Renal failure from advanced liver disease...but how?! Hepatorenal failure (HRS) is one of the diagnosis that you are bound to see in Critical Care. It can be an interesting one to manage, and even diagnose because it can initially seem like something else (i.e. sepsis, postrenal failure, over-diuresis, etc...) Ruling out of other diagnoses is part of the discussion to eventually land on HRS. However, a few things are absolutely true: ✅Severe liver disease with ascites ✅Oliguric AKI HRS stems from a refractory compensation that eventually leads to systemic vasodilation and renal vasoconstriction. Liver failure releases vasodilators such as nitric oxide, which cause splanchnic and systemic vasodilation. This, plus central hypovolemia due to ascites, leads to: ⬇️SVR ⬇️ BP ⬆️ CO The body hates all of that and tries to compensate by stimulating the release of vasopressin and activating the RAAS and SNS. Initially, compensation does occur but eventually fails. The patient will develop refractory systemic vasodilation, but thanks to the RAAS & SNS activation we will see renal vasoconstriction. This systemic vasodilation and renal vasoconstriction leads to poor renal perfusion and failure. tl;dr: The body tries to compensate, compensation doesn't work, leads to more problems, failure. 😔 Treatment involves: ✅Improving BP via vasopressors and reducing splanchnic vasodilation. ✅Albumin administration Unfortunately, liver transplantation is the true cure and some patients may not be an candidate for that. This is where goals of care conversations typically begin. What has been your experience with HRS patients? The four P's of AKI ➡️➡️➡️Proper hydration, prefusion, pharmacology, and prevention. It is important to become familiar with the three categories of AKI that you will see in critical care so we can work collaboratively to prevent further damage. (Remember: AKI can affect 30-60% of critically ill patients 🤯So, you will encounter it!) Each AKI category comes with a generalized statement: ⌛Act early versus late in addressing causes or contributing factors of AKI 💧Proper Hydration💧 ➡️Address volume deficits while preventing overload. Administer diuretics if fluid overload exists. ⚖️Goal = euvolemia 🧂Data suggest benefits over using other crystalloids over 0.9% NaCl for resuscitation due to the risk of hyperchloremia ➡️ been shown to increase the incidence of AKI and the need for renal replacement therapy. 💞PERFUSION 💞 ➡️Support perfusion via inotropes or pressors. ➡️Know when to WEAN medications as well. Vasoconstriction is a contributing factor. 💊 PHARMACOLOGY 💊 ➡️ Address medications early. Nephrotoxic and non-nephrotoxic medications. ➡️Dose adjustments or cessation of medications in AKI. 🚫 PREVENTION 🚫 ➡️ Prevent further injury or secondary injury through the recovery process. Maintain perfusion! ➡️ Don't overdue renal replacement fluid or diuretics. ➡️Premedicate for contrast if there is a risk of AKI. ➡️ Communicate with providers in advocating for early or proactive approaches to prevent AKI. What is pulse pressure? Pulse pressure is the difference between the systolic and the diastolic. To calculate a pulse pressure you just subtract SBP - DBP = Pulse pressure Small differences we call a “Narrow pulse pressure” Huge differences we call a “Wide pulse pressure” Narrow Pulse Pressures are usually associated with low stroke volume states: Cardiogenic shock Hypovolemia Pulmonary Embolism Pneumothorax Cardiac Tamponade Aortic stenosis (The list goes on and on…) Wide Pulse Pressure has even more reasons that could cause it but is often associated with vasodilation: Sepsis Anaphylaxis Neurogenic shock Etc… But there are limitations: ➡️ The automatic bp machine doesn’t provide the most accurate systolic blood pressure (it’s more of an estimate determined by a mathematical algorithm, not the actual first korotkoff sound heard with a traditional manual bp cuff and stethoscope) ➡️ Aging can also affect your pulse pressure as vessels become more stiff with age ➡️ So many factors affect blood pressure from how much volume is on board, to how constricted or dilated or stiff your blood vessels are, and how effectively your heart is squeezing. Sometimes you can have conditions that overlap causing a normal looking pulse pressure. Bet even with all those limitations, the pulse pressure as a tool still has value. How does your facility assess for fluid optimization? Comment below! Have you heard of the Goldilocks principle that has been used to describe fluid optimization? 🐻🐻🐻 Well, it speaks to finding the "just right" spot. Just like the three bears --- too much of something isn't a good thing, and too little of something is a good thing. The goal is finding that sweet spot where you have a nice balance. This is how fluid optimization works, and why assessing for fluid responsiveness is so incredibly important. 