White coat syndrome, or white coat hypertension, is the term for when you get a high blood pressure reading in a doctor's office and a normal reading at home. The anxiety of being around doctors in white coats can make your blood pressure rise. Most people don't need treatment if their blood pressure is normal at home. This is why checking your blood pressure more often, at home, when you are relaxed is helpful in better understanding your blood pressure. Remember a blood pressure test is a ‘snap shot’ in time, which could be influenced by your stress levels that day, what you have eaten and many other factors. Continuing to test your blood pressure over a few days or weeks helps to provide more accurate results as it gives you a view of your blood pressure over time. 🔋🧠 Neurished News 🧠🔋 Title: Intravenous Thrombolysis in Patients With White Matter Hyperintensities in the WAKE-UP Trial Summary: A recent study published in Stroke Journal (PMID: 37226772 DOI: 10.1161/STROKEAHA.122.040247) investigated the impact of white matter hyperintensities (WMH) on the efficacy and safety of intravenous thrombolysis in patients with ischemic stroke of unknown onset. The study found that although WMH burden is associated with worse functional outcomes, it does not affect the treatment effect or safety of thrombolysis. Additionally, in patients with severe WMH, thrombolysis was shown to increase the odds of an excellent outcome without a significant increase in the rate of hemorrhagic transformation. New paper out! The OCS-Plus is designed to be a sensitive measure to detect domain-general cognitive changes, using a tablet-based app. This means the test is mainly focused on executive function and memory meausures. Given previous findings that this is an effective tool for sensitive detection of subtle cognitive problems, it is important for the tool to be directly compared with current standard clinical assessments. This research aimed to determine the sensitivity and clinical validity of the OCS-Plus, a stroke-specific tablet-based cognitive screening tool, in comparison with the MoCA, a routinely used screening tool, after stroke. Eighty-six patients from the Oxfordshire stroke rehabilitation unit completed both screens: the OCS-Plus on a tablet and the MoCa using pen and paper. Main takeaways: (1) OCS-plus is a highly sensitive cognitive screen (picking up all those impaired on MoCA + more mild impairments) - see also previous paper by Webb et al 2022 comparing OCS-plus to neuropsychological assessment battery. And yes, most of the people we tested yesterday were young but that does not mean they can't have high blood pressure, in fact there were a few who did! Because you can have high blood pressure even if you are fit and well and young!! So the only way to know, is to check it! Because then you can manage it rather than running the risk of that high blood pressure creating major problems in your life. Ultimately it's all about 'Preventing the Preventable'. It can be quite confusing can't it? You are told to check your blood pressure? But great...what are you checking? What are the numbers you are looking at? When you take your blood pressure there are 2 numbers because the blood pressure varies with the heartbeat. e.g. 120/80 The top number, the higher pressure, represents the pressure in the arteries when the heart beats, pumping blood into the arteries: this is called systolic pressure. The bottom number, the lower pressure represents the pressure in the arteries when the heart is relaxed between beats. This pressure is called diastolic pressure. Most doctors agree that the ideal blood pressure for a physically healthy person is around 120/80mmHg. If your reading is higher than this, please make an appointment with your GP. When checking your blood pressure keep still and quiet. Moving, chewing, talking and laughing can affect your reading. Make sure you don't cross your legs, as this will raise your reading too. Take two or three readings, each about one to two minutes apart. If your first reading is much higher than the next, ignore it and take an extra reading. Once you have two to three readings, you can work out the average. Keep a record of your measurements. PUBLISHED TODAY: Internationally known #strokeexpert Tudor Jovin, MD, medical director of Cooper and Inspira Neuroscience and Professor of Neurology and Neurological Surgery at Cooper Medical School of Rowan University @CMSRU, is the co-principal investigator and co-lead author of a study published today in The New England Journal of Medicine which demonstrates the benefits of treating stroke patients with severe #braindamage. The study, led by researchers from Montpellier University Medical Center @umontpellier in France, in collaboration with researchers from Hospital Foch @hopitalfoch in Paris, France, and from Cooper, focused on stroke patients with severe brain damage due to blocked blood vessels without any restrictions in the size of seemingly irreversibly damaged brain as ascertained by brain imaging studies. Traditionally, doctors believed that restoring blood flow to severely damaged brain areas was futile and could even cause harm. However, this study challenged that belief. “These findings could revolutionize how doctors treat stroke patients with a large vessel blockage