When you have a child with Down syndrome, the Otolaryngologist or ENT is a specialist that you may see regularly and possibly early on in your journey. For 60-70% of children with Down syndrome, there is conductive hearing loss attributed to fluid build-up in the middle ear. In general, the anatomy of our babies tend to be narrower so that unfortunately makes them more susceptible to illnesses and in the case of hearing, the inability to properly drain fluid from the ears. For this type of hearing loss, inserting grommet tubes is sometimes the solution to help drain the fluid. You will hear parents say that their child had to “get tubes put in” and this is what they are referring to. For 10-15% of children with Down syndrome, there may be sensorineural hearing loss which is usually permanent and may due to the chochlea not functioning properly. In this case, the ENT will recommend a CT scan and brain MRI to be able to determine the reason for hearing loss. Based on the results, the ENT may inform you that your child is a candidate for a cochlear implant or determine that there is no treatment and accommodations for hearing is the only option. For my son, we had a mix of conductive hearing loss on his left side and sensorineural on the right. Over time, the fluid cleared on its own (no tubes) so determined that he had normal hearing on the left (this occurred when he was 1 year old). Unfortunately, on the right, he has permanent deafness due to a hypoplasia (small or absent) of the auditory nerve. This also makes him ineligible for a cochlear implant since there is no nerve functioning. As a result, our only option is to make special accommodations in the classroom or environment (example speaker on his left side and sitting in front of the classroom for school).