🏜️Too little fluid can lead to a variety of complications such as low cardiac output, organ hypoperfusion, tissue hypoxia, acute kidney injury, increased length of stay in the hospital, and compounded complications. 🌊Hypervolemia can lead to complications as well. Including pulmonary edema, interstitial edema, tissue hypoxia, and increased LOS and compounded complications. What we really want to achieve is a balanced state ⚖️ - euvolemia - the "just right" zone. Where we have adequate perfusion with a balanced volume status which will reduced complications related to fluids. So, how does your facility find the "just right" spot for your patients? CVICU nurses use these values frequently, however, I am curious if you are seeing them being used in other critical care specialties. Let me know in the comments! ScvO2 and SvO2 are sometimes thought of as being the same, but they are slightly different - simply because of where they are actually obtained. Both numbers are influenced by oxygen delivery and oxygen demand, and when we see deviations from an expected ScvO2 or SvO2 - we have to ask ourselves: is this an oxygen demand issue, an oxygen delivery problem, or a combination of both 😐 Once we can establish that, then we can respectively look at the possible causes of the deviation. #CriticalCareNurse #ICUNurse #ICUNursing #CriticalCare #PCUNurse #NewGradICUNurse #CVICU #PulmonaryArteryCatheter #Hemodynamics #CCRN

It never fails to surprise me how life can change in an instant. Working in neuroscience, I see it all too often. But I feel like I never get used to it. One minute, someone is living their normal day to day life, and then it could all change. Sometimes it gets better, sometimes it doesn’t. We see both sides of the coin. One minute could change everything. Although some days are tiring and often emotionally strenuous, I feel like I forget to acknowledge the heart I put into what I do. And even on my worst days, I find that I still can find ways to love it. And I can’t believe I’ve been doing it for 6 years. Life goes faster than you think 🧠🩺 #nurse

Is this part of your routine during shift report? Do you tailor your alarm parameters to your patient? Here is the thing: 🦺 Alarms are there to alert us that something needs to be attended to on our patient. They are there to keep our patients safe. We know that, though. AND 😩 We know the impact that alarm fatigue has on us...including desensitization to said alarms (yikes 😳) Checking alarm parameters and tailoring them to our patients can address both of these things. But, we know our patients are forever changing...so just be sure you do your due diligence in checking the alarm limits when you assume care of the patient. Just because the limit was appropriate at five hours ago doesn't mean it is now. Lastly, just trust me on this one... check them. Did you know that a portable chest X-ray isn't what actually confirms that the endotracheal tube is in the correct position? 🤯 Wait -- So, why do we always get one after we RSI a patient? First, let's start off with discussing what tells us proper placement of endotracheal tubes ➡️ waveform capnography or capnometry/ colorimetric EtCO2 if that isn't available. (aka "we have color change") This can confirm that the ETT is in the patient's airway compared to the esophagus. However, it doesn't tell us if the endotracheal tube is in the optimal position/depth. Optimal position = 5 cm (+/-) 2 cm above the carina. A portable chest radiograph can tell us the depth of the endotracheal tube and provide us with information if the ETT is too deep and thus in the right mainstem. However, it will not differentiate between esophageal vs airway intubations. So, we can use a few different tools in our toolbox WITH the gold standard capnography...which includes three points of auscultation. Let’s talk hemodynamics 🤓 🫀🕒🩸Cardiac Output (CO) is the amount of of blood that comes out of the heart after A MINUTE’S worth of myocardial contractions. 🫀🩸 Stroke Volume (SV) is the amount of blood that comes out of the ventricle with EACH myocardial contraction. 💪 Stroke Volume has 3 components ⬇️ ⛽️ Preload: Blood volume “Gas in the tank” 🗜️ Afterload: Resistance the heart has to push against to get the blood up and out of the ventricle “how tight are the pipes?” 🏋️ Contractility: Strength of myocardial contraction 💡If CO = HR x SV, then we can either fix the HR or fix the SV to improve CO LET’S FIX THE HEART RATE: 🙌 📉 We can give drugs to slow the HR down (increasing fill time and possibly improving CO) 📈 We can give drugs to increase HR (more myocardial contractions per minute) LET’S FIX THE STROKE VOLUME: 💪 ⛽️ Preload: give volume (IV fluids, blood products, etc…) or pull off volume (diuretics, dialysis, etc…) 🗜️ Afterload: Either increase afterload with vasopressors or decrease afterload with vasodilators 🏋️ Contractility: Positive inotropes will increase the strength of myocardial contraction It’s a lot to juggle when titrating vasopressors and inotropes!
