After Tommy John Accolades Molitor played a total of 1,900+ games after Tommy John Surgery, leading the league in hits 3 times, earning 6 All-Star game selections, and ultimately finishing with 3,319 career hits and a .304 batting average. I do lots of consults and rehab on those having their 2nd, 3rd, and 4th elbow surgeries. Here’s a little secret: when I'm prepping for those consults, I do some digging on previous training, throwing, and therapy videos. Under most circumstances, we know the surgery fixed the tear, so the primary moving parts are proper therapy to restore function, proper training to restore strength and optimal biomechanics, optimal authentic throwing mechanics, and adequate ramping and adaptation of throwing volume. In most of these cases, I find many of the following: -An aggressive trail leg hip flexion (examples #atlhf) -Inverted W at FFC -Returning to mound work before long toss and pulldowns have been fully reintegrated at max effort -Poor lunge form -Poor cervical rotation -poorly executed bench form (why are you straight bar benching?) -Full elbow extension not restored -2/3 reps when performing chin-ups (see the overlap there?!) -Pain in the elbow after more than a year post-op (this one drives me insane) -Poorly executed squats or box squats (it’s always the 1/2 depth box squat, I think it relates to never letting the hip flexors door their job) -Breakdown of lunge and deadlift form at 90%+ of 1RM -"Ibuprofen, I be playin" in their commonly used phrases What did I miss? #throwingprogram #tommyjohnsurgery #longtoss #90mphformula #athletefirst

Ok let’s see if this thing works. I went to the Ortho Dr cuz my elbow has been bothering me lately from an injury I got back in 1983 when I worked for the PD and I’ve had 8 surgeries on this elbow and lately my fingers have been tingling and the swelling in no bueno. So come to find out I need a total elbow replacement now and the cartilage is gone and the ulnar nerve is entrapped so he wants me to wear this at night to keep the elbow straight. Well this elbow doesn’t really straighten anymore but I’m hoping this helps me cuz I don’t want the Tommy John surgery that I need. When I sleep I sleep with the elbow bent and when I wake up in the morning the elbow is pretty much useless so I’m hoping this helps! Matt Morris is a former @mlb pitcher who played for the @cardinals and @sfgiants spanning a 13-year career. He was selected 12th overall by the #cardinals in 1995 and was considered a top of rotation mainstay for nearly a decade. Tommy John Surgery Morris would by plagued by elbow discomfort before the 1999 season and finally ruptured his UCL in Spring Training. He would undergo #TommyJohnSurgery in 1999, becoming sidelined for the next 14 months. Return to Play Morris would return to the mound midway through the 2000 campaign for the Redbirds, regaining his form leading up to the 2001 season. After Tommy John Accolades Morris went on to lead the league in wins (22) his first full season after surgery, receiving two consecutive All-Star selections in 2001-02. Retiring in 2008, Morris's career logged 7 consecutive double-digit win seasons, 1,400+ innings, and 10 post-season series. #TJ50 #MLB

My biggest piece of advice is NOT to do any aggressive deep tissue work around the incision. ⚡️I don’t believe in the “theory” that scar tissue needs to be broken up. Drafted in 1989 by the @Yankees, Sterling Hitchcock made his @MLB debut with the team in 1992. After a stint with the Seattle Mariners in 1996, Hitchcock was traded to the @Padres where his memorable playoff performances secured his spot in baseball history. During the 1998 playoffs with the #Padres, Hitchcock was the winning pitcher in both of his starts with a 0.90 ERA. Over the Padres playoff run, he went 3–0, allowing just 3 runs in 22 innings with 32 strikeouts—and was named the NLCS #MVP. Hitchcock’s successful years with the Padres were followed by struggles with injuries. He underwent #TommyJohnSurgery in 2000, performed by Dr. Lewis Yocum, and returned to the majors in 2001 with the Yankees. Hitchcock would go on to be the winning pitcher for the #Yankees in Game 5 of the 2001 World Series. Sterling Hitchcock finished his career with the San Diego Padres in 2004, tallying a total of 997 strikeouts and a 4.80 ERA over his 12 years in #MajorLeagueBaseball. #TJ50

