In 1937 – Katz and Wachtel suggested that large diphasic complexes in the standard limb leads were pathognomnic of congenital heart disease. They examined the ECGs of 43 children with congenital heart disease and proposed that the QRS contour may represent combined right and left ventricular ‘strain‘. 🫀 Katz-Wachtel phenomenon: Tall diphasic QRS complexes (>50 mm in height) in the mid-precordial leads (leads V2, V3 or V4) typically associated with Biventricular Hypertrophy (BVH). This is the classic ECG pattern of BVH, most commonly seen in children with congenital heart disease e.g. ventricular septal defect (VSD). Katz-Wachtel phenomenon has been reported in a case of unrepaired tetralogy of Fallot with hemitruncus in an adult as well. There was anomalous origin of right pulmonary artery from ascending aorta known as right hemitruncus. TIP: The QRS amplitude is often confusingly large overlapping other precordial leads. The source for AT tend to originate from predefined anatomic locations rather than occurring randomly throughout the atria. This supports the use of PWM to provide some guidance as to the likely site of origin. Important revisions were made to the P-wave algorithm to reflect advances in 3D mapping and recognition of additional sites. The 12-lead surface ECG remains highly relevant in the field of electrophysiology. It is useful to accurately predict the site of origin of focal AT, which continues to demonstrate preponderance to occur at specific sites across the atria. Having the ability to predict LA or RA origin allows for better planning of mapping and ablation strategies, which will lead to improved time and resource efficiency during ablation procedures. The revised algorithm does not include uncommon but potential sites of focal AT, such as the ligament of Marshall or posterior left atrial wall. The predictive value of the algorithm ideally needs to be validated at these other sites. Later, the criteria for tombstone were modified by Guo et al. 🫀 Accordingly, the ECG characteristics for tombstone STEMI are as follows: • Absent R wave or an R wave duration of <0.04 second with minimal amplitude. • Convex upward ST segment merging with descending R or the ascending QS/QR • Peak of ST-segment is higher than the R wave • ST-segment merges with the T wave The above-mentioned characteristic features help in recognition and early discrimination of tombstone ECG from other types of ECGs and MI. Tombstone STEMI is usually associated with poor prognosis with reperfusion polymorphous ventricular tachyarrhythmias due to extremely rapid myocardial damage, poor collateral flow, diffuse coronary artery disease, inadequate myocardial protection effect of pre-infarct angina and elevation of wall tension. In addition, myocardial recovery is shown to be limited despite revascularization, leading to higher incidence of unsuccessful PCI. Sato et al and Aizawa et al affirmed that the characteristic lambda-like ST-elevation pattern was associated with VF during myocardial ischemia.This rare ECG pattern may be a new risk marker for fatal ventricular arrhythmia in patients with AMI. The presence of elevated J-wave followed by a steep downsloping elevated ST segment that merges with the inverted T wave, forming a Lambda or shark fin shape, is reported to be associated with fatal VA, cardiogenic shock and in-hospital mortality. It is closely related to early abnormal ventricular repolarization, which involves acute myocardial ischemia and genetic abnormality. Patients with this pattern have multivessel disease and It was found to be a predictor of larger infarct size and higher mortality. The Aslanger pattern is a specific finding in acute inferior MI with multivessel disease and allows the lead of a rapid revascularization. Limitations: The pattern was found to be present in a small amount of patients without acute MI, which may be a result of chronic change from a previous ischaemic events. They like to call it “Dead Man Sign”, as this sign resembles a dead man lying on the ground as shown on the circles below. This sign is formed on ECG by very high ST-Elevation in leads II and aVF associated with similar mirror images reciprocal changes in leads I and aVF. The anatomical lesion associated with this ECG sign is suggestive of total or near total occlusion of right coronary artery. We have observed that this sign is easily recognisable and associated with RV extention of MI associated with multiple tachy and bradyarrythmias mostly SVTs or ventricular tachyarrhytmias and predicts a stormy course. 🫀 Hypokalaemia is defined as a serum potassium level of < 3.5 mmol/L. ECG changes generally do not manifest until there is a moderate degree of hypokalaemia (2.5-2.9 mmol/L). The earliest ECG manifestation of hypokalaemia is a decrease in T wave amplitude. ECG features of hypokalaemia (K < 2.7 mmol/L): • Increased P wave amplitude • Prolongation of PR interval • Widespread ST depression and T wave flattening/inversion • Prominent U waves (best seen in the precordial leads V2-V3) • Apparent long QT interval due to fusion of T and U waves (= long QU interval) With worsening hypokalaemia: • Frequent supraventricular and ventricular ectopics • Supraventricular tachyarrhythmias: AF, atrial flutter, atrial tachycardia • Potential to develop life-threatening ventricular arrhythmias, e.g. VT, VF and Torsades de Pointes Hypokalaemia is often associated with hypomagnesaemia, which increases the risk of malignant ventricular arrhythmias. Check both potassium and magnesium levels in any patient with an arrhythmia. Replace potassium to ≥ 4.0 mmol/L and magnesium to ≥ 1.0 mmol/L to stabilise the myocardium and protect against arrhythmias – this is standard practice in most CCUs and ICUs. 🫀 South African Flag Sign is found in patients associated with a pattern of STEMI caused by acute occlusion of the first diagonal branch of the left anterior descending coronary artery. In 1994, Iwasaki et al researched found ST-elevations in leads I and AvL as the most important criteria. In 1996, Birnbaum et al researched the ECG's of 61 patients with ST-elevations in AvL and found D1 occlusion in 8 of then. Doesn't seem relevant, but ALL 8 had ST-elevations in lead I, aVL, V2 & ST-depressions in lead III. More research was done, all proving the same pattern after which Laszlo Littmann came with the name 'South African Flag Sign' as a response to a study done by Edward Durant and Amandeep Singh in 2015. With the 4×3 display of the ECG, the location of the most impressive ST deviations resemble the South African flag. 