El 16 de julio de 2004, el vuelo 1504 de Blue Panorama estaba despegando de Roma, Italia cuando en la carrera de despegue el motor derecho del avión explotó y comenzó a incendiarse, los pilotos igualmente continuaron el despegue y se elevaron, ya en el aire apagaron el motor dañado pero no pudieron detener el incendio, luego de realizar un circuito devuelta al aeropuerto, el avión aterrizó con sobrepeso reventando todos los neumáticos del tren de aterrizaje, por suerte se detuvieron de manera segura, sobreviviendo todas 283 personas a bordo. El 10 de agosto de 2016, un helicóptero estaba realizando trabajos de lucha contraincendios en La Palma, Islas Canarias, luego de realizar 12 descargas de agua el helicóptero se dirigía a Pico El Cabrito, La Palma, el helicóptero comenzó a girar sin control a la izquierda donde impacto varios pinos en una ladera de una montaña y terminó estrellándose, los 2 tripulantes a bordo sobrevivieron. La causa del accidente se debió al fallo del rotor de cola del helicóptero, la tardía reacción del piloto sobre el fallo y otros factores contribuyentes fueron que el agua que transportaba el helicóptero no fue descargada y que el helibalde no fuese desenganchado. El 11 de marzo de 2024, el vuelo 800 de LATAM Airlines volaba de Sídney, Australia con destino Santiago, Chile y con una escala programada en Auckland, Nueva Zelanda, durante el trayecto Sídney-Auckland mientras el avión volaba a 41.000 pies sobre el mar de Tasmania sin aviso el avión entró en una picada a alta velocidad, por suerte los pilotos lograron recuperar el control del avión después de caer más de 300 pies, finalmente el avión aterrizó en Auckland luego de 1 hora, todas las 272 personas a bordo sobrevivieron al incidente, pero 50 personas salieron con heridas de distinta gravedad. El incidente actualmente aún esta bajo investigación por lo que la causa exacta es desconocida, se especula que los instrumentos de control de la cabina de mando fallaron o también la manipulación accidental del control del movimiento del asiento del capitán por una azafata, esto causó que el asiento se fuera hacia adelante y el capitán empujara el timón de control y provocará la entrada en picada. This lovely photograph was taken in the garden of Slepe Hall School, St Ives, probably in summer as there are leaves on the trees and the flower beds in bloom. If you look carefully in the background is the parish church with part of its spire missing which is very helpful in dating the picture to between March 1918 and June 1923 when work began to repair it. The spire was damaged when Lieutenant Wastell crashed his De Havilland DH6 on route from Wyton. He was killed instantly and terrible damage was done to the church. El 3 de junio de 2012, el vuelo 0992 de Dana Air estaba realizando un vuelo entre Abuya y Lagos, ambas en Nigeria, cuando el avión alcanzó su altitud crucero los pilotos se dieron cuenta que el motor N°1 (izquierdo) había fallado, pero en ves de desviarse a otro aeropuerto o volver a Abuya el capitán decidió continuar el vuelo, en el descenso a Lagos el motor N°2 (derecho) también fallo, los pilotos intentaron reiniciar ambos motores desesperadamente pero ninguno funcionaba, el avión comenzó a caer, perdiendo velocidad y altura drásticamente, finalmente el avión terminó estrellándose contra varios edificios del barrio Iju, en Lagos, muriendo todas las 153 personas a bordo y 6 en tierra. El accidente se debió a un doble fallo del motor provocado por fugas en las líneas de combustible de los motores por mantenimiento inadecuados y el error de los pilotos en no seguir los procedimientos correspondientes para el fallo de ambos motores. Ten people are believed to have been killed when two Malaysian Armed Forces helicopters collided in midair at the Royal Malaysian Navy (RMN) base in Lumut this morning. According to initial unconfirmed reports, everyone on board the two helicopters died, The M503-3 Maritime Operations Helicopter (HOM) is thought to have had seven crew members, while the M502-6 only had three. Both helicopters were undergoing formation training in preparation for Navy Day, which is scheduled to take place from May 3-5. According to sources at the RMN Strategic Communication Unit, they