With over 90 people in attendance, our team including Dr. Rob Lehman, Shari Myhill-Jones, Nick Gaskin, Amanda Dixon and Katie Clogg presented About the Sunshine Coast Primary Care Network, followed by a Q&A period from the audience. A 23 year old M presents unable to close his mouth. He is drooling copiously and difficult to understand. Looking through his record he had previously dislocated his jaw which makes this likely to be a recurrence. This instance initially occurred 12 hours prior while he was drinking heavily at a house party. He had went home and “passed out” in his bed hoping he would wake up to find it had returned to normal I had only seen a few jaw dislocations at that point in my career but this was very clearly one of them.  There are 3 “facts” I recalled about jaw dislocations in that moment 1)  Once one has a first joint dislocation, they have a greater risk of a recurrence 2)  The longer a joint is dislocated for the more difficult it becomes to reduce as the muscle start to stiffen up 3)  There is always alcohol involved. In his case it likely relaxed his already vulnerable joint and surrounding muscles, he yawned and “pop” I initially made my attempt using the technique pictured above, with no luck. This had always worked for me in the past, so I never needed to master any other approaches. I had been working that day with Dotty, a superhero old school ER doc who had been practicing for decades. She always had wisdom to share so her being there was always reassuring. She came to help me out and was sure that some Valium would help relax the muscles. He was maxed out on Valium and it was still too tight to reduce. Then Dotty sat there and methodically massaged his TMJ joint for what seemed like 10 minute before “pop” it was right back in In the time since I’ve tried this massage technique and had no luck. In the time since I’ve also watched her massage shoulders back into place Sadly she’s retiring as I understand it, but the skills and techniques she has taught thousands of ED clinicians over the years will be carried out for decades to come **You can see the technique I generally use if you swipe. 16 year old female presents with her mother for evaluation of a fever that has been present for 8 days. Her mother took her to the ER on day 6 and again on day 7, both times to be discharged with a non-specific diagnosis and ultimately placed on Augmentin for “PCP pneumonia”. On day 8 the patient presents to office covered in a maculopapular rash, abdominal pain, fatigue, headache, fever, sore throat and neck stiffness. Symptoms onset during a trip to Florida, but worsened on return back up north. She reports that she’s lost 6lbs due to lack of appetite since this began Vitals: Temp 102, BP 110/70, HR 116, RR 18 On exam very ill appearing she has a diffuse maculopapular rash on trunk and extremities. Tonsils are erythematous and nearly kissing each other. No exudates. Neck is supple, FROM, Tender bilateral deep cervical lymph nodes. Lungs clear, cardiac normal. Abdomen is tender, with palpable liver and spleen Labs were sent stat—pertinents as follows: WBC 16, Plt 30k, Alk phos 190, ALT 450, AST 478, EBV IGM +, Mono + Chest x-ray shows no signs of pneumonia. Abdominal US shows “gross hepatosplenomegaly” I sent the patient to the childrens hospital where she was admitted and spent more than a week Diagnosis: #Mono w/ organomegaly. Rash was secondary to amoxicillin interaction with mono. When diagnosing a child with #ADHD I always make it a positive and play up their strengths 💪 I'll start with "This is your superpower. Where typical people can focus a little bit of their attention on a bunch of different things, you can focus 100% of your attention on things you find interesting. That means if you're interested you can do things with much more focus than anyone else. Now that we've identified your superpower it's time for us to hone it and strengthen it" ⬜ Usually at this point I'll mention #CBT or medication if needed but most importantly I have their buy-in At the moment I'm quick to medicate because the data consistently shows that kids undertreated or untreated in their formative years have greater likelihood of bad outcomes I have very high hopes for the future though and envision a day with infrequent meds and no meds at all. 16 year old F presents with complaints of upper respiratory symptoms. On review of systems she mentions bilateral knee pain “that hurts all of the time” and has been present for as long as she can remember. On exam she is noted as having swan neck deformities in all digits. She has also previously seen a podiatrist who diagnosed her with bunions and flat feet. She then mentions having back pain that has been present for the last several months. At this point the pieces are starting to fit together and I began to question her family history. Her mother, who was in the room states that she herself has #Crohns disease, #RheumatoidArthritis, and nail patella syndrome Initially CBC, BMP, Iron profile, CRP, ESR, and RF were ordered, along with x-rays of b/l hands, knees, and pelvis The initial diagnosis is clinical, and she clearly has #NPS, though Crohn’s as a separate diagnosis is not yet ruled out #NailPatellaSyndrome aka Hereditary osteo-onychodysplasia, affects 1 in 50,000 people and is an autosomal dominant disorder. These patients have to be followed lifelong by orthopedics, nephrology, ophthalmology, and gastroenterology. Of note, this syndrome is the only disorder that will show iliac horns on a pelvic xray. These patients usually have no patella or hypoplastic patella. This isn’t going to be a mystery case, or even something with a twist ending. This is going to be a story about a regular patient.. A 58 year old male, “healthy as an ox” came in for his annual physical. He doesn’t get in too often, and last had a physical in 2016. He’s up to date with vaccines, and colonoscopy screening. He’s just overall a uniquely healthy person for his age. He does #Ironman competitions, and at the oldest looks about 45. I did my usual workup, which for a man his age includes a Hepatitis C screening. It came back positive, and this wasn’t such an extraordinary occurrence. Anyone born between 1945 and 1965 is supposed