We get far too hung up on finding balance or telling ourselves we just need to be stronger. All we are doing is perpetuating a story that something is wrong or not quite right within us. This couldn’t be further from the truth. My passion is helping women discover what their body needs to feel good. This could be called balance but as our bodies continuously change - chasing balance is pretty exhausting as the terrain within us is ever changing. I spent years working to make myself stronger. What do I mean when I talk about BALANCE? Often when we talk about finding BALANCE, we are referring to managing the rollercoaster nature we experience as women. The peaks & troughs, and subsequent symptoms we feel throughout our journey.⁣ During periods of transition, women often find their hormones are in the driver’s seat; they are just along for the ride. Think about after you had a baby or the months spent wondering if you're in perimenopause...do you have a feeling you have to just get through it? Knowing it may get better, but for now, it's out of control. You have lost a synergy and it feels uncomfortable. So you search for a sense of BALANCE that will help you feel more in control. What if I told you, you will never reach a state of BALANCE...hormonally at least. As women, we have a natural fluctuation that occurs & our hormones are at the centre of this. This rollercoaster is the power we can use to find comfort in the synergy we most desire. We don’t want to stop the roller coaster...instead, we want to find ourselves in the front seat able to see the journey ahead. Knowing with confidence, we are in control of the ride. Our life choices on diet, sleep, exercise, beauty products, relationships, responsibilities....they all play a role in the functioning of our hormonal system. 2023 ~ Een bewogen jaar. Dat is denk het allesomvattende maar ook het meest kort samengevatte van hoe het was. Een rollercoaster, emotie-achtbaan, hoge pieken vs diepe dalen, leerzaam, moeilijk, slopend maar liefdevol jaar is de langere versie. Ze zeggen wel eens dat een mens krijgt wat hij ‘aan kan’ , nou ik weet niet wie dat bedacht heeft maar die is dan vast op z’n achterhoofd gevallen. Het was me het jaartje wel! Een jaar waarin ; * we ons gezin uitbreidde met Juul . * waarin Senn naar groep 3 ging en ineens een puber is voor m’n gevoel qua taal en wat hij kan. * Cas bijdehandjes gegeten heeft tot hij er bij neer viel en ik hem afwisselend achter het behang plak of plat knuffel om z’n heerlijke logica. * Waarin ik na 6 maanden thuis met m’n meisje weer ging werken en die maanden een vingerknip waren voor mn gevoel. * Cas van helemaal geen zin in plassen op de wc naar hup ik ben zindelijk en toen ook die luier ‘s nachts niet meer wilde.. Ehh oké vriend! * Senn z’n A-diploma haalde en ik me ineens bedacht dat hij pas gister geboren was. Hoe kan dit? * Juul vriend en vijand verbaast met haar ontwikkeling en ik als emo-moeke extra hard jank bij elke mijlpaal. #blameitonthehormones * Waarin de auto bijna op auto-piloot naar AMC kan rijden en ik daar om gniffel maar eigenlijk van binnen om jank * Waarin ik mezelf als mens/vrouw/partner ongeveer twintig keer verloren ben omdat ik in mama-overleef-modus was/ben * We niet meer Engels onderling kunnen praten als je iets ‘geheim/stiekem’ wil doen voor de kids want Senn begrijpt te veel * We 1 jaar getrouwd waren en ik had dat eerste witte broodsjaar wel iets anders voorgesteld.(eigen schuld dikke bult misschien maar tja .. i’m only human) En dan nu… 2024 ! Ik hoop/wens/gun mezelf/ons een jaar met meer rust, meer genieten, minder stress, ontwikkeling over hoe ik weer meer mezelf kan zijn, een meisje wat (op haar tempo) goed blijft groeien, twee kanjers van jongens die de beste broers zijn en daarnaast zichzelf blijven ontwikkelen,meer tijd voor elkaar als man en vrouw en eigenlijk gun ik mezelf nog zo veel meer maar laten we hier is mee beginnen! 34 years old already! 🤭 For the first time in 34 years, the first thing that appeared in my mind once the clock striked 12 was.. "Wow, this was the very day Mummee brought me into this world. 🥹" I guess she also faced the same fears, worries & stress that I now do. Likewise, she never had all the answers, & definitely had way less support & resources than I do. Yet, she did all that she could to protect me, love me, & keep me safe. & then ya, I started crying. & that was I started my 34th birthday. 