this is 6 months postpartum… My stomach is still stretched out and I’m okay with it. it’s a process and this is a chapter in the story.❤️ read my honest 6 mth postpartum reflections… (And swipe if you want to see the most handsome baby boy) 🔹At 6 mths I’m feeling kind of in a plateau…I want to be further ahead than where I am. It’s so fun to be back to the activities I love…but I’m not as fast or as strong as I was and I’m not always patient with the process. 🙃 ❓Does anyone else struggle with being patient with the process? I know change is happening but it’s slower than I’d like! Today is day 6 and we are focusing on postpartum core and pelvic floor! While it may be common to have lower belly bulging (DR) or leak a bit when you sneeze or jump it’s not NORMAL or something you have to live with. YOUR PELVIC FLOOR It’s not a floor at all. It’s a diaphragm of muscle consisting of 3 layers, acting as support for your pelvic organs! It’s not just a passive hammock either. These muscles can engage, relax, and lengthen. Strength is important, but so is the ability to let go of tension! Kegel that focus solely on engaging those muscles, and maybe only one part of those muscles, is not the answer to having a strong, functional pelvic floor. Read my honest 5 month postpartum highs & lows… and swipe to see my first postpartum race highlights! 🔹5 mths in and I don’t think I’m quite used to having 3 kids yet…It’s a little bit of a circus🎪 I love having my girls and a baby but it’s still 3 different humans to keep alive! I realized the mental load I carry when I was packing clothes for all 3 of them for the vacation we’re headed on! I’m sure I’ve forgotten at least 5 things for each of us but we will make do 😅 🔹As I’ve been packing for a beach trip I’ve been surprised that a lot of my pre-pregnancy clothes actually fit…but not completely how they used to.🤣 My belly is hanging on to more this time around. Can you relate?!? I’m trying not to compare this postpartum experience to my other 2. I think it’s crazy how you can do everything in pregnancy and postpartum the same but with multiple babies it just takes a larger toll on our bodies but that’s normal and okay! 🔹I’m so thankful that my body let me race my first 10k at 5 months postpartum! It has felt challenging to train properly because 3 kids is crazy 😂🤣 I have really been prioritizing my s postpartum strength and it has paid off! I’ve been so grateful that 1) the strength classes are short…no longer than 15-20 minutes so I can fit them in and 2) I can do them from home so I don’t have to load up the kids or drive anywhere! 💾Save this + follow for more core + pelvic floor workouts! 📌 A few fun things that I’ve been playing around with lately. 🔥Building trust, confidence and resilience with core and pelvic floor moves in your workout doesn’t need to look like lying on your back, kegels, and marching. 🔥 You know I love a good foundation, and you can’t out exercise your issues, but you need to progress and challenge your core and pelvic floor in ways that support their function. 🔥Dynamic strength + endurance and overloading are 🔑🔑🔑 to taking your core and pelvic floor workouts to the next level. 🔥Trust with dynamic eccentric loading - quick drops & catches w a focus on length for muscles that tend to clench and grip are also 🔑. This is always after learning to eccentrically load in a controlled (yes, foundational) level - like slower squats, split stance deadlifts, deadlifts, lunges, pushes and presses. Here’s my honest 4 month postpartum reflections… 🔹At 4 mths I’m feeling even better than I expected! I’m back to running and lifting and I’m currently training for 4 races this spring and summer! Its so fun to be back to what I love…coaching exercise classes and training for a half marathon!🏃‍♀️ I’m focusing on building back my speed and strength these next few months. 💾 Save this progression! (And follow for more PF 🫐 gold nuggets!) Kegels are great when you need to learn an isolated pelvic floor contraction. But most of us need more strength everywhere else to balance our pelvic floor strength, NOT a ton of isolated kegels. 🤫Probably not even kegels while you do these exercises… So why glutes?! Strong glutes take the load off of your back pelvic floor muscles. They hold you up from the back and power you forward when you walk. Weak glutes often look like clenching, which can cause PF tightness or gripping. Weak glutes can also look like a very ‘high lift’ and your pelvic tipped forward. This is stuck in length and your glutes can’t contract to tip your pelvis under. Both are not great for pelvic floor and core. Focusing on tap backs, hinging and step ups can work the whole range of your glutes, from lengthening to contracting. 🫐 This series is a progression! Work up to stepping into higher surfaces with control in your spine, core, and pelvis. 