Stan’s home!! 🐈⬛🥳 After a scary wake up to cat howls yesterday morning, an emergency vet visit, overnight stay, and a $3k bill, our little guy is back. He had a urinary obstruction and his bladder was the size of a grapefruit 😳 This condition can be fatal in cats (esp. males) in as little as 24 hours if it’s not treated. I’ve been doing lots of research about this, and it’s quite common. I found a blog that was pretty heartbreaking as it had countless owners talking about their cats passing from this - either due to the condition developing too quickly, or repeat expensive vet visits they couldn’t afford so they made the impossible choice to euthanize. 🥲 It’s really sad that our pets (and even sometimes human lives) can come down to money. We luckily had enough for this bill, but not another one anytime soon. Chances are that this will probably occur again for him. So we plan to make some changes that are fairly affordable and humane to give our guy the best life he can have! - Making the switch to wet food. Cats’ diets need moisture in them and kibble doesn’t give them enough in its own. I read that it’s very important to give them extra fluids, by either soaking dry food or giving wet food, especially for boy cats! - Dust free litter. So cat litter doesn’t go in his little pee-pee 🤭 potentially causing another obstruction. - Using apple cider vinegar if we notice more flare ups. Farmers swear by this and I found an entire YouTube video comment section saying this saved their cats from having a blockage! - Keeping it calm. They said an underlying possibility is stress. Stan seems unphased by everything, but we’ll do our best to keep him that way. 😆 Thanks for all your well wishes for Stan!! Meet Dewey 💧 Dewey came to see dr. Maharaj because he was straining to urinate and unable to pass urine. He was diagnosed with a urinary obstruction so we quickly got to work unblocking him. Cats that have urinary obstruction require emergency treatment. Sedation or general anesthesia is needed to allow placement of a catheter into the urethra to flush out the plug or force the stone into the bladder. The bladder is thoroughly flushed and drained through the catheter to remove any remaining sediment. The urinary catheter is then typically left in place for a few days until urethral swelling subsides. My rescuer reached out to Meowmy to see if she wanted to take another Sphynx. Unfortunately, Simon had his third urinary obstruction and was going to be owner surrender. Simon was probably going to need a PU surgery in the immediate future. Well, Meowmy said OK we would take him thinking Simon was in Florida because that’s where the rescuer lived. Meowmy could just drive to pick him up. He was actually in New York! Fortunately, Meowmy has a good friend up there who said she would help with taking him to his vet visits up there and caring for him til he could get to New Orleans. After he left the first hospital for the obstruction, he re-obstructed before getting a flight to New Orleans and had to have his PU surgery up there. So many may not know the chaos that has been these last few days. Gus had loosely obstructed 3 weeks ago and spent time at the Hershey Animal Emergency Hospital. He was diagnosed with struvites and placed on C/D. Unfortunately there was so much grit in his bladder that did not get flushed out and the food didn’t have enough time to do it’s job, that he re-obstructed on Monday morning. After getting all the other animals sorted and him into a carrier we headed back to the emergency hospital. It was determined that he would need a PU and a cystotomy (to clean out the sludge and grit in his bladder, not because there were signs of stones). This was quite fortuitous as his bladder or urethra showed potential of a previous slight rupture. His bladder was flushed, and his omentum was removed from his bladder, where it had adhered. His abdomen was also flushed, because the same debris was in his abdomen. And he was walking around acting perfectly fine prior to Monday morning. I did have comfort in the fact that the veterinarian who did his procedure was a veterinarian that I worked with several years ago. I am extremely thankful for all my friends and coworkers that have been there these last few days. Wet only for FLUTD? Yay or nay? 🟡 Vets! This one is for you! ⚠️ Making a blanket recommendation that feeding any wet food as a sole diet will prevent, cure, treat or manage FLUTD is incredibly dangerous - and here's why: Crystalluria is primarily the result of oversaturated urine; consuming a diet high in mineral content (specifically the precursors to urinary crystals), regardless of the water content will lead to the urine becoming oversaturated as the amount of water taken in is still not in balance with the amount of solute (minerals) being consumed. 🪨 When comparing a retail wet diet not designed for FLUTD, to one that is, not only can we not access any information on mineral content, the protein content is significantly higher than the upper end we aim for in FLUTD - this is also why a raw diet does not "cure" FLUTD, as increased protein content has a direct relationship with increased mineral content. This is also why a high protein diet encourages water intake - because it's excessive in mineral content and is stimulating water intake to try and regain balance...however this is still not enough to prevent oversaturation of urine. And yes, cats on a raw or all-wet diet still block.🥄 What *is* the case for wet diets? If your cat has stress-induced cystitis (FIC) that does NOT involve crystalluria, then studies do show a benefit to feeding a wet urinary diet or calming food as the urinary OUTPUT of water is greater (helping to reduce small frequent urination that increases inflammation). 🤏🏻 The same benefit has never been shown in FLUTD cats due to the way in which urine becomes oversaturated in these patients. ⚠️ Vets - recommend a therapeutic urinary diet, wet OR dry, as both are equally effective at managing your patient's risk of recurrence. Causes of poor urinary stream in men. Imagine urine flow like water flowing out of a tap. There can be two causes of water not flowing with good stream out of a tap: 1. Tap tubing or channel( URETHRA) is blocked. 