to be screened for Hep C, once in their lifetime This patient was sent to the hepatologist where he underwent further testing and ultimately completed a course of treatment and now no longer has detectable levels of Hep C RNA in his body So why him? #HepC can live dormant for decades after contracting it. It spreads from person to person through contact with blood. In the 60s and 70s as drug use climbed the risk of sharing needles was unknown to most. Tattoos and piercings done without proper sterilization also lead to unknowingly spreading the virus. I'm absolutely guilty of over prescribing antibiotics to keep my patients happy but we need to remember why it's so important that we don't. For millenia bacterial infections were the most deadly killers and one day that changed. A 42 YO female smoker, on oral contraceptives presents to the ED with sudden onset of right (R) sided face, arm and leg weakness that began 45 min prior to arrival. In addition she is also experiencing a mild frontal headache and some anxiety.  She does "occasionally get headaches", but has never experienced "migraines" or had prior weakness or numbness Her neuro exam is significant for (R) central facial weakness (preserved forehead wrinkle), 3/5 weakness in the right upper and lower extremities as well as decreased sensation to light touch and pin prick in the same distribution. Her exam was otherwise normal The pt was taken for CT head without contrast which ruled out hemorrhage, as well as CT angiogram head and neck, which ruled out significant stenosis, occlusion or dissection.  Because of her stroke risk factors and sudden onset, the patient was given IV tPA and admitted to the ICU An MRI was completed later that day but found no stroke.  The patients headache however gradually worsened overnight and she began to have accompanying sensitivity to light and sound. It was then that her diagnosis began to take shape. She was experiecing a complex migraine A post-tPA CT head without contrast was performed after 24 hours and the the patient was given an IV migraine cocktail of magnesium, solumedrol, zofran and depacon.  Her HA resolved after approximately 40 hours and she was discharged with follow up to the headache clinic Those that don't experience #Migraine headaches can often overlook the impact that they have on a persons quality of life #NeuroPA #emergencymedicine #emergencymedicinePA  #PASdothat  #familymedicine  #familymedPA  #PrimayCare #internalmedicine   #PAschool  #PhysicianAssistant  #PAstudent   #medicine  #medicalschool   #AAPA  #palife   #physicianassociate  #nursepractitioner  #premed  #prepa #aapa19 aapa2019

A 45 year old M presents to ED complaining of right sided jaw pain where he was seen by @antoniaisnotgreen . He presents with “nagging” jaw pain x2 days for which he took Tylenol with minimal relief. At the time of the visit (day 2) he feels that his face has become more swollen. He denies any fever, drooling, sore throat, stiffness or facial trauma On exam vitals were stable and he was tender and warm over the area of the pain. He had obvious difficulty opening his mouth and a large localized area of redness and swelling in the back-right side of his gums Antonia was suspicious for an abscess. A CT was ordered and showed findings that are suspicious for osteomyelitis, a deadly infection that has spread to the bones. 84 year old M with history A-fib, CHF, and HTN presents complaining of “weakness” and progressively worsening shortness of breath for two weeks with occasional dizziness. He has no other complaints. Vitals: BP 100/70, HR 89, RR14, 02 94%  EKG: Unchanged from previous Exam: Uncomfortable appearing and pale.  LLL crackles consistent with baseline. Normal cardiac and abdominal exams. No peripheral edema Initially my thought was CHF exacerbation vs MI vs #AFIB, etc. but considering normal ECG  and that he was recently started on Xarelto for his afib I did a rectal exam and found occult blood. I gave him IV Fluid (yes with CHF), and sent out a STAT CBC which had come back with a hemoglobin of  8.5. 33 year old M presents to the office for headache (HA) x 1 day. It began in the AM as he was standing on the train when he began to feel “numbness” in his feet and later had the onset of headache and nausea. His headache was atypical, and “unlike any HA” he had ever had so I sent him to the ER. In the ER a CT was normal and he was discharged with a dx of #migraine The following morning he was back in my office. His numbness has now spread up to his calves and to his hands. I drew blood (CBC, CMP, EBV, CMV, ESR and tick panel) and sent him straight back to the ER where an MRI and LP were both normal. He was admitted overnight for obs and DC’d w/ a #neuro follow up after feeling “a bit better” The next morning he was helped into the office by his wife. He was feeling “much worse” and unable to walk unaided. His numbness had worsened in his hands and feet, and now spread to his face. His HA was now accompanied by severe stinging pain in the T2-T4 area. Exam + for muscle weakness in lower extremities, normal in upper. He had unilateral loss of pupillary reflex,  anisocoria and was having trouble moving his tongue and swallowing. He was sent back to the ER  by ambulance which took him to a different hospital. The PA there called me for history performed an LP which found elevated cerebrospinal fluid protein with a normal white blood cell count. The #PA called me back to tell me that he was admitting him for “likely Guillain-Barré syndrome”, a diagnosis that suddenly clicked. He received plasma exchange was eventually discharged. He required #physicaltherapy and #occupationaltherapy and after more than 3 months was ultimately back to normal While admitted the labs I drew resulted. They had shown a recent Epstein-Barr virus infection. He later recalled having been sick for a week or two about a month earlier.  EBV was presumed to be the cause of his GBS The lesson I learned here is weird neuro symptoms warrant questions about recent illness #PASdothat  #familymedicine  #familymedPA  #PrimayCare  #neuropa  #PAschool  #PhysicianAssistant  #PAstudent   #medicine  #medicalschool   #AAPA  #NYSSPA #palife   #physicianassociate  #nursepractitioner  #premed  #prepa