🫀 ECG Features: • ST elevation primarily localised to leads I, aVL + V2 • Reciprocal ST depression and/or T wave inversion in inferior lead III (often aVF) The ST-segment is usually electrically 'silent' but changes due to injury currents: a small electrical current between healthy tissue and ischemic tissue. - The ST-vector goes left and upward in high lateral STEMI: in the same direction as the exploring electrode of lead I and AvL, so they get ST-elevations. - Lead III is the mirror image of the ST-vector in high lateral STEMI so lead III gets ST-depressions. - Lead V2 is upward and to the left of the center of the heart, which also makes the ST-vector go towards the exploring electrode. Right ventricular hypertrophy (RVH) is an abnormal enlargement or pathologic increase in muscle mass of the right ventricle in response to pressure overload, most commonly due to severe lung disease. Diagnostic criteria: • Right axis deviation of +110° or more. • Dominant R wave in V1 (> 7mm tall or R/S ratio > 1). • Dominant S wave in V5 or V6 (> 7mm deep or R/S ratio < 1). • QRS duration <120ms (i.e. changes not due to RBBB). Supporting criteria: • Right atrial enlargement (P pulmonale). • Right ventricular strain pattern = ST depression / T wave inversion in the right precordial (V1-4) and inferior (II, III, aVF) leads. • S1 S2 S3 pattern = far right axis deviation with dominant S waves in leads I, II and III. • Deep S waves in the lateral leads (I, aVL, V5-V6). Other abnormalities caused by RVH: • RBBB (complete or incomplete). There are no universally accepted criteria for diagnosing RVH in the presence of RBBB; the standard voltage criteria do not apply. However, the presence of incomplete / complete RBBB with a tall R wave in V1, right axis deviation of +110° or more and supporting criteria (such as RV strain pattern or P pulmonale) would be considered suggestive of RVH. Fontaine bipolar precordial leads (F-ECG) can be used to increase the sensitivity of epsilon wave detection in ARVD. This placement increases sensitivity of detecting epsilon waves so that they are detected in three leads (FI, FII, FIII) rather than one lead in the regular placement. The Crochetage sign is an EKG pattern with a small notch or "bump" near the apex of the R wave in the inferior leads (II, III, aVF) 1958 – first described by Toscan. This sign is also known as the "notch sign" or "the slur". The term "crochetage" comes from the French word for "hook," and the sinus pattern resembles a hook or crochet-like shape. This sign is most commonly seen in patients with Atrial Septal Defect (ASD), although it can also be seen in other conditions such as PFO, PE, RV hypertrophy, and patent ductus arteriosus. This sign has been associated with the severity of the shunt and has been suggested as an ECG marker of a PFO associated with ischemic embolic stroke. The mechanism behind the sign is not fully understood, but it is thought to be related to the delayed depolarization of the right ventricle in patients with ASD. The notch represents a delay in the electrical impulse as it travels from the right ventricle to the left ventricle across the atrial septum. The Crochetage sign has high specificity but relatively low sensitivity for detecting ASD. In other words, the Crochetage sign can be a useful diagnostic tool in identifying ASD. However, it should be interpreted with other clinical and diagnostic findings. Diagnostic tests, such as echocardiography and cardiac MR, are needed to confirm the diagnosis. Patients with MI with either new or existing LBBB have higher morbidity and mortality. Therefore, the correctly interpreting the electrocardiogram (ECG) in these patients is extremely important as it can affect treatment and subsequently the outcomes. With LBBB or during ventricular pacing, the diagnosis of MI is challenging because of the left ventricular activation is delayed; the directional change of septal activation from right to left instead from left to right; and the secondary ST-T wave changes in LBBB. In the current guidelines, LBBB is not always considered an ST segment elevation myocardial infarction (STEMI) equivalent and should not be used as a sole diagnostic criteria of AMI. 🫀Chapman's sign is a notching ≥ 0.05 sec of upslope of the R wave in leads I, AVL or V6 and was first described in 1957. It has a low sensitivity, but a high specificity reaching up to 92%. 🫀Chapman’s sign can enhance diagnosing AMI in patients who have LBBB and do not meet the original Sgarbossa’s criteria. An ECG interpretation of all leads and serial ECGs are important for evaluating patients with cardiac ischemia. In patients with left bundle branch block (LBBB) or ventricular paced rhythm, infarct diagnosis based on the ECG can be difficult. Abnormal depolarisation should be followed by abnormal repolarisation, manifesting as ST-segment and T-wave deviations that do not necessarily indicate acute ischaemia (“appropriate discordance”) 🫀 Original Sharbossa Criteria The original three criteria used to diagnose infarction in patients with LBBB are: • Concordant ST elevation > 1mm in leads with a positive QRS complex. • Concordant ST depression > 1 mm in V1-V3. • Excessively discordant ST elevation > 5 mm in leads with a negative QRS complex. The Smith modified Sgarbossa criteria for Occlusion Myocardial Infarction (OMI) in LBBB have been created to improve diagnostic accuracy. It is worth mentioning an electrocardiographic sign described in clinical traces by Cabrera and Friedland in 1953 and interpreted by Medrano et al in 1963 based on experimental findings: an electrical window can be established in the interventricular septum, which allows the exploring electrode of a cable, V3 or V4 or both, record the potential variation of the mass of the right posterior septum, ie. and. a notch in the ascending branch of the S wave, lasting 40 ms or more. This is significant on the ECG if it lasts that long. Notching of the ascending limb of the S wave in leads V3 or V4 is called Cabrera's sign.