🫣 #BlameItOnTheHormones Wouldn't expect that our last birthday celebration together with just the 2 of us (heheh love that we are born just 3 days apart) will be such a memorable one. Only some of our loved ones will know why 😂 Our birthdays next year will be entirely different, with our lil' buddy joining in the candle-blowing & cake-cutting. While I kept talking about how I want to cherish the current days with the freedom that we can still enjoy, truth is - visualising how it will be like next year, is making my heart smile already. Do you know the difference between total and free testosterone? So many providers and patients are still ONLY looking at total testosterone levels and don't have an understanding of why the Free testosterone level is so important. Total testosterone in the serum is really inactive. To be active, it needs to make its way through receptors on the cell–hence “free testosterone” or bio-available. It's the Free levels that really tell us how much Testosterone the body is actually using. Men and Women may have plenty of total testosterone but not a lot of bio-available testosterone. If this occurs, a patient will still have many symptoms, even though the doctor may say, “Your testosterone is normal or is optimized” when only looking at the Total testosterone. Understanding the difference makes the difference in treatment plans. Understanding why this happens in the body and how to help the body use more of the testosterone being produced is key to individualizing treatment plans for patients! Question: So why did the WHI (women's health initiative) show an increased risk of Breast cancer when ADDING the synthetic progestin to one of the arms in the trial? Answer: Progestin (medroxyprogesterone acetate) (Provera is the brand name) Actually BLOCKS the androgen receptor (AR) aka, testosterone generally speaking. Testosterone is incredibly protective of the breast, as we see down regulation (shrinking) of ER pos. tumors with certain levels of testosterone (Check out Dr. Rebecca Glasers work). So, when Progestin blocked the androgen receptor in the WHI (women's health initiative) it also blocked the positive benefits of testosterone, which is why we saw a slight increase in breast cancer in that trial. Takeaway, don’t let your doctor put you on any synthetic progesterone… Ladies, check your birth control. 1 out of 7 in the US will have a breast cancer diagnosis. 1 out of 7 with or without hormone replacement. To date, the only hormone that has ever been implicated in increasing the risk of breast cancer is a synthetic progestin, NOT ESTROGEN. If Bioidentical drugs or even synthetic Estrogen were shown to increase the risk of BCA, wouldn’t that statistic be much worse knowing millions upon millions of women every year are on HRT of a variety of kinds? Spoiler alert, we don’t. If we did, it would be so statistically significant that we would see it published everywhere. If HORMONES caused cancer, particularly estrogen, wouldn't BCA rates be higher at younger ages when those hormones are at their highest? Thyroid is a metabolic hormone. It is so vitally important in maintaining optimal health. Most lab reference ranges will measure Free T3 between 0.4-4.0 some may vary. Let's say that you are in that normal range, say your number is 1.0. So, at 1.0 you are “normal,” right? Why do you still feel tired? Why are you are losing your hair? Why are your eyebrows are thinning? Why do you have brittle nails? Why is it that, no matter what you do or try, your weight keeps going up and up? You may have suboptimal thyroid. Even though your labs are normal, it does not mean that you are optimal. Think about a bell curve–you know, what we learned in highschool. Bell curves illustrate the low, medium, and high percentiles, right? In the example above you would be at less than the 25% percentile, whereas someone else may be around 3.7 for their Free T3, so they would be considered the 75%+ percentile. Who do you think feels better? Feeling stressed? Wanna know a cool fact? The coolest thing about the endocrine system is that it is auto-regulated (it regulates itself). If one hormone up-regulates a system and the whole system revs up, there is always another messenger hormone sitting around waiting to go back and turn things down again. For example, your brain gets the message that your body wants more thyroid hormone: thyroid releasing hormone (TRH) from the hypothalamus, tells the pituitary to secrete thyroid stimulating hormone (TSH), TSH travels via the bloodstream to the thyroid gland, which in response to this TSH, releases more