will issue a media statement shortly. Earlier, a 21-second video went viral on social media, showing one helicopter colliding with the rear of another before both crashed to the ground. In the same video, helicopter debris is seen falling to the ground near where hundreds of other navy personnel were practicing marching drills. According to footage published by local media, one of the helicopters clipped the pther's rotor before colliding with the ground at a stadium. "The RMN confirms that an RMN Maritime Operations Helicopter (HOM-AW139) and a Fennec helicopter crashed during a rehearsal for the 90th Navy Day Parade at the RMN Lumut base at 9.32am this morning," the statement said. 'This incident involved a total of ten personnel, including seven RMN HOM crew members and three RMN Fennec crew members." El 19 de noviembre de 1977, el vuelo 425 de TAP Air Portugal estaba aproximándose a Madeira, Portugal, esto desde un vuelo que se originó en Bruselas, Bélgica y tuvo una escala intermedia en Lisboa, Portugal, la aproximación a Madeira se realizó de noche y con condiciones de lluvia torrencial, las primeras 2 aproximaciones fueron frustradas debido a que no se pudo hacer contacto visual con la pista, en el tercer intento el avión aterrizó a casi la mitad de la pista, pero este no pudo detenerse y se terminó saliendo por el final de la pista cayendo en un barranco y posteriormente en una playa, muriendo 131 de las 164 personas a bordo. El accidente se ocasionó por diversos factores, entre ellos el error de los pilotos en continuar la aproximación bajo condiciones climáticas adversas, la longitud corta de la pista, que está también se encontraba mojada causando que el avión sufriera de hidroplaneo y el viento de cola presente durante la aproximación. APRIL 15 #ColdWar 1969- Mission “Beggar Shadow” was a recon program launched by the #USNavy in the late #60s to gather intelligence and communications on the Soviets and Soviet allies while staying over international waters. #OTD, an EC-121M Lockheed Warning Star #aircraft took off from Japan on one such recon mission with 31 US military personnel on board. The crew’s mission was to gather signal intel while orbiting over the Sea of Japan off the coast of #NorthKorea over international waters. About 6 hours into the mission, two North Korean #AirForce MiG-21 fighter jets took off East Tongchong-ni. These jets were tracked by the Americans however they were lost on radar 21 mins later. About 40mins later, the MiGs were re-acquired closing in on the Warning Star and the crew on the US aircraft were advised to abort and return to base. Due to the speed of the MiGs however the American plane had little time to react to the warning. One of the MiGs launched a single air-to-air missile which downed the EC-121M. The US military considered several options in response up to and including the use of strategic nuclear weapons on North Korea. Ultimately, the only response was a naval demonstration in the Sea of Japan which lasted only a few days. The deaths of the 31 men on the Warning Star was the worst single loss of life of American aircrew during the Cold War. #Aviation 2002- Air China Flight 129 was a scheduled flight from #Beijing, #China to #Busan, #SouthKorea. OTD, the flight was being done with a Boeing 767-200ER aircraft and had 166 people (155 passengers; 11 crew) on board. Two hours after take-off, CA129 received clearance for ILS approach to runway 36L followed by a circle approach to land on runway 18R. Weather at the time was iffy with precipitation and fog with gusty winds. During the circling approach, the plane flew outside of the approach area and the crew fell below the minimum safe altitude. At 11:21 local time, the 767’s right wing struck a tree and slammed into the ground. El 18 de febrero de 2024, el vuelo 324 de Air Serbia estaba despegando de Belgrado, Serbia en un vuelo con destino Düsseldorf, Alemania, durante el rodaje para llegar a la pista de despegue los pilotos decidieron despegar de la intersección DELTA 6 (D6) dándoles una distancia de pista de 2.275 metros y no de la cabecera de la pista, pero accidentalmente terminaron entrando en la intersección DELTA 5 (D5) que se ubica 1 kilómetro antes de la