80 year old female with similar findings on the feet. Diagnosis: polydactyly (supernumerary digits). Our patient has 12 fingers and 12 toes, all functional. Multiple other family members also have extra digits, and no one, including herself, has other associated abnormalities. Polydactyly is one of the most common congenital limb anomalies. It can be as an isolated finding or in association with other congenital anomalies (syndromic polydactyly). Supernumerary digits of the hand are most often found on the ulnar surface of the fifth digit. They range from small pedunculated papules to fully formed digits. Treatment is surgical. For the hand, it can occur in any of three places: 1) on the small finger side (ulnar), 2) on the thumb side, also called thumb duplication — less common (radial), and 3) in the middle of the hand — least common (central). Polydactyly affects about 1 in 500 - 1,000 babies. Usually, only one hand is affected. The extra fingers are usually smaller and abnormally developed and can be made up of: 1) skin and soft tissue, 2) skin, soft tissue, and bone but no joint, and 3) skin, soft tissue, and bone with joint (closer to a fully-formed finger). During normal embryonic development, the hand initially forms in the shape of a paddle, and then eventually, at about 6 to 7 weeks gestation, splits into separate fingers. Polydactyly results if there’s an irregularity in this process. An extra finger forms when a single finger splits in two. Most occurrences of polydactyly are sporadic, but some are due to a genetic mutation or syndrome. Abnormalities of the associated metatarsal commonly occur in polydactyly. 🧑🧑🧒Positive family history in 30% of cases. What are the syndromes with which polydactyly has been associated with? Ellis-van creveld syndrome Trisomy 13 Tibial hemmemelia Trisomy 21 📚Adam et al observed clinical findings in 18 cases of diabetic embryopathy and preaxial hallucal polydactyly to identify the features most suggestive of diabetic embryopathy. 📚They found that proximally placed preaxial hallucal polydactyly, particularly when coupled with segmentation anomalies of the spine and tibial hemimelia, is highly suggestive of diabetic embryopathy. They added that diabetes in the mothers pointed to a possible genetic predisposition interacting with teratogenic effects of poor glycemic control. ❓The incidence of polydactyly is 1.7 cases per 1000 live births. The frequency is higher in african Americans (3.6-13.9 cases per 1000 live births) than in caucasians (0.3-1.3 cases per 1000 live births). Here we have Poly D another former RICR inmate. Here’s an update on how’s she doing in her furever home: Poly has really opened up. She always needs to be sleeping right next to her mom. Even if her mom moves, she moves closer, so she’s pressed against her. She will also find the oddest place to nap and stretch herself all out. She’s also getting along a lot better with her sister! They always chase each other – not sure if it’s fighting or just playing, but now they’re comfy enough to sleep on the bed together! However, one must be near the top and one on the end. Poly can now be picked up but only with Churru treats or else she gets a little wiggle. She’s also the biggest food thief! You can always find her trying to sneak some nibbles! She’s even tried chewing open Temptation treat bags before dinner when she’s hungry! Additionally, she’s been trying to explore more. Overall Poly is doing great! She’s made to be a house cat! She needs pampering and lots of cuddles, which she gets all the time! We need your help. Again 🫣 EVE came to us with three others from a large owner surrender intake at a shelter on the eastern shore. All four ladies were on pregnancy watch and honestly, they had me convinced they were pregnant but nope, just that good SOMD eatin’ 😅 Upon intake we noticed EVE was a polydactyl; extra toes on her back feet. Although they don’t bother her or cause any immediate health concerns, we do worry about the future and possibility of them getting snagged on something. If that were to happen it would then become an emergency removal. Not to mention, it is terrifying even for the most experienced piggy nail trimmer, to trim those dangly toes. Recovery should have a rather quick and easy but she will need to be sedated for the removal, which unfortunately comes with quite the vet bill. Estimated procedure cost could be anywhere from $450-715 (depends if there is any type of tiny little bone in it or just skin)! 🎨いっちゃんの絵🧑🎨 ストーリーで反響が凄かったので いっちゃん画伯について お話ししますね😆 絵らしい絵は ほとんど描きませんでした。 クレヨンや絵の具も 黒ばかり好むし 小さい丸を延々描いてたり… 病んでるのかな😰と思うくらいに。笑っ そんな中で お友達のちかよん@ateliershimaneco_arttherapyに 絵を見てもらったり 親子アート教室に参加させてもらって。 『たぶん黒は筆圧弱くてもハッキリ見えるから』 『この丸はきっと人じゃないかな？』 とか、わたしには思いつかないような 沢山の見解を教えてもらって。 アート教室では 見本もない。 正解もない。 好きなように 思ったように ホントに自由に 描く楽しさ 描く開放感を 味わえて…🥺 今までは やっぱり同級生達のように 目や鼻や口がある 顔らしい顔を描いて欲しいって 思っていたの。 でも今日は いっちゃんの目に映るままに 描いてくれたら それが母の顔なんだなーって 『お母さんの顔、描いてー！』って お願いしたら 『うん👍🏻』ってスラスラ描き出して。 わたしが描いて欲しいなって思ってた 目も鼻も口も（髪まで🥹）ある絵を 描いてくれたもんだから もう、家宝にします。 棺桶に一緒に入れて。 っていう仕上がりでしたので ここに記録として残します。 一年生の時に 『絵を描いて』と言われても 具体的指示がないと固まってしまう兄が 白紙で出した画用紙が 名前だけ下に付いて貼られていた 参観日の廊下を思い出して なんだか涙が溢れるのは 歳のせいでしょうか？笑っ #ダウン症 #21トリソミー #t21 #downsyndrome #Polydactyly#多指症 #theluckyfew #5歳11ヵ月 #人らしい顔を描いた記念日 #先輩からもDMが来て #あんなに閉じない目がいっちゃんには…笑っ #って寝ても半目開いてることを #ニヤニヤジワジワとイジられましたが #とにかくこんな成長にも #寿司でも食うか？くらいに #喜べるのもまた幸せです