2. Motor( BLADDER MUSCLE) pumping the water into the tap from the reservoir(BLADDER) is not functioning properly generally due to faulty wiring( nerve supply to bladder) Causes of a blocked tap( urinary passage) 1. Stricture ( scarring ) of urethra: can only be corrected surgically. 2. Enlarged prostate( benign or cancerous): can be treated with medicines and surgery. 3. Fun fact: I have been to the vet twice due to a urinary obstruction. Had I not gone sooner the first time, I could have died! I’m a stubborn boy and never drink water so my Masters had to buy this specific food so I no longer get urinary obstructions. For those of you who don’t know what a urinary obstruction is, I basically cannot pee no matter how hard I try. I will attempt to use the litter box numerous times but nothing will come out. Sooner or later my urine will back up towards my kidneys and my kidneys could end up failing. Anyways that was a very dark time but I’m glad I made it through! Make sure you cool cats always drink water! Here’s a throwback to me coming back from my second visit from the vet! Dusty is not a happy camper today. This morning, the vet said his temperature was high again, and the "obstruction" stopped moving, so he had to have exploratory surgery. Update from vet: Dusty's bladder was ENORMOUS and contained a lot of infectious looking material/chunks of material. I've seen many cases of bladder sludging (that always show up on x-ray), but haven't seen this before. His bladder had evidence of bruising and inflammation. I opened the bladder to make sure there were no tumors, stones, etc. I didn't find anything. I flushed the bladder and urethra from the inside and made sure there were no blockages. Before I flushed, however, I do think Dusty had a urinary obstruction. I recommend culturing the urine, and sending out for biopsy a sample of the bladder wall to see if we can identify why this happened and see if there's anything we can do to prevent it from recurring. Hoomin mom: Uggghhhhhh!!! My baby boy! He's breaking my heart! He recovered well, and when the vet went to check on him after recovery, he thumped at her, which he rarely does. He's now hiding in the back of the cage at the vet tech's house. She's going to let him calm down and check on him later. Meet Louie! Louie is a 2-year-old male neutered cat who came in to see Dr. Rubi on emergency for decreased urination and hiding under the bed. His mom had noticed a decrease in urination over the past month that would return to normal periodically. However, when she came home and found him crying under the bed, she knew something was clearly wrong. On physical exam, Dr. Rubi found that Louie's bladder was small and firm, and she was unable to manually express him. Struvite crystals were also found in his urine on cytology. Louie was suffering from a urethral obstruction. A urethral obstruction is a common occurrence in male cats and it is a medical emergency. Failure to seek treatment can lead to fatal electrolyte imbalances or the bladder becoming so large it ruptures. Fortunately, Louie was brought in and Dr. Rubi placed a urinary catheter so that he could pass urine. She also started him on IV fluid therapy, antibiotics, and pain medications as the condition is quite painful. After a couple of days, the urinary catheter was pulled. After ensuring Louie was able to pass urine on his own, he was sent home with medications to continue his recovery. Our ICU technician Becca drew this cute little portrait of Louie while he was hospitalized with us. GARRY UPDATE!! Many of you know and love our hospital cat Garry (and if you don’t, here he is in all his glory!) So allow us to take this moment to provide some education on bladder stones! 🤔🪨 Garry came to us as a very sick kitten 2 months after we opened – can you believe he turns 11 in August?! Garry has lived a happy and healthy life as our Official Client Ambassador/Greeter Cat, and as he has entered his senior years he has had some age-related back pain we have been treating and keeping an eye on. As part of his routine wellness labwork this past winter, it was discovered that he has a bladder stone! Bladder stones are rock-like mineral formations that can occur when minerals exceed a certain threshold of concentration in the urinary system. They typically consist of either Struvite, which can be dissolved with a prescription veterinary diet, or Calcium Oxalate, which cannot be dissolved. Since Garry has been eating his prescription Hill’s c/d Stress diet for years, which would have dissolved any Struvite stones, we knew that his was a Calcium Oxalate stone which would eventually need to be removed. Garry had not shown any urinary difficulties at the time the stone was discovered, so we opted to monitor him closely to watch for any straining in the litter box, blood in his urine, or decreased urine output. Last week Garry had a minor blockage (he was yowling in his litter box) – so Dr. Bookmyer sedated him to pass a catheter and flush out the blockage. Since he has now shown signs of trouble with his bladder buddy, it’s time to begin eviction proceedings. That lil’ stone is scheduled to come out on June 22nd! 🗣️HYPERKALEMIA in the blocked 🐈: 👉🏻🩺On presentation, you may see the patient come in “yowling”/vocalizing, lateral/uncoordinated/ataxic, pale mm, prolonged CRT, hyperpnea, hypothermic and/or non-responsive. 🐢🫀They may have mild to profound bradycardia (110bpm-140bpm mild to moderate, 100bpm-60bpm profound) due to the potassium build up that’s now within the blood stream. 🖥️On EKG, you may see TALL tented T waves, lack of P waves, widening of QRS complexes, prolonged QT intervals. 🩸On bloodwork, you typically see hyperkalemia with hyponatremia (remember the Na:K channels???), & azotemia. 🚨In severe cases, the “sine” wave my be present which indicates cardiac arrest is imminent!!
