Measuring follicles I have been asked quite a bit and seen a variety of ways in which people are measuring their follicles, but these are the most commonly seen. Single longest distance Average diagonal distances Average L x H distance Let’s start by saying, I don’t care how you measure and if it’s working for you then keep doing you :). That said, in ultrasound we generally measure structures with a single distance measurement or a 3 distance volume. We never do a diagonal averages as the information is always skewed and heavily influenced by the operator and their skill. Single distance measurements are not limited to being cranial to caudal (from head to tail) but rather the single largest measurement of the structure (follicle) being evaluated. This allows for a single repeatable measurement of the structure to be tracked (assuming you scan the same follicle every time). Coupling the primitive understanding we have of follicle size relative to development, we operate largely on a single distance value anyways. Any measurements based on averages simply would not corroborate with the accepted growth to follicle size that we have. That doesn’t mean there isn’t potential value or increased accuracy, but you can not apply principles to which the input data is acquired differently. Nearly all multiple measurements in ultrasound are used to calculate volume. To do so, 3 measurements must be acquired and calculated out (cranial/caudal, anterior/posterior and lateral/medial). This would likely yield the most information about follicle size to development we could obtain, especially with blood panels. All that said…who cares? If you like data and want to understand deeper, have at it. Otherwise, get your single longest distance measurement and keep moving. YES - Normal Delivery with 4 Loop of cord around neck 👌📣 Nuchal cord is the medical term for when the umbilical cord becomes wrapped 360 degrees around the neck of the baby inside the uterus. 📣Nuchal cords are surprisingly common and unlikely to cause problems during pregnancy or at birth. Study suggest that 20 to 30 percent of all deliveries involve a nuchal cord! 👨⚕️However seeing them on SONOGRAPHY in the absence of fetal distress and with normal dopplers means we don’t do anything! The presence of nuchal r cords are NOT cause for intervention (Operation/ c- section ) Nuchal cords are highly unlikely to cause problems during pregnancy and birth! If noted at birth we either slip the cord over baby’s head or if extremely tight, we clamp and cut it. ⚠️Of course there is the rare occasion where a nuchal cord can affect the blood flow, usually in labour, causing baby to become distressed! In this case intervention is recommended. ⭐️We have delivered many baby’s with 2-3 (sometime 4) loops around them and they come out just fine! So please don’t let this scare you into agreeing to interventions when they are not indicated! Ultrasound tip of the day…knowing situs. Situs refers to the side of the body relative to the structures within. This image shows the exact same follicle, just evaluated from different sides of the snake. This specific example is of a follicle within the LEFT gonad. The image on the left is showing the evaluation when scanning from the left side of the snake. The image is more defined and more details can be seen, such as embryonic development. The image on the right is the exact same follicle, just evaluated from the right side of the snake. The follicle is deeper on the image which means there is more attenuation (loss of beam strength). This attenuation reduces image quality, visible details as well as structure dimensions (oblique viewing of structures). You want to ensure you are measuring a follicle from the gonad ipsilateral to the side you are scanning from. The follicle on the left measured 34.1 whereas the follicle (same follicle) on the right measured 32.5. Does this difference matter…NOPE. However, if you are taking the time to freeze and measure anyways, you may as well be accurate. For a better understanding on anatomy, check out this video I made a while back. A quick glimpse at follicular development and viability. Quick answer - Left follicle is bad, right follicle is good For those who want to learn - Echogenic density is displayed as the brightness an object appears. The more dense an object is, the brighter it will appear. Once an object is so dense that sound can no longer penetrate, that’s when shadowing appears. This is often seen with rib and vertebral shadows when oriented to close the the snakes spine. Likewise, a less dense object will appear black/darker. This is most commonly seen with the gallbladder. Since the gallbladder is filled with bile, the sound can penetrate nearly uninterrupted, thus no echos are made to display an image within. Follicular health/viability can often be tracked using ultrasound as well. A healthy follicle will generally appear as a thin uniform wall, uniform echoes within, and a darker embryonic ovoid structure may be seen. This may vary based on the stage of development and fertilization. A follicle that has failed to fertilize (slug) may appear smooth walled, but will generally have a bright texture throughout…bright walls as well as within. A follicle that is likely to fail and be reabsorbed will generally have bright thick walls with irregular shape, bright to slightly brighter to “normal” texture within and may show dark areas around the follicle itself. Keep in mind, throughout the hormonal changes and development process, there will be natural variations in appearance which may have overlap good/bad appearance. Keep scanning and use ANY information you see as a guide to your own learning process.
