The diagnostic criteria for migraine is complicated. We still do not have an imaging test or biomarker to definitively diagnose migraine, so we must rule out other conditions and compare them with a clinical history. Migraine attacks have dozens of symptoms that cannot be fully encapsulated by a short definition. This diagnostic criteria for migraine without aura says that the patient must have experienced at least 5 attacks with the following criteria and cannot be better explained by another diagnosis. The headache attacks must last from 4- 72 hours; include at least 2 of the following qualities: unilateral location, pulsating quality, moderate or severe pain intensity, aggravation by or causing avoidance of routine physical activity; and must include 1 of the following features: nausea or vomiting, photophobia and phonophobia. Let’s talk about imaging scans for migraine. It can be frustrating when you know something is wrong, yet no results appear on scans or blood tests. In fact, normal scans are required for a migraine diagnosis. Know that a normal, or negative, scan does not mean your pain is not real. Migraine does not appear on routine CT or MRI scans. Normal results rule out certain conditions, but they do not discount your symptoms. It’s natural to want solid proof of our diagnosis even just to ease our nerves that something “worse” is not present. WARRIOR OF THE WEEK ⭐️ This week’s warrior is Gaylene. Here’s her story… “My name is Gaylene and I am from the Eastern Sierra in California. I think my ON is caused by the mass on my pituitary gland, not sure yet. I started experiencing ON symptoms in early 2021 and was diagnosed the summer of 2023. I was diagnosed by my general physician, psychiatrist, and neurologist! I also have another family member who has ON! My ON symptoms are specific to the right side of my neck, side and back of head. I experience nerve pain, neck aches, muscle spasms, and arm numbness. I also have chronic migraines with aura and can have both types of headaches simultaneously. Before I found an effective treatment, I was in fluctuating pain 3 out of 4 weeks in a month. In the past I have tried many Calcitonin agonists for treatment with no luck. Currently Botox injections are reducing my pain greatly. My advice to other ON Warriors is that you are your greatest advocate for you health, be firm in your approach. Even though it’ll be surgery number 42 tomorrow, the nerves never get any easier. Was trying to distract myself with the view from the hospital neuro ward earlier before it got dark. Now just too anxious to sleep. Although I have a permanent intracranial pressure sensor in my brain due to the complex hydrocephalus and isolated cystic fourth ventricle, the brain just doesn’t work as it should causing quite an issue with horrible symptoms as it currently is so a more thorough intracranial pressure monitoring is needed.