and severe brain damage on pre-thrombectomy scans, potentially saving more lives and reducing disability. As a result of this study, more stroke patients who were previously considered ineligible for #thrombectomy may now benefit from this life-saving therapy,” said Dr. Jovin. “These studies lend additional support to the significant discovery emerging from LASTE,” Dr. Jovin said. “Following the LASTE study, an extra 20% of stroke patients with major artery blockages, previously thought ineligible for mechanical thrombectomy, can now potentially benefit from this life-saving and disability-reducing treatment. Do you need to check your blood pressure when you are young? Many people reduce blood pressure to being an "Old person's problem". But Hypertension affects all ages. In a report from the ONS, they show that the age group 16-35 has the highest percentage of undiagnosed cases of high blood pressure proportionally to the number of cases. Moreover they show that a large percentage of people who have undiagnosed hypertension would self diagnose themselves as being in 'good health', they would not be obese nor would they smoke. Some of you may or may not know but two years ago, Lori suffered from a stroke. It was terrifying but thankfully she was at work and was able to get medical help right away. After several tests, the doctors determined that her heart was the real culprit. Apparently, Lori had a hole in her heart since birth (but didn’t know it!), which allowed a blood clot to go straight to her brain after it entered her heart. Side note: Doctors estimate that roughly 25% of us have this hole but are not aware of it. Many of us live a full life and never realize we have it! A couple months after her stroke, Lori underwent a heart surgery to fix the hole. Thankfully she is 1) still with us here on Earth, and 2) has made a full recovery from the whole ordeal. Our family definitely learned a lot about strokes and heart health through this whole process and feel incredibly fortunate that the situation was not worse. So as a way of giving back, we will be donating $1 from every bag of coffee sold during the month of May to support stroke research and prevention. 🧡 And in case you’re wondering...coffee can help prevent strokes!!! According to the Harvard Medical School, researchers found a 32% lower risk of stroke among people who said they drank two to three cups of coffee and two to three cups of tea daily compared with people who drank neither beverage. We need to not risk our health and our lives simply because we haven't checked our blood pressure! We need to make checking our it more of a habit. And we need to understand how to manage it. It's five years after her stroke, and Alanna is still waiting for answers. 😞 💔 As a young mum with two children, she has a whole life ahead of her. But Alanna has a fear of what her future holds. She’s worried about having another one, because the cause of her stroke remains unknown. “It’s frustrating not knowing why I had a stroke,” she says. “You just worry. You constantly worry.” “It happened just like that. No warning. Nothing,” Alanna remembers. “I just felt something go in my head and then I couldn’t talk.” - Her husband, Andrew, quickly rang 000, and the ambulance arrived. The paramedics suspected she was having a migraine due to her age. - CT scans and other tests followed. Doctors began treating her for a migraine. It was another three days before doctors ordered the MRI that confirmed her stroke. But she was so young! Alanna had just given birth and was only 32! - “You think it just happens to old people but it doesn’t,” she says. “More research could help give more awareness to doctors and professionals. So, if someone presents with a stroke [no matter how young they are], you treat them for it. You don’t wait.” - She’s scared another stroke will strike and what it could mean for her husband and two young kids. She’s scared because no-one – not even specialists she saw after her stroke – could uncover why it happened. That’s why she’s asking for more research to be urgently funded. - “Oh, it would be a relief if I found out why it happened!” says Alanna. “I don’t want any other young mum to go through what I went through.” - That’s why Alanna and her family are sharing their story to raise money for more research into stroke – they want answers, we all do! New paper out now! The Oxford Cognitive Screen (OCS), developed in our lab, is a tool for post-stroke cognitive impairment, evaluating the cognitive domains most often affected by stroke, including language, attention, memory, praxis, and numerical cognition. Here we linguistically and culturally adapted the OCS to generate a Korean version. To validate this Korean version of the OCS, 97 Korean participants completed the new OCS. The percentile scores from the Korean participants were compared with those provided in other populations (Danish, Italian and UK validation studies). The cut-off score for each subtask was similar to that of the original UK version. With this normative study, the K-OCS was successfully developed and validated, which holds significant applications for enhancing clinical practice in Korean stroke survivors dealing with post-stroke cognitive impairments. #oxfordneuropsych #strokeresearch #cognitivescreening