thyroid hormone (T3 and T4), then, these higher levels of T3 and T4 go back up to the hypothalamus and say, “Here are the new levels of T3 and T4.” If the hypothalamus is happy, then thyroid releasing hormone is no longer secreted. If the hypothalamus still wants more thyroid hormone, then it releases more the TRH, the pituitary gets the message and continues to release TSH which stimulates T3 and T4 levels to go up some more. What’s the difference between Total Testosterone and Free Testosterone? Glad you asked! When we check Testosterone levels, we always check both Total and Free Testosterone levels. The Total Testosterone number tells us how much your body is making or how much of an increase of Testosterone we achieved after replacement. But the Total Testosterone is inactive in the blood serum. The Free Testosterone levels tell us how much Testosterone your body is actually using. In other words, Total testosterone is not an indication of what is happening at the cellular level. Wrapping up our FAQ on “What options do I have for hormone replacement” In the last several posts, we highlighted the BHRT options that are available, along with the pros and cons. The next thing to consider is where these products are made. As much as I love supporting “local,” sometimes local (or smaller compounded) pharmacies just don't have the funds to do as much rigorous testing standards as others and unfortunately, this testing is not necessarily required based on the pharmacy designation. Choosing a compounded pharmacy that goes above and beyond in testing for purity, potency, sterility, endotoxins, etc. is worth a premium, especially because this is for your health. You want the highest quality products! Many nationally licensed (Licensed in multiple states) typically have the buying power to conduct above-standard testing with better quality control along with the buying power in volume which may actually be less expensive for that medication than a smaller pharmacy. I’m such an advocate for educating yourself and becoming more aware of hormone replacement. And you all know how I feel about synthetic drugs…so just like usual, this post is all about bioidentical hormone replacement. Today let's talk about Pellets. Disclaimer, I have personally been on hormone pellets for almost 10 years. So, this is my bias of course. However, I have seen many providers who are neither properly trained nor up to date on the latest advancements in this therapy modality, and unfortunately, patients are the ones who miss out on how great the therapy can be. Below are some of the benefits of this therapy! Pellet therapy ➡️ is the oldest form of HRT done here in the United States (since the late 1930s). ➡️ is also the closest form of endocrine mimicry. ➡️ doesn't first pass through the liver. ➡️ has the longest therapeutic window and avoids the rollercoaster, up and down effect like any other modality creates. ➡️ is 100% compliant and there is no need to remember to take a pill, rub a cream, or apply a patch. Due to the subcutaneous delivery method, pellets are absorbed on demand and not time-released, based on cardiac output. This is why Pellet therapy gives the body what it needs when it needs it–another great advantage. Pellets have been studied to replace both Testosterone and Estradiol. Progesterone Pellets are not studied in the literature, like Testosterone and Progesterone are. Pellets are pretty tiny, just a little larger than the size of a grain of rice or a tic tac. They are placed under the skin in the fatty tissue and will absorb over 3-6 months. What options do I have for hormone replacement? Today let's talk about Patches. We want to always be aware of and educated on the pros and cons of these hormone replacement options. Because what do I always say? There is no such thing as one size fits all. I’ll share without bias, except when it comes to synthetic drugs. SYNTHETIC DRUGS= NO, NO! So, please note all of my recommendations are in the realm of bioidentical hormones. Now, back to Patches! Hormone Patches are adhesive-like stickers that can be placed weekly or twice a week to deliver estradiol and/or progesterone to be absorbed through the skin. Patches, like other transdermal creams applied on the skin, may become less and less effective over time due to receptor site resistance. Some people just do not absorb well through the skin. Application frequency is less than daily or multiple times a day like some modalities, so they tend to be a little more convenient