EKG 71: Sinus rhythm with bigeminal outflow tract PVCs 👶🏻 Beginners should notice that the p waves are upright in the inferior leads, and lead I, and negative in aVR suggesting sinus rhythm. The are wide QRS beats seen in an alternating pattern. These are PVCs in bigeminy. When they are seen every third beat, that’s trigeminy. 🧒🏻 Intermediate readers should be able to localize the PVC site of origin. In this case, you can see tall R waves the inferior leads suggesting the the vector is going strongly towards the inferior wall. In other words, coming from high up like the outflow tract. Further evidence lies in aVR and aVL (high right and left lateral leads) being completely negative. 👨🏻🦳 Advanced readers should be able to further delineate the site by looking at the precordial transition. In this case the QRS goes from negative to positive in V4, which is late transition. That means the site is more anterior ie. the right ventricular outflow tract. V1 has a slight isoelectric segment followed by a negative deflection suggesting a location not too anterior (QS) and not too posterior (initial r). To further hone in on the site, the next lead to look at is lead I. It is negative! On initial review this may appear strange because I just said this is from the RVOT so why is the vector going left to right? The key lies in understanding anatomy. As we go higher up in the RVOT, it goes leftward producing this vector. When we’re higher up in the RVOT at or above the pulmonic valve and not on the anterior free wall, we have to consider the possibility of being close to the left coronary circulation - distal left main or proximal LAD. Be cautious in energy application and if there’s any doubt do an angiogram to check. Image modified from Ho. Подготовка к Step 1 Я составила свой план подготовки таким образом, чтобы сдать экзамен сразу после 6 курса. Что я делаю? - Ежедневно просмотр First Aid (чаще старые и пройденные темы) - Просмотр тем в First Aid или в AMBOSS по учебному плану университета - Раз в неделю решение задач вместе с напарницей. Раньше мы решали вопросы только по тем темам, которые читали на неделе. Теперь мы решаем по 40 вопросов рандомных тем. - Создание собственных карточек по необходимости. На данный момент подготовка выглядит так. Моя подготовка не очень активна, на фоне других стептейкеров. В своем графике я оставила для себя время на работу, встречи с близкими и учебу в университете. К сожалению мне пришлось отказаться от дежурств в больнице, на которые я активно ходила с 1 курса. Но я не жалею, для меня сдача USMLE самая главная цель в карьере на данный момент.
