In February 2024, the FDA approved lifileucel (LN-44), a tumor infiltrating lymphocyte (TIL) therapy for advanced melanoma. This is the first of its kind for solid tumors and represents a significant step forward for melanoma treatment. TIL therapy harnesses a patient's own immune system to fight cancer. Doctors extract immune cells from a patient's tumor, grow them in a lab, and then reinfuse them back into the patient. These TILs are specifically targeted to fight the patient's unique cancer cells. This therapy is particularly promising for patients whose melanoma has not responded to other treatments like checkpoint inhibitors or targeted therapy. Studies have shown a 32% response rate among patients who received TIL therapy, and over half of those who responded maintained that response for more than 3 years. Jose Lutzky, MD, a skin cancer physician at Sylvester Comprehensive Cancer Center, emphasized the significance of TIL therapy as it offers a new option for patients with limited treatment choices and builds upon the success of previous immunotherapies in melanoma. Sylvester is also participating in a clinical trial to further improve TIL therapy by genetically modifying the immune cells to enhance their effectiveness. While TIL therapy offers great promise, it is not without challenges. The treatment process can be demanding, involving surgery, chemotherapy, and post infusion treatments. However, the potential benefits outweigh the difficulties for many patients. The approval of TIL therapy marks a new era in melanoma treatment, offering hope for patients with advanced disease. Home to cutting-edge technologies, prestigious research institutions, and unrivaled health care facilities, the Lone Star State continues to lead the way in the diagnosis and treatment of many ifferent types of cancers. While cancer of any form can be a difficult and scary diagnosis for both patients and their families, melanoma can impact all Texans and Americans, regardless of race, ethnicity, or gender. Melanoma is a skin cancer most often caused by ultraviolet rays, and those with weakened immune systems or a family history of melanoma face a heightened risk. This type of cancer can be prevented by using sunscreen, wearing protective eyewear and clothing, and limiting sun exposure. Early detection and prevention measures are critical to defeating this cancer, and many patients have found effective treatments that win the battle. In Texas, we know that it is not our challenges that define us, but rather how we rise above them. In the summer of July 2022, I left my previous career and took some time off. I cashed in my 401(k) and lived off my credit cards and I didn’t know how but I knew I wanted to be an inspirational speaker. When there is no blueprint, you gotta create one. I believe everything happens for a reason and facing my mortality is the best thing that ever happened to me. It forced me to see whats truly important, time. You cant rewind it and you cant get it back. I used to pride myself on my 800+ credit score, everything I did was based around finances. I was always chasing the idea that once I was debt-free and had $ in the bank that I could move forward in life. I was wrong. My current credit score is horrible, in fact, I don’t even have any credit anymore or even two nickels to rub together. I believed in myself, and so I invested in myself, and because of that, I’ve been able to share my story and help others globally which is priceless. There is the saying that if you can’t help yourself, you can’t help others which is another reason why I wrote and self published this book. I also have to generate an income. The long-term financial toxicity of a cancer survivor is a whole other post but there’s no financial discounts or rewards for being in a clinical trial or surviving cancer. You still have the same finances as the rest of the population always has but now you also have the additional cost that adds up when you hit your out-of-pocket deductible for your insurance every year. Day 2 of 3 in Washington D.C. with my #MelaHomies. #IYKYK but becoming a MelaHomie is not something anyone seeks out or asks for. To become a member you have to be loud, proud, and feral as F*** as a voice for something bigger than yourself, leaving all Ego at the curb. The reward is the gift of connection, compassion, and empathy b/c the lived experience 1st hand is not only what people want but what they seek. Being able to provide this patient to patient perspective is made possible b/c of the technology of what is today, the present but also the future. It’s 2024 and the internet is in everyone’s pocket and the landscape of engagement is in the hands of everyone. Life is moving forward, not backward which is why I stress the need to adapt, and overcome. You can’t change the past, but you can do your best to prepare for the future. It all boils down to being proactive instead of reactive. “Experts fail b/c they are experts in a world that no longer exists”. As the #PatientExperience holds a value that is so priceless, nothing else can