After all the stress of the last blood draw, the sample did arrive in Vancouver on Friday. After the weekend, we got the results: Arends oxi burst testing his neutrophil function came back at 19%, previously at 20% (plus the extra 6% peak). There is not much to say about that result; and as the chimerisms take weeks to receive data, we won’t be able to relate this to much until the next oxi burst and chimerism test in a month from now. Here’s to hoping it is stabilizing and that there may be a chance of the donor myeloid cells re-populating his bone marrow over time, reaching a mixed chimerism and a higher, stable neutrophil function. Time will tell…. Thank you to everyone for the mass response and encouragement to report the phlebotomist and experience at NRGH last week. My sadness has turned to anger and I plan on filing a report as soon as I find the time & energy to write it out kindly and coherently. I’m glad to report he hasn’t shown signs of fever which hopefully means he hasn’t picked up a bacterial or fungal infection from the raw exposure. Day +407 (BMT), +271 (Boost) Our son has to push through such trauma it breaks my heart over & over again. In order to skip needing to travel over to Vancouver every month, our team arranged a blood draw at our local hospital on Vancouver island. We are giving it a go with hope the blood arrives in time. We arrived on time, waited forever for them to sort out their paperwork for this, and did the blood draw. Arend kept saying “I don’t want to do this” 💔 - how many times do I have to break his trust as I hold him down and tell him he is brave. The poke was quick and on point, no drama. As we drive off to get his favourite treat @burnt.honey , I get a call saying there was an error and we need to come back and do another blood draw. As we pull into the hospital again, he asks why are we back and went in to absolute hysterics asking WHY. Over and over. “Why mama? I don’t want to do it AGAIN”. Back in the hospital the nurse confirmed they used the wrong colour tube. They used an alcohol swab and before it could dry & disinfect, they poked the butterfly into his arm. They began DIGGING in his arm for the vein while Arend screamed. My eyes were glued to that needle as they moved it around and I wondered if it was painful. Their unkept & dirty looking fingernails were pressing around where the needle was inserted and I sat there wondering when I’ll speak up. I took a deep breath and politely asked they wear gloves. I absolutely hate that this is something I have to do. The blood draw was done and I managed to say thank you as we stormed out of there both crying. We sat together in the car bawling our eyes out. I haven’t cried in months, I’ve become so numb to the things I do not have control over. But this? This was human error. Negligence. Sloppy work. Our cheeks are salty as we try to move past another traumatic experience. As I try to convince myself Arend will forgive me for all this. I’m so angry that it has to be this hard. Day +401 (BMT), +265 (Boost) With less anticipation than previous BMT follow ups, I was able to meet with Arends team today to discuss the latest chimerism results. There truthfully isn’t much to say other than we need to wait and continue to gather data to see if his BMT engraftment/boost will stick. Will the functioning neutrophils from his donor remain at a percentage that we can successfully remove prophylactic medicines or enjoy life with less worries. He is currently holding two immune systems within his bone marrow. For inexplicable reasons, certain types of blood cells have inhabited pockets in the bone marrow, where other cells have chosen somewhere else to replicate. (Ref. Lymphoid vs myeloid cells) With a couple decades of Bone Marrow Transplants on CGD patients, BMT & Immunology teams internationally were able to find the long-term outcomes with intense chemotherapy regimens less desirable; as is living managing this awful disorder. In more recent years, they have opted for less intensive conditioning treatments to improve long term outcomes. This holds especially true for younger CGD patients that are within their developing years. In these recent years however, we have all seen the outcomes of less intensive regimens: GvHD, mixed chimerisms and graft failure. So this leaves the big looming question - what is the best treatment protocol for CGD patients? Is this the same for all patients, based on past infections, treatments and age? Then looking at the science - questioning gene therapy, where they gain the power to manipulate cells and re-infuse them into the patient; which has not shown much advancement or success for CGD. Or digging deeper to understand why and how stem cells engraft; where they choose to engraft & how their lifespans affect successful engraftment long-term. The chemo regimen that Arend received, did not empty his bone marrow completely. This is why he has mixed chimerisms. This was however, placing an importance on his life and future. I would not have wanted to give him a stronger regimen with adverse long-term outcomes; yet going through a BMT without near full success is gutting. (Continued in comments)

Well here we have it. What started as a growth (post-BMT) on Arends chest and lead to ultrasounds, MRI’s and this most recent surgery, we finally found out that the vascular growth is NON-MALIGNANT 👏🏽 Most of the history of this occurrence has been through photographs sent to doctors as Arend has been reluctant to show them for much longer than a split second. I even received a phone call moments after he was admitted to OR as the surgeon for the first time was able to look at it and needed further consent to remove a larger area. During my briefing about the surgery, which I did not know about until arriving in Vancouver, it was mentioned that this could be cancerous. Although the whole growth was to be removed, it left treatment plans very uncertain. We’ve been anxiously awaiting results and feared that he developed cancer after his BMT. This reality was devastating to me. I’ll rest my heart knowing he’s safe from this now.