This one is for all the bros and 🤡 out there that read the headline of a post or just the summary of a journal article. Yes— we have loads of data that is shows a significant link to hitting 1.5x bw reverse lunge, 2x deadlift, and 13+ bw chin-ups and being less likely to have a UCL surgery. No— this data does not mean we can stop every UCL injury. I absolutely believe that this information should be used as a baseline for athletes looking to maximize their performance and limit their injury risks, but this doesn’t mean that mechanics (past and current), health history, muscle mass, workload, etc don’t play a massive role in elbow injuries. And if you look at how we train our athletes, you’ll see that it’s much more comprehensive than just hitting the formula metrics, although that is a great first step. Of course, when I see a big league guy that throws the piss out of the ball and is 50lbs under our baseline bodyweight metrics for the #90mphformula, I always wish he was bigger to help give him a higher ceiling and decrease injury risk factors, but that doesn’t mean I think he’s going to blow his elbow up. I do believe it is sub-optimal, and I’m never surprised when they do have unreliable velocity and durability over a season, or they do blow their elbow up, but don’t let that mislead you to think in absolutes. #tommyjohnsurgery #uclsurgery #sportsmedicine #90mphformula #90mphformulastudy #dontletsciencehurtyourfeelings

4/29 巻き肩改善の考察 結論：巻き肩改善には肩の前面(小胸筋等)だけではなく、 後面(大円筋等)をほぐす事が重要と思われる。 手術後、患部(右肘)を庇うために自然と右肩に力が入り、巻き肩の状態になってしまい、動画のような肩の内旋動作時にゴリっと音が鳴り痛みがあった。 今までは肩の前面(大胸筋、小胸筋)をほぐして改善を図っていたが効果はあまり出ていなかった。 しかし、今日のリハビリ時にストレッチポールにて肩の後面をほぐしていたところ、巻き肩が改善されて痛みやゴリっという音が生じなくなった。 肩の後面にある大円筋、広背筋は背中から上腕骨の前面に付着しており、これらの筋肉が硬結していた事により筋収縮が続き、肩が前に捻り出された状態＝巻き肩になっていたと思われる。そして今日、これらの筋肉にアプローチが出来たため、自然と巻き肩も改善したのではないか。 ネット上の情報やストレッチ指導などで、今までの経験上巻き肩改善には肩の前面をほぐす事にフォーカスを当てられていたように思うが、実は逆の後面をほぐす事も同じくらい大事な事かも知れない。 #陸上競技 #やり投げ #トミージョン手術 #javelinthrow #tommyjohnsurgery

For this week's #TommyJohnSurgery 50th Anniversary spotlight, we're looking at one of the most dominant pitchers to ever play in the @mlb: Kerry Wood. Wood played in all or part of fourteen seasons, coming to prominence as a 20-year-old rookie striking out a record-tying 20 batters in a single game against the #Astros—and later being named National League Rookie of the Year. Bothered by elbow soreness during his 1998 rookie year, Wood would later rupture his UCL in spring training of 1999, requiring Tommy John. The surgery, performed by Dr. Frank Jobe's contemporary and renowned orthopedist, Dr. James Andrews, would bench the rookie phenom causing him to miss the 1999 season. With advances with the now common procedure, Woods returned to the #mlb after a little over one year of rehab and would go on to record 23 starts and log nearly 140 innings on his reconstructed elbow in 2000. Woods returned to full form in 2001 and didn't miss a single start during the campaign. He would total 13 more seasons after surgery and later transition from starter to reliever. He was selected to the NL #AllStar team in 2003 and 2008, leading his team to the NL Central Division crown in back-to-back years in 2007-2008 and would appear in 6 post-season series. #TJ50

Tommy John Surgery Wells would pitch 21 seasons for 9 different clubs in the #MLB, but was originally drafted by @BlueJays in 1982 out of high school. Wells suffered his UCL injury while in the #BlueJays' minor league system in 1985 and would undergo the reconstruction procedure, setting him back a year. The surgery was performed by a Frank Jobe, MD, contemporary and renowned orthopedist, James Andrews, MD. Return to the Mound His teammates affectionately nicknamed him "Boomer," and he would return to pitch after a prolonged rehab period with his newly reconstructed elbow in 1986 and make his Major League debut in 1987 for the Toronto Blue Jays, marked by a successful stint with the upstart Blue Jays and post-season runs from 1989 - 1992. After Tommy John Accolades Wells' @MLB career would span two decades after Tommy John surgery, appearing in 660 games, 239 career wins, and over 3,400 innings pitched. He made 17 postseason appearances including two World Series championships and was selected to three All-Stars games selections. Wells may be best remembered for his performance on May 17, 1998, when he pitched the 15th perfect game in baseball history, when he blanked the Minnesota Twins, 4–0. #TJ50