or will ever compare to bonds formed when someone just gets it. With time comes change and with change is the evolution that the way things used to be, is not the way that things are and that’s okay. That’s why I always stress that It’s so critically important to become comfortable with being uncomfortable, coming outside of yourself, and knowing the worth of your story will be the source of change in someone’s. Everywhere I go and everything I do is so I can be a source for the next person, provide more than what I had when I was in that position in 2018-2019. Very cool to meet Denise Hamilton yesterday at the NSA North Texas monthly meeting. Loved her perspective on “Sharing Your Message in a Noisy World” as her content for the morning was directly aligned with adapting and evolving to what is and mot resting comfortably in what was. A couple of notes I took in the morning really resonated with me. -“It’s. Unlocking the Potential: Understanding TIL Therapy for Solid Tumors What is TIL therapy? According to the immune editing theory, during the process of tumor development, the body's immune cells always play a role in killing tumor cells. Different immune cells (mainly lymphocytes) are transported to the tumor site, and these lymphocytes that have infiltrated into the tumor tissue and exerted cytotoxic effects are called tumor-infiltrating lymphocytes (TILs), including T cells, B cells, NK cells, macrophages, and various mononuclear and multinuclear immune cells, which can recognize, resist, attack, and kill tumors. TIL therapy belongs to Cellular Immunotherapy, which is a method of treating using immunocytes that are activated, proliferated, or genetically engineered. The main steps include isolating specific immune-active cells (including T cells, NK cells, DC cells, and macrophages) from tumor patients, genetic modification or amplification in vitro, functional identification, and finally, returning for treatment. TIL therapy refers to isolating tumor-infiltrating lymphocytes from tumor tissue, culturing them in vitro, expanding them massively, and then reinfusing them into the body for treatment. The main routine steps of TIL therapy include: 1. Obtaining patient tumor tissue: mainly obtained through surgery or biopsy to include TIL cells. 2. Grinding tumor tissue: This step makes it easier for T lymphocytes to grow and expand from tumor tissue. 3. Adding interleukin-2 (IL-2) for cultivation: After adding a high concentration of IL-2, on the one hand, it provides survival signals for existing TIL cells, and on the other hand, stimulates larger-scale proliferation. 4. Conducting tumor-specific recognition testing: By conducting tumor-specific recognition testing on TIL cells, screening for TIL preparation processes that can more specifically kill tumors to ensure treatment effectiveness. 5. I’ve rang 3 Bells throughout my #Cancer Journey. The first was at @mdandersoncancercenter in Houston TX after I completed my radiation treatments targeted at my primary source of cancer in October of 2018. The 2nd was at @cuanschutz in Aurora CO after I completed my #TILtherapy treatment and was released from the hospital on January 20, 2020. And the 3rd was at @texas.oncology.foundation in Plano TX in January 2021 when I was going over a second PET scan that was confirmed zero uptake and that I did have a complete metabolic response. Since today is my 4 year anniversary from this date from finishing my TIL Therapy and being released from the hospital, it gives me chills. To everyone involved from the doctors, oncologist, nurses, and especially my family, I can’t thank you enough. I love you. Also, sending a giant thank you out to #IovanceBiotherapeutics, Inc. When I was admitted to the hospital on January 8, 2020 I was wheelchaired in there and could not walk. The fact that I was able to walk out of the hospital is a miracle and means so much. On this Day in History, December 10, reflecting on the following events. 2005-Graduated from University of Texas at Dallas with a Bachelors of Arts Historical Studies 2015-Member Services for the golf department at GlenEagles Country Club in Plano, TX 2019-(picture not shown) Received news from baseline scans for #clinicaltrial for TIL therapy that I had a 1 cm metastasis to the right occipital lobe of the brain. Told I was excluded from the clinical trial due to metastasis, and should consider calling hospice and settling my affairs. With this being a phase, 2 cohort 4 clinical trial, only way to get back in the trial and have a chance to live was to stabilize or reduce brain metastasis. 5 sessions of SRS Radiation needed but insurance approval needed 1st to have on schedule to move forward. Scans to show improvement had to be completed on or before December, 24 2019-14 days. After 6 days, Insurance approval was granted. 5 sessions completed in 4 days and scanned completed on December 24, 2019. 