WTF is Baby Botox? 👶 Baby Botox is wrinkle relaxer that is geared towards a younger age, or dosed in smaller amounts. If a 28 year comes to see me, it is unlikely I will recommend they need 68 units which is somewhere close to the amount the pharmaceutical company recommends. Likewise, if a middle age patient really wants a very light treatment and they have muscles that aren’t very strong, they may be just fine with a lighter dose. The controversy comes in with DOSE 🟰 DURATION. The paper I referenced (below) combined 5 mostly randomized control trials examining Botox, Dysport, and Xeomin and reviewed the data on administration of large dosages (sometimes 4x the recommendation). Patients DID get longer treatment duration (sometimes up to approx a month longer) HOWEVER, large dosages are: ✔️ EXPENSIVE ✔️Complications like eye ptosis or droop MAY carry greater risk with large dosages because of diffusion. In my opinion, I treat Tox like any other drug I might administer as a Nurse Practitioner and I may titrate that according to a number of things- including patient anatomy, muscle strength, their job (ie. Would I give a first time lip flip to a trail lawyer presenting next week? 🤔 ) Most importantly, I consider the outcome my patient is hoping for. Now, bottom line, a full face of Tox CAN and DOES look natural if done well. And if a patient is not getting the duration out of their treatment, dosage would be one consideration. I have so many patients ask about “these white spots” that show up around eyes, on forehead etc. They are white, waxy and hard and simply trapped keratin skin cells 😱 BUT they are totally benign. We see them in babies and they’re no big deal, but as adults they can be annoying. As we age, our pores tend to enlarge, we have hormonal fluctuations and our skin isn’t turning over as quickly, all of which can lead to these bumps. It’s important to cleanse well daily, avoid a clogging moisturizer, and use an alpha hydroxy acid like alumiermd Bright and Clear, a retinol and a once or twice weekly Polish to keep those dead skin cells moving along. In order to get rid of them, I’ve had great results with MOXI which also helps shrink those pores down. What has helped you with milia?? WTF Melasma 🤎 WHO? More common in darker skin tones, women, during pregnancy, and during other hormonal fluctuations. Can be genetic. 🧬 WHY? Increased deposits of melanin or pigment mostly in the top layer of the skin (epidermis), although some may have it stored within macrophages (thinking garbage dumpsters) in the deeper dermis layer which can be permanent. TRIGGERS: Heat, UV rays and hormones. TREAT: Most important thing is protection from UV exposure with sunscreen 🧴! You can do everything else but if you skip this step it won’t matter. Other topicals: Vitamin C, Retinol to increase cell turnover and bring melanin to the surface, and exfoliation to sweep it away such as an AHA. ⚠️ Hydroquinone should only be used under direct provider supervision as long term use can cause pigment to be pushed deep into the dermis which can result in permanent blue hue (ochronosis). 👌🏻 Instead choose lightening agents which have licorice root or Kojic acid. ⚠️As much as I love BBL, it is not good for melasma patients as the heat and light can worsen pigment. MOXI is an awesome, safe option for my melasma patients. Other considerations: Discuss birth control with your provider and the addition of oral vitamin D. WTF is a Tox brow lift 🤔? Are you a candidate and have you liked your results? I’d love to know! I’ve heard injectors promise they can give patients a lifted brow, but it won’t work on everyone. Here’s the test I use which I think I learned from naturally_rana or jubilant.julie 💕 If you don’t already belong to their Patreon you have probably been living under a rock 🪨 because they are amazing educators. Patients with hooded eyes can sometimes really benefit from these injections, but a true brow “lift” or a Bleph done to the tissue overlaying the eye can only happen surgically. Yes, dermal fillers can result in blindness. 😳This is probably the scariest topic for both patients and providers alike. I share this not as a scare tactic but just to educate. There are areas of the face which have been classified as higher risk because the arteries communicate with the retina or eye. These include the glabellar complex (between the eye brows), the nose (non surgical or liquid rhinoplasty) forehead and the temples. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5955635/ Even if we avoid injecting these areas, we still can’t see everything under the skin. And we know from cadaver courses that anatomy is unique and variable. NOT predictable. Please do your research when receiving fillers. 1. Go to a reputable medical clinic and preview their before and afters. 2. Ask them if they’ve taken cadaver dissection courses. 3. If they are injecting these high-er risk areas, what special training did they receive? Who do they have on speed dial for an ophthalmic emergency? Because studies indicate you have minutes, if any, in these cases. 