Baseball players expose their elbows to a tremendous variety of forces during the whole act of throwing. Normally there is an amazing balance between the muscles, tendons, ligaments, and bones that allow these athletes to perform painlessly. But sometimes that balance is lost, and injury may occur. One of the most devasting elbow injuries involves the ulnar collateral ligament (UCL). Numerous studies have shown that the UCL is the primary stabilizer of the elbow during the act of throwing. When an injury such as partial or complete tearing occurs, baseball players, particularly pitchers, are often unable to throw competitively. This ligament may be injured with one throw (acute injury) or progressively over some time (chronic injury). How does injury to the UCL present in the thrower? On this day in 1976, Tommy John returned to an #MLB mound after a layoff of 18 months, 22 days, becoming the first pitcher to recover and return from UCL reconstruction—known today as #TommyJohnSurgery. On Friday evening, April 16, 1976, at Atlanta-Fulton County Stadium, in front of 15,825 people, 639 days after a projected career-ending UCL injury, Tommy John made his official return to a major league mound. Although Tommy John would pick up the loss, it really was an amazing pitching performance. Still needing proof, Dodgers manager Walter Alston told John that he would get only one more chance to prove he still could pitch at a major-league level, John would throw seven scoreless innings in his second start and pitched seven innings, allowing only one run on his third, winning his first game since July 7, 1974. He finished the season 10-10 with a 3.09 ERA. In 1977 he was 20-7, in 1978 he was 17-10, in 1979 he was 21-9, and in 1980 he was 22-9. He ended up pitching 14 seasons after surgery totaling MLB 288 victories. Once projected to have a 1% chance of ever pitching again, with his resolute and reconstructed elbow innovated by Dr. Jobe, John was the first of hundreds of careers saved through Tommy John surgery. #TJ50

From his early days in the minors to becoming a top-of-rotation fixture in the majors, Colby Lewis’ resiliency marked his career in baseball as one of persistence and resolute. Undergoing #TommyJohnSurgery at age 16, Lewis was considered one of the first prominent @MLB players to undergo the surgery while in high school, performed by Dr. Frank Jobe. He would battle through other baseball injuries and surgeries, but would overcome long odds to achieve a successful baseball career. Making his #MLB debut with the Texas @Rangers in 2002, Lewis would eventually be sent back to the minors and even do a stint with the Hiroshima Carp of #Japan’s #CentralLeague. Lewis would make his return to the majors in 2010 again with the #TexasRangers, leading them to the club’s first-ever #WorldSeries appearance while earning the team their first-ever World Series game win the same year. Lewis made his last MLB appearance on October 1, 2016, with the #Rangers— logging more than 1,200 innings and nearly 1,000 strikeouts on his reconstructed elbow. #TJ50
Little League elbow is an overuse injury commonly seen in young baseball players, particularly pitchers. It results from repetitive stress on the growth plate of the inner elbow due to the repeated throwing motion. Symptoms include pain, swelling, and tenderness on the inner side of the elbow, which may worsen with throwing. Rest, rehabilitation, and gradual return to pitching are key components of treatment. Pitch counts Innings limits Pitch clock Increased velocity Increased ball movement Removal of sticky stuff These variables all can be minimized as risk factors if you possess: -optimal authentic mechanics -adequate total body volume tolerance -adequate localized tissue tolerance -adequate total body/muscle mass -aqequatly primed nervous system These are all factors I've been talking about for 10 years with the #90mphformula. It's not a few exercises put together at random, its a system of checks and balances to mitigate these types of injuries. As a bonus, if you attack the above qualities correctly, you'll clean up those yips. This week's #TJ50 spotlight, Norm Charlton, was drafted in the first round of the 1984 #MLB draft by the Montreal Expos and would later make his MLB debut in 1988 with the Cincinnati #Reds. He was best known as being part of the infamous “Nasty Boys” relief pitching corps for the 1990 Reds team who won the World Series and was coined "the Sherriff" by his teammates. He would later be diagnosed with a ruptured UCL and underwent #TommyJohnSurgery mid-way through the 1993 season. After surgery under the care of Dr. Frank Jobe in July of 1993, requiring the entire 1994 season for rehab, he would return in April of 1995 and be involved in a mid-season trade that would make him the Seattle Mariners closer and contribute to the first ever post-season appearance by the "refuse-to-lose" franchise that year. Charlton went on to pitch an additional seven years after returning from Tommy John in 1995 and appeared in 330 games and 900 innings pitched on his reconstructed elbow. He made appearance in five post-season series including his role in the Mariners' record-setting year of 2001 that would set the #mlb record for most regular season wins at 116 that catapulted them to the ALCS. Charlton retired from baseball after the 2001 season with a career record of 51-54, an ERA of 3.71, 808 strikeouts, and 97 saves.