2021-Follow up appointment with Dr. Le at Texas Oncology. Celebrated over 1+ year of CMR (complete metabolic response) 2023-Blessed to be Alive, Healthy, and continuing to celebrate life thanks to #TILtherapy which eradicated stage 4 Terminal #MucosalMelanoma. What still hits home every time I share my story is that I believed in myself and never gave up. Cooper First: Researchers at MD Anderson Cancer Center at Cooper are leading the way in new approaches to treating advanced melanoma using tumor infiltrating lymphocyte (TIL) therapy. Earlier this year, MD Anderson at Cooper became the first site in the world to offer and enroll a patient in TILVANCE-301, a new randomized Phase III clinical trial evaluating the efficacy of #TILTherapy as a first line treatment versus pembrolizumab (current standard of immunotherapy care) for advanced #melanoma. “TIL therapy is a promising, personalized approach to cancer treatment,” says Young Ki Hong, MD, MPH, surgical oncologist at MD Anderson at Cooper, and principal investigator of several TIL studies. “Our patient has had a remarkable response to this therapy. I guess call it a #transformationtuesday Each of these pics where taken within 1 week of this exact same time each year. I exist because of one single treatment that worked when all others had failed. A literal “one and done” 1 time treatment #CellTherapy treatment called #TIL or #TILtherapy which stands for Tumor Infiltrating Lymphocytes. Everyone naturally already has these in them but when you extract them, grow them into the billions, bring your White Cell Count down to Zero, and then you get them back along with a little jump start from some IL-2 to get them moving. As a rare cancer survivor of #MucosalMelanoma, I do everything I can to advocate and support in every shape, form and fashion that I can. I connected with Janelle, in March of this year. She reached out to me directly, seeking advice about this extremely #rarecancer and I shared with her everything that I could, and that I know. Larry was actually accepted into the #TILtherapy program with the expanded access, compassionate use through Iovance and was set for his extraction in October but unfortunately that did not happen due to some unfortunate circumstances. Larry passed away yesterday, and when Jenelle told me, my heart dropped. I cannot begin to explain how devastating it is to lose another survivor from this already rare community. Jenelle reached out to me and asked where to donate to and I recommended the @melanoma RARE Registry and I set up this #JustGiving page for her. The past couple days have been a whirlwind. I got my last IV, and had a central line catheter put in (no more needle pokes!). Today I checked in for my inpatient stay, and currently getting lympho-depleting chemo to prep my body for my TIL infusion next Friday. This is my first overnight hospital stay and it’s been interesting today. I’m only here for tonight, and will her discharged tomorrow. So excited to finally be at this step in my treatment! In honor of #breastcanceawareness month and as a cancer survivor, I also support other cancers, including #breastcancer. After my #clinicaltrial for #tiltherapy, around the same time, I was discovering I had a complete metabolic response from #mucosalmelanoma and was at a dermatologist getting checked, and felt some lumps in my chest. Apheresis phase of TIL trial completed yesterday! I was so nervous about them putting in the catheter so I asked for sedation. I honestly never even felt different with the drugs, but the process was so quick and easy, that whatever amount of lidocaine they shot into me was enough to get me through with no issues. The procedure only took about 5 hours (I thought it would be closer to 8 hrs) and was easy peasy. The removal of the catheter wasn’t too bad but the site is very tender. Now I just have some slight restrictions for a few days and I’m good! I start my bridge therapy chemo today for 21 days to cover some of the gap since things won’t be ready until Oct/Nov. Hello to Everyone, “I’m out the sunroof yelling thank God it’s Sunday, y’all let’s show a little love” Yeah, everyone that knows me knows my favorite thing to do in this world is share my story of cancer survival. When you get diagnosed with metastatic malignant Mucosal Melanoma, the only thing that shows up when you re-search it says that you have a 14% or less chance of surviving five years and once you get to stage for your odds are less than nine months. Well I’m approaching my five years since diagnosis and I still have no clue and neither did the doctors on what caused it or why none of the treatments that I tried worked until I got into a clinical trial. This RARE registry right here is going to be the tool that the doctors in researchers can used to collectively find out exactly what it is that has in common with this extremely small rare community so we can further figure out a way to fight it and treat it, and furthermore prevent it. Please take the time to click on it register and fill it out. I myself have a lot of history with the disease and thought it would take quite a while, but to be honest it’s actually very super easy. Yes, there are a few parts that do take a little bit more time than others but trust me, it is