I received a call today telling me that Alfie had cut his leg and the bone was exposed. The good news is that he has missed the 1 tendon in that vicinity of his leg and the knee joint when tapped showed no signs of being affected. The not so good news is because the bone was exposed we do not know if there is damage to it. Right now he is on strict box rest with as minimal movement as possible and the vet is back on Wednesday to reassess. Look as this for an impalement wound......poor cat, massive wound and if you look closely at the big wound picture you can actually see all the way through to the other side!! The wound was clipped, cleaned and flushed copiously, before being sutured closed, the wound was quite heavily contaminated and infected, and despite antibiotics and copious flushing we were worried about wound breakdown, it was also a large area with a lot of dead space so we didn't want to risk fluid build up so........we placed a DACC dressing (a gauze type dressing which is impregnated with DACC which binds the bacteria and causes them to lyse) rolled up placed along the inside length of the wound, with the end coming out the end of the wound, to act as a make shift drain, with extra properties, you can see the end poking out if you look carefully at the last photo. #svn2rvn #svn #rvn #vetnurse #studentvetnurse #vetnurselife #wound #woundmanagment

▪️Arterial ulcers also referred to as ischemic ulcers, are caused by poor perfusion (delivery of nutrient-rich blood) to the lower extremities. ▪️Symptoms of Arterial Ulcers Arterial ulcers are characterized by a punched-out look, usually round in shape, with well-defined, even wound margins. Arterial ulcers are often found between or on the tips of the toes, on the heels, on the outer ankle, or where there is pressure from walking or footwear. These ulcers are generally very painful, especially while exercising, at rest, or during the night. ▪️The most common causes of arterial ulcers are: *Restrictions to blood vessels due to peripheral vascular disease *Chronic vascular insufficiency *Vasculitis (inflammatory damage of blood vessels) *Diabetes mellitus *Renal failure *High blood pressure *Arteriosclerosis (hardening of the arteries) *Atherosclerosis (thickening of the arteries, due to the buildup of fatty materials) *Trauma *Limited joint mobility ▪️Treatment of Arterial Ulcers The primary goal of the treatment of arterial ulcers is to increase circulation to the area, either surgically or medically. Surgical options range from revascularization in order to restore normal blood flow to amputation and rehabilitation in patients who cannot be revascularized. As for non-surgical measures, modifying contributing factors can slow or stop the progression of the local ischemia. Ja ostatnio np. krojąc ogorka skaleczylam sb palec 😆bo chciałam szybko zrobic sniadanie,a maly plakal w drugim pokoju i spieszylam sie do niego. Pacjentka ze zdjecia zasnęła usypiajac dziecko. Na snie ręka obsunęła sie na kaloryfer. Zmeczona mama miala twardy sen i nie poczula ciepła na ramieniu. Inna znowu mama miala oparzona dlon goraca herbata wlasnie w godzinach porannych,po nieprzespanej nocy. Inna znowu zas poparzona dlon i przedramie przez wrzaca wode jak wrzucala do garnka pierogi. Standard to poparzenia goracym olejem przy smazeniu kotletow 😆 Czyli gotowanie bywa niebezpieczne jednym słowem a kuchnia jest miejscem najczęstszych urazow 😆 Sama terapia powyzszych ran to dla specjalistow leczenia ran - pikus i czysta przyjemność 😉pacjent mlody,bez obciazen,chorob,zdyscyplinowany. Terapia skupiała sie na miejscowym oczyszczaniu i regularnych zmianach opatrunkow. Terapia zakonczona sukcesem w 16 dobie od oparzenia 👏😁😉 A Wam zdarzalo sie oparzyc/skaleczyc w kuchni? Wound dehiscence (treatment) Step 1: ID your risk factors (what led to the breakdown?). These can be patient related (e.g. disease), external factors (e.g. medications, infection) or surgeon related. I will focus on surgeon related factors. Surgeon related risk factors are; - Poor technique – incorrect suture/needle selection or knot-tying, poor choice of suture placement, fat within the wound edges or poor closure of dead-space - Tension – increased tension will put more pressure on the sutures and may lead to poor blood supply to the wound edges and ischemic necrosis of tissues. Seromas increase the tension on a wound. - Poor timing or conditions for closure (common) Step two: Classify type of dehiscence. 1 – A small breakdown that affects the superficial layer only and is non-infected. 2 – A small infected area but otherwise the wound is healthy 3 – A medium - large healthy wound with a complete breakdown 4 – A medium - large infected or unhealthy wound with complete breakdown Step three: treat Treatment differs for each type of dehiscence above. I will cover type 4. - General anaesthesia - Sterile technique including drapes - Remove tissue for deep culture (aerobic and anaerobic) - Explore the extent of the wound and debride any obviously devitalised or necrotic tissue. Take care not to remove any healing tissue. If in doubt, leave for the bandages, topical wound therapies or autolytic debridement. - Start broad-spectrum antibiotics whilst awaiting culture results. - Consider histopathology if neoplasia is suspected at the wound margins - Irrigate with copious amounts of saline DO NOT ATTEMPT RE-CLOSURE. Consider one of the below options; a) manage as an open wound with bandaging techniques, then repair prior to the formation of granulation tissue = delayed primary closure b) manage as an open wound with bandaging techniques then repair post the formation of granulation tissue = secondary closure c) leave to heal via granulation, contraction and re-epithelialisation = second intention healing. 74 days . . . #veterinarian #veterinarymedicine #wounds #vetstudent #surgeon #woundmanagment

Ouch! After several hand cuts due to the explosion of a glass container in July 2020, these last two glass debris were removed two weeks ago. The patient is now painfree, has full freedom of movement and is back to work. Ouch!