intersección correcta, con solo 1.275 metros de pista restante los pilotos decidieron despegar igualmente, pero el avión no ganó suficientemente velocidad saliéndose por el final de la pista impactando las luces y una antena del ILS al final de esta, el avión se elevó luego de recorrer 500 metros. Luego de 55 minutos agónicos que involucraron un paso bajo para revisar el tren de aterrizaje el avión aterrizó de emergencia nuevamente en Belgrado sobreviviendo todas las 111 personas a bordo, el avión sufrió daños graves en el fuselaje, incluyendo un enorme agujero en el lado izquierdo del fuselaje central y una fuga de combustible masiva en el ala izquierda. La causa del accidente no se a resuelto actualmente, pero lo más probable fue un error de cálculo de los pilotos en determinar si era posible el despegue desde la intersección DELTA 5 (D5). El 22 de diciembre de 1999, el vuelo 8509 de Korean Air Cargo se desplomó poco después de despegar del Aeropuerto de Londres-Stansted, en Londres, Reino Unido, donde el avión se estrelló con una inclinación de casi 90° grados a la izquierda, muriendo todas las 4 personas a bordo. British Midland Airways Boeing 737-4Y0 reg. G-OBME Delivered new 25 October 1988. Operating as Flight 092 from London Heathrow to Belfast on 8 January 1989, this aircraft crashed while trying to divert to East Midlands Airport. Engine 1 failed and the crew unknowingly shut down (the working) engine 2. As the stricken airliner approached East Midlands Airport, engine 1 (the failing engine) suffered a decrease in power and the airplane stalled. O mais antigo Embraer Ipanema, modelo EMB-200A, que estava em condições de voo e operação aeroagrícola, sofreu um acidente de grande proporção no final da tarde de ontem (09/04) em Uberlândia/MG. O PT-GBG era um EMB-200A, de número de série 200.007, era a 7ª aeronave registrada pela Embraer, fabricada em 1972. O acidente ocorreu em uma zona rural conhecida como Fazenda Bom Jardim, em Uberlândia/MG. Segundo o piloto, que estava na aeronave, uma falha no motor provocou a queda, tendo que efetuar um pouso forçado, ele ficou preso às ferragens da cabine. Segundo nota publicada pelo Corpo de Bombeiros que atendeu à ocorrência, o piloto estava com grau elevado de consciência e orientado, apresentava fratura no úmero esquerdo, fêmur e traumatismo craniano moderado, além de várias escoriações no rosto, mas sem sangramentos. A investigação no local do acidente e nos destroços da aeronave, será executada pelo Serviço Regional de Investigação e Prevenção de Acidentes Aeronáuticos (SERIPA III) para apontar as causas contribuintes do acidente. RAF Air Bomber Duncan Miller of 35 Squadron was here, having spent much of his POW time in hospital after Halifax JB786 was shot down by German flak and night fighters over Berlin on 23/24 August 1943. Shrapnel had severed the nerves in his right foot while in the aircraft, then his right fibula broke due to a heavy parachute landing while he was unconsious. Duncan was repatriated home in 1944 on one of the 'Mercy Ships' - Swedish cruise ship Gripsholm. The Archive holds Duncan's wartime memoir, photos and documents. El 9 de febrero de 2014, el vuelo 333 de la Royal Air Force (RAF) estaba realizando un vuelo de transporte militar desde la Base Aérea de la RAF Brize Norton, en Londres, Reino Unido con destino la Base Aérea Camp Bastion, en Afganistán. Luego de 3hrs 30min de vuelo el avión sobrevolaba Turquía cuando el avión comenzó una picada inexplicable, cayendo sin control desde los 33.000 pies, el capitán que se encontraba solo en la cabina intentó corregir la picada pero no pudo, finalmente el copiloto que se arrastró por el techo hasta la cabina pudo recuperar el control, habían caído 5.000 pies, por seguridad el avión se desvió a la Base Aérea de Incirlik, en Incirlik, Turquía donde aterrizaron poco después, sobreviviendo todas las 199 personas a bordo. La causa del incidente se debió al error del capitán que estaba sacando fotos durante el trayecto, pero al recibir un té corrió su asiento para atrás, sin que él supiera su cámara quedó entre él apoya brazos de su asiento y la palanca de control del avión, cuando el capitán puso su asiento en su lugar él apoya brazos empujó la cámara y esta