WOW!! Another HQSC - @uw_nurf collaboration in the books, this time to highlight some of the important work in health equity occurring across @uwmedicine. We welcomed an incredible group of speakers from the departments of surgery, anesthesia, endocrinology, geriatrics, palliative care, and medical ethics to share their stories and insight on health disparities. • We recognized the reality of our diverse, aging population and heard how to create opportunities for culturally competent, beneficent, patient-centered care. • We learned about the health impacts ("allostatic load") of systemic racism AND several ways to prevent or treat those health consequences. • The ultimate lesson was clear: health equity impacts everyone. To build a resilient, high-quality health system, care must be equitable! . . . An interesting proposition for a round table held during the 25-year anniversary meeting of the Central Committee on Research Involving Human Subjects (CCMO) #CCMO25jaar. After introductory talks there was an open discussion focusing on the use and acceptance of gene therapy. We raised critical questions about the challenges and hopes associated with gene therapies for rare conditions, emphasizing the importance of natural history studies, and the evolving landscape of patient participation in research. This was only one of four excellent sessions during this meeting, all centering about patient participation and various ethical considerations. Very important to have this focus in current day and age. I recently rediscovered the Oath of Maimonides, a Jewish physician from the Middle Ages. This oath is of particular interest to me right now. Because, for the second time in 5 years I am being dismissed by a primary care doctor for being too complicated. When I’m finally improving, thought I finally had a good team assembled, and thought my PCP was really willing to put in the work. She has other MCAS patients, she is familiar with my conditions. She’s been handling my complexities like a pro for two years. Truly, I am not being dismissed for being too complex, but because the office no longer wants to focus on sick people or even preventative care, but longevity and “wellness.” “Wellness” in this case meaning that they offer a lot of classes and seminars that you can pay extra for. I intended on using those services, and I’m finally getting healthy enough that I was considering looking into some of them. They upped their fees and are moving to the 5th richest city in America to start over with healthy, wealthy patients. They have dismissed seemingly all of their chronic illness patients whose cases are not straight forward. To do this during a major PCP shortage to people who are sick enough to need extra help is so unethical. The first time this happened to me, I was seeing my first concierge doctor after not being able to find anyone in regular medicine who could handle all my complexities. I was paying GOOD money to be a client, not just a patient, in the hopes that as a client I would matter a little more (because that’s the unfortunate state of medicine today). That doctor eventually got rid of me by answering every question I had with “I don’t know, I don’t know enough about that condition,” instead of “I don’t know, let me do a little research and get back to you.” Part of concierge medicine is working with someone who has the time to do the extra work. She was annoyed when I asked things of her that any PCP would need to do. Both of them want the money without the work. Cont'd in comments.

Tel Aviv University has finally made the long-awaited decision to remove the Sackler name from its medical faculty, reflecting the global movement against the family's role in fueling the opioid crisis. The Sacklers' establishment of Purdue Pharma and their misleading marketing tactics for OxyContin have been attributed to the opioid epidemic, causing immense damage to public health. In May, the Sackler family received immunity from lawsuits related to the crisis in exchange for a $6 billion payment. I have a growing interest in why therapy fails - likely due to the high regard in which I hold it and my own success as a client. There are HUNDREDS of therapeutic modalities such as Cognitive Behavioral Therapy or The Gottman Method and only a dozen or so of them are actually backed by research. 😧😵 Many of these methods are harmless except for robbing the client of valuable and precious time that could be spent working on what works. Some of these methods create secondary problems and do in fact harm people. A proper diagnosis and treatment protocol saves time AND LIVES. DID YOU KNOW that many of the same people who are railing against “mainstream” medicine and providers who prescribe for being in the pocket of the drug companies are themselves profiting from selling supplements? . I received this text yesterday, but it is not uncommon for me to receive several similar calls & emails a month. . Next time you see someone on IG or elsewhere selling name brand supplements, or products like pellets from their clinic, or touting the advantages of food allergy tests, or dried urine or saliva tests over conventional labs, ask them if they are “capturing revenue” from these. . How many “experts” do you follow are PR people for supplement and/or lab companies and “capturing revenue” in kick backs and salaries? . Full transparency, I get zero financial incentive to prescribe HRT or contraceptives, order labs, mammograms or DEXAs, or provide a hyperlink to Amazon for vitamin D or prenatal vitamins. . @langernutrition @drnadolsky @dr.adrian.chavez @drjengunter @unbiasedscipod @dr_idz among others have highlighted this issue recently and there is a whole field of medical ethics so I am not sure why this does not get more scrutiny. .