for patients that don’t want a daily hassle. However, compliance issues are still prevalent with patch applications depending on the strength and combination a few times a week. Another thing to remember is, like many modalities, you may get too much or too little and it is difficult to control the absorption rate. There may need to be some trial and error to get to the desired level and achieve symptom relief. I will, from here out, ONLY be referring to bioidentical hormones. :) Today let's talk about, Rapid Dissolve Tablets (RDT’s) Rapid Dissolve Tablets are another great way to optimize hormones. RDT’s dissolve in the mouth or under the tongue within about 60 seconds. RDT’s can come in a variety of strengths and can be titrated to the desired effect. RDT’s can be a little more pricey. Availability is dependent on the binders used, especially if there are national shortages in sourcing raw materials. RDT’s also have a shorter half-life, and patients may need to take their RDT a few times a day. Bioidentical creams are one way you can replace hormones! When it comes to evidence-based studies, Progesterone transdermal creams have NOT been shown to equate to tissue levels that protect the uterine lining, in an effort to oppose estrogen. This is not the best option to replace Progesterone. Transdermal creams can sufficiently replace testosterone and estradiol and can be very effective in optimal levels and symptom relief if applied in the Scrotal and Labia areas, added plus, great for libido when applied down there Due to the shorter half-life of creams, which is why multiple applications may be necessary. Transdermal creams can transfer to loved ones based on the application site. Transdermal creams may take a little trial. Monitoring levels through your prescriber is necessary to find your optimal dose. Cost can vary based on dose and combination. Available through compounding pharmacies. The biggest “CON” for patients is compliance and convenience. What are Bioidentical Hormones, and what is the difference between Bioidentical hormones vs. traditionally prescribed hormones? The term bioidentical should refer to the chemical structure and not the source of the hormone. So when we say “Bioidentical,” we’re referring to a hormone with the same chemical structure as the hormones that are naturally made in our bodies, such as 17b estradiol. Bioidentical hormones are also metabolized by normal metabolic pathways without the harm that we see from chemically altered hormones that can produce harmful metabolites and complications. So, what’s the difference between bioidentical hormones and synthetic hormones? Simply put, synthetic hormones are manufactured with a different chemical structure than what is produced by the body. This question comes from a lot of the false information spread in the marketplace. The FDA has mandated two designations for pharmaceutical compounders that serve patient-specific needs with the increasing demand for high-quality compounded drugs. These specialized entities are categorized as either 503A compounding pharmacies or 503B outsourcing facilities. 503A compounding pharmacies MUST comply with requirements according to their respective state boards of pharmacy regulations and USP requirements chapters 795 and 797. 503B outsourcing facilities produce large batches of medication for use in healthcare facilities, ambulatory surgery centers, healthcare systems, and physicians' office use. The Drug Quality and Security Act (DQSA) was signed into law in 2013. It created Section 503B, allowing this new type of compounding pharmacy to exist. Because these entities produce a much wider range of drug products, related regulations are decidedly more extensive. Sterile as well as non-sterile medications can be mass-produced in a facility that is designated as 503B. Registration requirements include the FDA as well as the DEA and the residing state boards of pharmacy. Regulations required of 503B compounders are much more rigorous than for 503A pharmacies. Like 503A pharmacies, compliance with USP 795, 797 and respective state boards of pharmacy are required. Most notably, 503B facilities must also strictly adhere to cGMP requirements (21 CFR Parts 210 & 211). In short, both 503A pharmacies and 503B outsourcing facilities are facilities that produce compounded medications. Both have specific requirements, and some even go above and beyond what is required to provide the highest quality and standards of compounds for their patients.
