2.5 weeks until we head in for palate surgery! . For those of you new here, @collette_cutiewithacleft was born with a cleft lip and palate. This happens in the womb during the first few weeks of formation. The tissue does not connect and leaves an opening in the lip/nose and palate. Due to this, Lotty Dotty will have multiple surgeries to correct the cleft. She had her lip surgery at four months old and will be just 1.5 weeks shy of a year for her palate surgery! After surgery we hope to learn to suck out of a straw, start pronouncing sounds and words that require the roof of her mouth and maybe eating without it coming out of the nose 🤭. . Right now we’re not focusing on that though! We’re enjoying the days at home with big brother and sisters, working on drinking from sippys and open cups and tiring mommy out by crawling, pulling up and not sleeping because we’ve got some teeth coming in! . A submucous cleft palate is where the palate may look relatively unaffected, but the muscles that lift the palate for speech and feeding are impacted. What are the classic signs of a submucous cleft? ➡️ The uvula (the dangly bit at the back of the soft palate) is split. ➡️ There is a bony gap at the back of the hard palate ➡️ The midline of the soft palate may look very thin - translucent. She hears 🙌 Hon hör! Igår var vi på hörselkontroll och konstaterade att rören i öronen fungerar och nu hör Juni. Barn födda med spalt får ofta stora mängder vätska bakom trumhinnan beroende av innerörats utformning och vinkel. Vilket blir blir bättre i takt med att ansiktet växer. Detta innebär att Juni kommer behöva rör som barn men mest troligt kommer vara fullt hörande som vuxen. Jag är så glad och lättad! Äntligen positiva besked! Vi älskar dig Juni 🩷 She hears! Yesterday Juni had a hearing test and er found that the hearing tubes in are working and finally Juni can hear. Children born with a cleft often get large amounts of fluid behind the eardrum depending on the shape and angle of the inner ear. Which gets better as the face grows. This means that Juni will need hearing tubes as a child but will most likely be fully hearing as an adult. I am so happy and relieved! Finally positive news! Winn here! Mahm took me to White Sands. It was unclear what that meant until I saw all the white sands! Mahm and me take our duties as Junior and Bark Ranger very seriously! Let’s talk about changing faces. There was at a lot of focus on this at the recent @americancleftpalate conference because it’s a tough topic! As a plastic surgeon I’m taught to restore form and function. As a person who cares about people with facial differences I also think it’s important to embrace the unique parts of someone’s identity. My goal as a cleft and craniofacial surgeon is to help kids live their best lives and grow up to be happy successful adults—not to erase differences. Restoring function of the mouth and face involves realigning anatomy. Setting someone up for good function usually alters form in a way that “normalizes” it. It can be difficult to talk with patients and parents about surgeries that change appearance. Some people are excited about what they see as improvements. Some are nervous about change. Sometimes people can even be offended by the suggestion that change would benefit them/their child. All of these responses are valid! I’d love to hear from people who have experienced these discussions about what worked and what didn’t. Happy 7th Smile Anniversary to this amazing kid! For those who don't know, Jack's Smile Anniversary is celebrating the surgery day where he got his "forever smile." Man, that day was tough. Sending our tiny baby back for a surgery like that was almost more than my heart could handle. Then not recognizing him right away in the recovery room was extremely difficult. But we did it and are all so much stronger for it! We fell in love with his first smile, and then got to fall in love with his new smile all over again. This kid is strong, resilient, hilarious, and gentle hearted. It sometimes doesn't feel fair that he experiences all of these cleft-related things, but we absolutely know he can do it, and so can we. These kids go through more than the average kid, and it develops a deep seated strength that is amazing to observe. Who doesn’t use a Dalmatian’s rump for a pillow? 🤣 These two are just so cute😍 I love seeing our animals all getting along and loving each other. I’ve been quiet here bc a lot of times when I don’t have puppies, it seems pointless to post 😂 there’s no late night feedings, talking about what we do for these neonates. But then I remember that I have a cleft dog sitting next to me all the time. I honestly forget that Roger our Dalmatian has a very significant hard and soft palate cleft. 🥲 you know why? Bc he’s normal in every way. I see in a lot of the Facebook groups about breeding or whelping or even the cleft ones, about their “quality of life” when there is a cleft born. Even the vet clinics talk about it. And if you know me at all, you know I despise the term “quality of life” bc it’s incredibly subjective. How one person feels about something can be very different to another. Tube feeding to one person can be a terrible thing, while those of us that do it all the time have no problems with it. So because of that, I don’t like using that phrase. These puppies can have an amazing life. 🙏🏼But there are those that have many more medical issues that therefore they do not make it. 😕But none of us can tell someone when they look at a day old neonate that it’s life is going to be fine. You have to take a chance and try. We will always try. I look at Roger and if his breeder decided it wasn’t worth it. He would not be here. She was turned away from being taught to tube feed 🤦🏻♀️ bc the opinion was that Roger would not have QOL. So she syringe fed him and he thankfully made it to 5 weeks before he came to me. It’s hard being an RVT and being in this sort of rescue. But after 4 and 1/2 years of saving these neonates, I know there is so much more work to do to bring awareness🖤🤍 📷 last two photos credit @sweetcamelliaphotography for her beautiful work!🙌🏼

*MTHFR* If this isn’t the best acronym, I don’t know what is. It took all of my effort to not use the key word from this song to go with my post 🙃 Methylenetetrahydrofolate reductase is a gene that all of us have. A huge percentage of us are actually affected by a mutation within this gene. A gene mutation can impact an individual hugely. I’m talking emotional stability, neurotransmitters, how the body can detoxify (meaning constant inflammation) and it can be associated with midline abnormalities (cleft?) to name a few. There a few different mutation variants but the amazing thing is that we can support the body and brain with lifestyle, nutrition and supplements. Talking of supplements: this mutation could be worth discussing if you’re cleft affected and wanting to start a family - as I understand it, the mutation can affect how you process folic acid. In the next couple of days, Will will be doing his test. I have chosen to pay and do an at-home kit. Why the test? There’s a couple of reasons! To establish if he needs help to detoxify and reduce inflammation, supporting his neuro and emotional development and for future if he ever wanted babies! Would I do the test? Absolutely. But as it’s paid for, Will is priority! Please don’t take this post as a trigger to call your doctor or worry that you or a loved one are affected. However, it may be something to consider and I do wish I had known more about it before now. To finish.. if I have the mutation and that means my body isn’t able to detoxify properly…….. is this why my hangovers have always been horrendous??!!!
This patient can breathe like never before! She has had several previous surgeries on her nose by other surgeons. Unfortunately, she had severe nasal obstruction and apnea. She also hoped for improved nasal symmetry. We performed an open cleft rhinoplasty, including release of scar tissue and reconstruction with multiple cartilage grafts (spreader, septal extension, columellar strut, lateral crural strut, and tip grafts). MTF cadaveric cartilage was used after discussion pros and cons of using her own rib versus donor cartilage. She is seen here at just 4 weeks, so the swelling will still go down another 50%. Fat graft was also performed for the upper lip, to add more bulk to begin balancing that with the lower lip. Jaw surgery will be performed in the future. Cleft rhinoplasty is usually performed after jaw surgery. However, in this case, her breathing obstruction was severe enough to warrant doing the nose portion sooner.