4. What do they keep in place for an emergency? 5. What do they do when they inject to increase safety? 6. Do you have their contact info for after hours? 7. Do they follow up with you after? Patients, have you been injected in these areas before? Did your provider discuss these risks with you? Providers, do you inject these higher risk areas? Why or why not? ⁉️WTF should I be doing in my 30’s??? This is a very common question I get asked! I started Botox when I was 35, but I didn’t protect my investment because I continued to sit out in the sun and I barely managed to wash my face. Now I’m paying for it and all these young gals come in who have it figured out! They’re on the skincare, the sunscreen, and they’re doing a little preventative ‘Tox- mostly Xeomin in my practice. It’s WTF Wednesday! I’ve been a bit lax on these but here’s a repeat that will be new to some of you. On this week’s edition, I bring you “an epic shit show”. Another in the foreclosure series during a time when anything and everything was possible. Had a listing on Adeline in Oakland that easily ranks in the top 5 most memorable in my career. When I got the listing assignment, I did the usual occupancy check and after determining it to be vacant, I walked the property. I’m hoping that you’re not currently eating lunch because this one is sure to curb your appetite. This one had a smell that was evident from the sidewalk and when I opened the front door I was immediately hit with an overwhelming odor. It was horrible, the smell so bad that you had to cover your mouth and nose in a futile attempt to mask your senses. Just inside was a small living room area and off to the right were two “bedrooms”, the condition of which were deplorable. In the first “bedroom” I discover a filthy mattress on the floor, clothes and belongings scattered about and human feces everywhere……..on the bed, on the clothes, the floor, the walls…..imagine that if you will. In the second bedroom there were similar conditions along with needles and used condoms, safety first! My continued search through the rest of the house reveals a common theme and, in the kitchen, piles upon piles of dog feces and urine, IN THE KITCHEN! I’m not terribly squeamish but the overwhelming odor made me gag and my eyes were watering big time. As if the conditions weren’t bad enough it was obvious that small children had been living there amongst the filth and safety hazards. I tried to imagine where a small child would have slept or been fed a meal amidst the chaos, it’s not a pretty picture. I also wondered where the occupants had gone and what led them down such a dark path. We hired a cleaning crew to take care of the place, man what a job that would have been. It cleaned up pretty well and I ended up selling it but it’s one I will never forget. Which I guess is a weird way to phrase: here’s some weird books I think you’ll like First we got A Botanical Daughter by Noah Medlock, a gorgeous(ly weird) Victorian gothic horror novel about an unconventional couple conducting some unconventional, Frankenstein-esque experiments. This one hits all my favorite notes: gothic, Victorian London, creepy, little shop of horrors-core queer goodness. Shoutout to @night_worms for sending this one in the April package 🫶🏼🫶🏼 Next we’ve got Chlorine by Jade Song, about Ren Yu, a teenage swimmer who will do anything it takes — spill any amount of blood — to make a life for herself where she can be free to be who she is. A mermaid tale in the grotesque tradition of the actual Little Mermaid, this sapphic coming of age horror is not to be missed. And finally, Bloom by Delilah S. Dawson. Described as a sweet sapphic romance that takes a dark turn — Rosemary “Ro” becomes obsessed with farmer’s market girlie Ash. As her fascination consumes her, the one doing the devouring becomes blurry…. Any of these sound good to you? Personally I want to read all of them now, at once, somehow, but I don’t have enough pairs of eyeballs to make that feasible. WTF Milia?! I have so many patients ask about “these white spots” that show up around eyes, on forehead etc. They are white, waxy and hard and simply trapped keratin skin cells 😱 BUT they are totally benign. We see them in babies and they’re no big deal, but as adults they can be annoying. As we age, our pores tend to enlarge, we have hormonal fluctuations and our skin isn’t turning over as quickly, all of which can lead to these bumps. It’s important to cleanse well daily, avoid a clogging moisturizer, and use an alpha hydroxy acid like alumiermd Bright and Clear, a retinol and a once or twice weekly Polish to keep those dead skin cells moving along. In order to get rid of them, I’ve had great results with MOXI which also helps shrink those pores down. What has helped you with milia?? Explanation below 👇 1. Retinol thins skin ❌ This is patently incorrect based on some very robust studies of skin histology which shows retinol increases skin thickness, elastin and collagen (citation below). 