the end of 2018! •Caring for so many amazing individuals and their families! Family-Centered Care: ⠀ ⠀ Midwifery care often boasts about a philosophy centered on the client and their family. This seems like a basic requirement when caring for a pregnant client, but the traditional medical model of care tends to focuses on traditional practices and provider preference rather than client preference. At OMGWH, we take family-centered care seriously, hoping it influences nearly every interaction we have with clients. Support from Partners in Labor⠀ There are many ways partners can provide support to a laboring person. The type of support the laboring individual desires may differ based on personality and sometimes even with different births. Here are some examples of support we have seen be helpful and powerful for our laboring clients ⠀ ⠀ • Physical Support: loving touch, hand-holding, hugging, massage, playing with hair, kisses on lips, forehead, hands. Counter pressure, hip squeezes, or physically holding the laboring person up ⠀ ⠀ • Presence: Attention is focused on laboring person and not distracted with the phone, TV or other people in the room. Eye contact can be grounding/centering and provide a feeling of not being alone. Sleeping and seperation is sometimes needed during long births, but overall separation should be minimized. ⠀ ⠀ • Verbal support: Soft words of encouragement, love, kindness and compliments. Verbal support can also include partners sharing private jokes, memories, hopes, and wishes with the laboring person. Using their voice as a tool for relaxation & guided medication. ⠀ ⠀ •Advocacy: Partners should know the laboring person's values and birth wishes better than anyone. They can be a strong voice in advocating to the medical team and any other family.