so well worth it, and we need it done. Also, please like and share. Hello and thank you to everyone who posted, called, or text me this past Tuesday for my birthday. I’ve been in Washington DC this past week advocating for melanoma patients for clinical trial diversity, telehealth access without restrictions from state to state, as well as a few other important initiatives on behalf of the MRF @curemelanoma I know this is one of the biggest challenges in the Melanoma community, especially rare melanomas and I couldn’t of thought of a better way to spend my birthday then to meet other survivors, thrivers, advocates, patients, doctors sponsors, and foundations that are all trying to do something about the most deadly cancer there is. Today’s question regarding cell and gene therapy: “What is TIL therapy?” TIL therapy is an emerging type of treatment that builds upon success in the fight against cancer. TIL stands for “tumor-infiltrating lymphocytes”, which are immune cells (T cells or B cells) that have successfully infiltrated tumors. Since the lymphocytes are already targeting the tumor, there are no genetic modifications needed to point them toward the tumor. The objective for scientists is to create more TILs. Doctors can remove TILs from the patient’s tumor and send them to a laboratory, where they are multiplied. The new TILs are inserted back into the patient’s body. Treatment of metastatic melanoma: immunotherapy according to the TIL protocol 🔍 Melanoma is the rarest type of skin cancer, but it is also the most dangerous one. It develops rapidly and metastasizes early. At the same time, this tumor is one of the most susceptible to immunotherapy. This type of treatment demonstrates good results even in metastatic tumors. The doctors abroad use immunotherapy according to the TIL protocol (i.e. tumour-infiltrating lymphocytes) for the treatment of melanoma at the advanced stage. TIL therapy is based on the separation and cultivation of tumor-specific lymphocytes. These are white blood cells which are responsible for immunity. During the treatment, human own lymphocytes are obtained from the tumor tissue. They are located inside the melanoma, as they migrate to the tumor to fight cancer. The next step is the activation of these cells and their cultivation in the laboratory conditions. Then the cells are injected back into the patient's body. 🔸 Why does it work? Our immunity can destroy cancer cells and does it all the time. But melanoma can defend itself against immunity. Therefore, despite the migration of lymphocytes into the tumor, they cannot destroy it. There are several reasons for this: ◾ Insufficient number of cells ◾ Their low tumor reactivity ◾ Excretion of substances by tumor, which reduce the activity of immune cells These problems are solved during the treatment according to the TIL protocol. The immune cells are extracted from the tumor and cultivated in the laboratory in order to increase their number. At the same time, their reactivity increases. The cells develop without inhibiting tumor action, so when they are injected into the body, the doctors usually obtain good response to treatment. 🛡 Immunotherapy for metastatic melanoma according to the TIL protocol (tumor-infiltrating lymphocytes) Melanoma is one of the most aggressive malignant formations. It takes lives of young people: many patients belong to the age category of 30-40 years. Melanoma develops rapidly and metastasizes early. At the same time, this tumor is one of the most susceptible to immunotherapy. This type of treatment demonstrates good results even in metastatic tumors. The doctors abroad use immunotherapy according to the TIL protocol (i.e. tumour-infiltrating lymphocytes) for the treatment of melanoma at the advanced stage. TIL therapy is based on the separation and cultivation of tumor-specific lymphocytes. These are white blood cells which are responsible for immunity. Good treatment results can be achieved in 50% of patients. Their symptoms are less severe, most tumor foci regress, the growth rate of the tumor slows down, and the duration and quality of life increase. In 20% of patients the tumor and its metastases regress completely. Although TIL therapy does not guarantee a cure, even such results should be considered impressive. After all, do not forget which patients can be treated using immunotherapy. These are mainly people with advanced stage of the disease, who no longer receive the results from other treatment methods. TIL (tumor-infiltrating lymphocytes) is when T-cells are grown from the tumor itself. Adoptive cell transfer therapy, or ACT, includes a number of different types of immunotherapy treatments. Among ACT for melanoma, TIL therapy is most common.TIL therapy is a procedure in which white blood cells that have left the bloodstream and migrated towards a tumor, TIL (tumor infiltrating lymphocytes), are enriched to help fight and reduce cancers. As part of the therapy, TIL cells are extracted from a patient’s tumor, amplified in a laboratory to increase their number, and then infused back into the patient.
