Nasieniak Pacjent lat 34, przerzut do lewej półkuli mózgu nowotworu jądra. Dwa masywne guzy spowodowały zaawansowany obrzęk mózgu oraz bezpośredni ucisk okolicy kory ruchowej (strzałki Mri). Przy przyjęciu pacjent bez kontaktu słowno-logicznego, z niedowładem prawostronnym. Podczas 5 godzinnego zabiegu udało mi się usunąć guz w całości (film) z pominięciem czesci masy martwiczej- nieaktywnej w okolicy ruchowej (Mri po zabiegu). Pacjent bardzo dobrze zareagował na leczenie operacyjne, niedowład wycofał się całkowicie, odzyskał pełną świadomość. 5 dni po zabiegu. Pacjenta w stanie ogólnym dobrym wypisano do domu. Konieczne jest dalsze leczenie onkologiczne. Tego t ypu guzy dobrze reagują na leczenie farmakologiczne. Seminoma A 34-year-old patient with a testicular cancer metastasis to the left hemisphere of the brain. Two massive tumors caused advanced cerebral edema and direct compression of the motor cortex area (MRI arrows). On admission, the patient without logical contact and had right-sided paresis. During the 5-hour procedure, the tumour was completely removed (video) with the exception of the inactive necrotic mass in the motor area (MRI after the procedure). The patient responded very well to the surgical treatment, the paresis disappeared completely and the patient regained full consciousness. 5 days after the surgery, was discharged home in good general condition. После операции легкий парез мимической мускулатуры НВ 2 , к моменту выписки полный регресс (лицо симметричное ) . Я всегда стараюсь показывать фото лица ( с согласия пациентов) при вестибулярных шванномах когда сохраняем лицевой нерв , но в данном случае пациентка против показа фото лица , поэтому фото лица не будут показаны. Dysplazja włóknista kości czaszki Zmiana okolicy skrzydła większego kości klinowej (białe strzałki w badaniu MRI). W kolejnych badaniach radiologicznych powiększenie się zmiany z kompresją w obrębie kanału kręgowego nerwu wzrokowego. 25 letnia Pacjentka podjęła decyzję o usunięciu chirurgicznym zmiany jeszcze przed pojawieniem się objawów ze strony nerwu wzrokowego (zaburzeń widzenia) Podczas zabiegu użyłem neuronawigacji przymocowanej do wiertarki szybkoobrotowej, co umożliwiało pełną kontrolę punktów anatomicznych, tak aby odbarczyć nerw wzrokowy bez uszkodzenia ściany oczodołu. Całość zabiegu to 5 godzin (film na końcu widoczny odbarczony nerw wzrokowy) Celem zabiegu było odbarczenie nerwu wzrokowego prawego (niebieska strzałka nerw wzrokowy w badaniu kontrolnym) co spowoduje że zmiana nie będzie mogła go uszkodzić. Całkowite usunięcie zmiany nie było konieczne a wręcz mogło spowodować osłabienie struktury kostnej czaszki. Pacjentka 3 dni po zabiegu w stanie ogólnym dobrym, bez objawów neurologicznych została wypisana do domu Fibrous dysplasia of the skull Lesion in the area of the greater wing of the sphenoid bone (white arrows in MRI). Subsequent radiological examinations showed enlargement of the lesion with compression compression of the optic nerve. A 25-year-old female patient decided for surgical remove of the lesion before the appearance of optic nerve symptoms (vision distrubances) During the procedure, I used neuronavigation attached to a high-speed drill, which allowed full control of the bone points. So as to decompress the optic nerve without damaging the orbital wall. The entire procedure takes 5 hours (at the end of the film decompresssed optical nerve) The aim of the procedure was to decompress the right optic nerve (green arrow - optic nerve in the follow-up examination), which would prevent the lesion from damaging it. Complete removal of the lesion was not necessary and could even weaken the bone structure of the skull. The patient was discharged home 3 days after the procedure in good general condition, without neurological symptoms #neurosurgery #neuronavigation #bonedysplasia #microsurgery

Various methods currently exist to guide fibular osteotomy positioning in fibula free flap mandibular reconstruction, but patient-specific navigation methods and cutting guides require experience and may be time-consuming and/or expensive. In this #ijoms article, the authors describe a novel robot-guided osteotomy technique for mandible reconstruction using a fibula free flap according to virtual preoperative planning. The method was assessed on five 3D-printed models and a cadaveric model.