empujó la palanca de control causando la caída en picada repentina. Some images from yesterday's walk up to the B29 crash site on Bleaklow Moor, Glossop near Snake Pass in the Peak District. The American B29 Superfortress crashed during a daytime flight to a US air force Base in Warrington on November 3rd 1948. A navigational error promted the aircraft to descend out of low cloud during poor visibility but sadly hit the hills of Bleaklow, killing all 13 crew. Chances are they never saw the ground. Much of the wreckage remains up on the hill. Mahon's report (on contrary to the official report), released on 27 April 1981, cleared the crew of blame for the disaster. Mahon said the single, dominant, and effective cause of the crash was Air New Zealand's alteration of the flight plan waypoint coordinates in the ground navigation computer without advising the crew. The new flight plan took the aircraft directly over the mountain, rather than along its flank. Due to whiteout conditions, "a malevolent trick of the polar light", the crew were unable to visually identify the mountain in front of them. Alle 17:06 del 27 marzo 1977, due aerei di linea si scontrarono sulla pista dell’aeroporto Los Rodeos di Tenerife, uccidendo 583 persone. Un numero impressionante. 61 i superstiti. I due #Boeing 747 erano tra i voli deviati quel giorno dal vicino aeroporto di #GranCanaria, a causa di problemi di sicurezza dopo lo scoppio di una bomba in un negozio di fiori all’interno del terminal, che causò 8 feriti. Così, tutto il traffico aereo fu dirottato sull’aeroporto di #Tenerife. Quando ai due Boeing fu dato il via libera per riprendere il loro viaggio, una tragica concatenazione di eventi (il grave congestionamento aereo, la nebbia e le difficoltà di comunicazione per motivi linguistici) fece sì che il volo statunitense Pan Am 1736, ancora sulla pista, venne colpito a tutta velocità dal volo 4805 dell’olandese KLM, autorizzato per il decollo. El 19 de julio de 1970, el vuelo 611 de United Airlines estaba realizando un vuelo de Philadelphia, Pensilvania, Estados Unidos con destino Rochester, Nueva York, Estados Unidos, durante el despegue de Philadelphia el motor izquierdo fallo en el ascenso, el capitán creyendo que habían fallado ambos motores dirigió nuevamente el avión a la pista, donde este la impacto bruscamente causando que se saliera por el final de la pista, deteniéndose con el tren de aterrizaje totalmente destruido, sobreviviendo todas las 61 personas a bordo. La causa del accidente se debió a una mezcla de factores entré ellos el error del capitán en suponer que ambos motores habían fallado cuando no era así y el mismo fallo del motor izquierdo por la fractura de un álabe de la turbina del motor que provocó daños internos significativos en este mismo durante el despegue. On March 10th we remember the 24 lives lost, the 45 lives who endured and survived, and all those who responded and were impacted by the 1989 Air Ontario flight 1363 disaster. This disaster impacted our entire community along with the aviation industry as a whole. As a result of the crash many changes were made to the Canadian Aviation Regulations in regard to aircraft refueling, de-icing, and general aviation safety. We recommend anyone interested in learning more to visit the Dryden & District Museum which is currently hosting an educational exhibit that looks into the incident, response, and impact it had on our community and the industry. At an altitude of about 29,000 feet, one of its main tires failed and exploded, creating a hole in the fuselage and the cabin floor. An emergency descent was initiated, followed by a successful landing at Qatar’s Doha International Airport. On January 15, 1943, a U.S. military Douglas C-54-DO Skymaster crashed at Warappa Creek near plantation Bakkie in Commewijne. The DC-4 was carrying senior military staff and secret documents en route to the Casablanca Conference, where it was later decided by U.S. President Franklin Delano Roosevelt and British Prime Minister Winston Churchill that Hitler should be eliminated. Also aboard this plane was Eric Knight, the creator of Lassie, who served as a Major in the U.S. Army. Knight and his 34 fellow passengers did not survive the crash.
