• 🍬Biguanides are the first-line treatment in all diabetic patients and decrease insulin resistance by suppressing gluconeogenesis and lipogenesis. They are the only class that reduces the risk for both micro- and macrovascular complications. The commonest adverse effect is GI upset, and because they increase lactate levels, they are contraindicated in lactic acidosis, as well as liver or kidney disease. • 🍬Glitazones also increase insulin sensivity, doing so by agonising PPAR-α. Because they act through gene regulation, they show their effect later than other antidiabetics. Linezolid is active against most Gram-positive bacteria that cause disease, including streptococci, vancomycin-resistant enterococci (VRE), and methicillin-resistant Staphylococcus aureus (MRSA). The main uses are infections of the skin and pneumonia although it may be used for a variety of other infections including drug-resistant tuberculosis. It is used either by injection into a vein or by mouth. Linezolid is a weak, non-selective, reversible monoamine oxidase inhibitor (MAOI), and should not be used concomitantly with other MAOIs, large amounts of tyramine-rich foods (such as pork, aged cheeses, alcoholic beverages, or smoked and pickled foods), or serotonergic drugs. There have been reports of serotonin syndrome when linezolid was given with the discontinuation of serotonergic drugs, particularly selective serotonin reuptake inhibitors such as paroxetine and sertraline. It may also enhance the blood pressure-increasing effects of sympathomimetic drugs such as pseudoephedrine or phenylpropanolamine. It should also not be given in combination with pethidine (meperidine) under any circumstance due to the risk of serotonin syndrome. ◆ La #prise_de_poids provoquée par les #médicaments est surtout néfaste chez des personnes qui sont déjà en surpoids ou qui souffrent d’ #obésité. Les principaux médicaments qui peuvent faire prendre du poids : . . ✔ Certains #antidépresseurs, ces médicaments agissent sur les neurotransmetteurs et favorisent l’envie de manger. Il s’agit notamment des molécules suivantes : #amitriptyline, #paroxetine et #sertraline. . . ✔ Certains #neuroleptiques, ces médicaments agissent également sur les neurotransmetteurs et donnent parfois de véritables “fringales”. On peut citer les molécules suivantes : #haloperidol, #clozapine et #lithium. . . ✔ La #pilule_contraceptive, Les pilules contenant des #oestrogènes sont plus sujettes à faire prendre du poids que ceux à base de progestérone car les oestrogènes favorisent la rétention d’eau et donc la prise de poids. En cas de prise de poids avec la pilule classique demandez à votre médecin s’il est possible d’utiliser une minipilule ou d’autres techniques de contraception (par ex. l’anneau contraceptif). . . ✔ Les #corticoïdes comme la #prednisone. Cette classe de médicaments peut malheureusement avoir divers effets secondaires comme la prise de poids. . . ✔ Certains #somnifères et/ou #antihistaminiques : les médicaments à base de #diphenydramine peuvent faire prendre du poids, car cette molécule a un effet sédatif et diminue votre consommation d’énergie (diminution du métabolisme). Par conséquence vous allez brûler moins de calories et allez prendre du poids. . . ✔ Certains #antidiabétiques. On peut citer l’ #insuline, les #thiazolidinediones et les #sulfonylurées. Mais attention, certains antidiabétiques comme la #metformine peut au contraire mener à une perte du poids. . . ✔ Certains #antiépiléptiques. Notamment le #valproate et le #carbamazépine. . . ✔ Certains #hypotenseurs, comme les #bêta_bloquants. . . ⚠ N’arrêtez jamais un médicament sans l’avis de votre #médecin même si vous estimez qu’il pourrait vous faire prendre du poids. Si c’est le cas, le médecin pourra éventuellement le remplacer par un autre mais il faut savoir que dans certains cas (ex. insuline), il n’est pas possible de remplacer un médicament.