2. All retinols are the same ❌ False. The most potent is prescription tretinoin, which is the gold standard for acne treatment, however, this is often too potent and not well tolerated by patients wanting to treat general aging skin. Most patients do well with a retinol or retinaldehyde. 3. Redness and peeling mean I need to stop/conversely, I need to see redness or peeling to know my retinol is working. ❌ False. Some patients need to experience a little redness and peeling as they get started on their retinol journey as the anticipated side effect of retinol is increased cell turnover. HOWEVER, this is not always the case. If patients start low and slow with a couple times per week they may gradually adjust to their retinol and increase as tolerated. 4. If my skin is sensitive, I can’t use retinol. ❌False. Even some rosacea patients can benefit from a low dose retinol. Speak with you provider to see if you might be a candidate. 5. Retinol must be applied everyday to see results. ❌ False. Retinol is vitamin A which is a fat soluble vitamin. That means it resides in our cells (which is the reason we don’t allow pregnant or BF mamas to use it because it can accumulate). If you forget to use your retinol for a couple days but you’ve been regular with use, you likely have some built up in your skin cells. 6. You must stop using in summer. ❌ False! Retinol is NOT photosensitizing. It is not listed as such in any formulary. In fact, the reason it should be applied at night is because UV rays break most forms down rendering them inactive. Obviously, if you’re adjusting to a retinol and have redness and peeling, getting a sunburn is not a good idea! (But is it ever??!). Inspired by some messages from someone hoping to break into injecting and medical aesthetics. TBH I get these messages weekly. The internet can make this field look incredibly glamorous and I’ve heard from people that it looks “easy” and “less stressful”. While it’s true that this field can be SO rewarding it is also something that takes an incredible amount of time and personal effort and finance. I continue to pay for trainings yearly which amount to something upwards of 20-30k. I do this because I want to BE the best and bring the BEST to my patients. Injecting is high risk.. but also high reward. The looks I see on my patients faces when I’ve made them feel like their inside matches their outside is a unique fulfilling feeling that is truly unmatched. BUT you must remember that it comes with risks - especially when you are taking about another person’s face. Bottom line, if you don’t have the passion and drive you will not be doing it for the right reasons. drfelixbertram taught me the best lesson about finding your PURPOSE. If it is your purpose it will all fall into place and it will never be work. WTF is Baby Botox? 👶 Baby Botox is wrinkle relaxer that is geared towards a younger age, or dosed in smaller amounts. If a 28 year comes to see me, it is unlikely I will recommend they need 68 units which is somewhere close to the amount the pharmaceutical company recommends. Likewise, if a middle age patient really wants a very light treatment and they have muscles that aren’t very strong, they may be just fine with a lighter dose. The controversy comes in with DOSE 🟰 DURATION. The paper I referenced (below) combined 5 mostly randomized control trials examining Botox, Dysport, and Xeomin and reviewed the data on administration of large dosages (sometimes 4x the recommendation). Patients DID get longer treatment duration (sometimes up to approx a month longer) HOWEVER, large dosages are: ✔️ EXPENSIVE ✔️Complications like eye ptosis or droop MAY carry greater risk with large dosages because of diffusion. In my opinion, I treat Tox like any other drug I might administer as a Nurse Practitioner and I may titrate that according to a number of things- including patient anatomy, muscle strength, their job (ie. Would I give a first time lip flip to a trail lawyer presenting next week? 🤔 ) Most importantly, I consider the outcome my patient is hoping for. Now, bottom line, a full face of Tox CAN and DOES look natural if done well. And if a patient is not getting the duration out of their treatment, dosage would be one consideration. WTF! If your provider isn’t talking to you about risks related to any procedure you should run the other way 🚨 Although wrinkle relaxer, delivered by a trained medical professional, is relatively low risk, my patients and I always have this discussion. 🫣 Even the most well trained can have one of these adverse events or side effects and patients should be informed. Fortunately, none of these are permanent as wrinkle relaxer gradually wears off over 3-4 months. The most common fears I hear from patients are “Spock brow” or fear of looking “weird” or frozen 🥶 In the right hands, these shouldn’t be an issue as your provider will dose you appropriately.