🚩And make sure you’re NOT bearing down! This can lead to pelvic floor tightness, belly bulging, leaking and prolapse. 🔥If you’re not ready to step onto higher surfaces, add weights to the lower step ups or hinges. Weights are 🔑 to growing your glutes, and I don’t mean puny ankle weights or 2lbs. Start those bigger weights 5, 8, 10, 15lb as soon as you can! Are you working your glutes well? Lmk if you’re adding these in! Glutes for pelvic floor and core support is a huge part of what makes Strength + Restore clients so successful at reaching their goals to get strong, stop leaking, heal Diastasis and keep the strength! Meet the team at District Pelvic Health! Dr. Rachel Payne is a physical therapist, Pilates instructor, and the founder of DPH. Born out of her own struggles navigating care during infertility, pregnancy, and postpartum, she set out to provide an individualistic approach the care she sees as NECESSARY for birthing people. . After a decade of experience as a physical therapist, Rachel was motivated to start District Pelvic Health after her own struggles navigating the medical system through infertility, pregnancy, and postpartum complications. She is a doctor of physical therapy, earning her DPT from the University of Miami in 2013.  She began her career in outpatient orthopedics, laying a solid foundation for understanding how the body works as a system. After completing her comprehensive Pilates training through Polestar Pilates in 2017, she became the go-to ortho PT for pregnant and postpartum patients to work with in conjunction with their pelvic floor PT.  While simultaneously struggling to get pregnant herself, Rachel began specialized obstetric physical therapy training in 2018, eventually doing internal pelvic floor training through Herman and Wallace. . im going to be honest…life is crazy at 3 mths postpartum with baby # 3.😅 Here’s my very honest 12+ week postpartum reflections… 🔷my brain is still gone. I’m literally still searching for my husband’s christmas gift that is “hidden” in our house...please send help!🤣 i’m choosing to laugh at all the funny things I say and/or forget! Please tell me that I’m not the only one?!? 🔷i have been SO HAPPY doing run:walk ratios! I am so excited to have passed the Return to Run Checklist and to be doing my run:walk workouts either outside or on treadmill depending on the weather. it feels good to get back to running but its crazy how hard it feels even though i ran through pregnancy. i can tell that i have some work to do on my posture and mechanics with postpartum running. I’m not back to my pre-pregnancy weight. My stomach is stretched out and squishy. My daughter tried to call me “big belly mommy” like she did in pregnancy the other day.😅 But this is postpartum. I’m here to help normalize it. There’s a reason I did photos at 8 weeks postpartum (and it’s not just because of the cutest baby smiles ever,) it’s to show you the process. Did you know your breath and your pelvic floor are connected? Imagine a piston where your diaphragm is the top and your pelvic floor is the bottom. Here’s how it works: . 💨 When you INHALE the piston goes down. This means your diaphragm flattens and descends and your pelvic floor lengthens. . 💨 When you EXHALE the piston goes up. This means your diaphragm rises and goes back to a dome shape and your pelvic floor lifts. . Why is this important? Because breathing is the absolute easiest way to influence your pelvic floor! It’s the first thing I give all my postpartum people to focus on. The way you breathe while you push during birth or having a bowel movement impacts your pelvic floor. Many pelvic floor dysfunctions such as leakage or escaping air with activities can be improved by changing a breath pattern because of these impacts on pressure management. . You aren't alone! ⁠ ⁠ Here are some possible causes:⁠ 🎈 Inflammation- caused by certain inflammatory skin conditions, as well as contact reactions from products or infections. Inflammation can also be caused from hormones, tight pelvic floor, and nerve factors which then leads to pain with insertion.⁠ ⁠ 🤰🏼 Hormones- a lack of testosterone and estrogen at gland openings can cause pain. These hormones help with natural lubrication. During times of breastfeeding, postpartum, perimenopause, menopause, or even certain medications may decrease these hormones in your body which can cause pain with insertion.⁠ ⁠ 🤸 Tight or Weak Pelvic Floor Muscles- can lead to irritation and burning of the nerve endings at the v@ginal opening.⁠ ⁠ ⚡️ Nerves- irritated nerves or too many nerve endings in the tissue of the opening can cause pain.⁠ ⁠ 😓 Are you suffering from this type of pain? We all saw Barbie 🎉 EXCITEDLY 🎉 announce she was ready to go to the gynecologist, so don't you think Barbie would do something about it if bladder issues were keeping her from having fun?!  A few months back, some friends and I enjoyed some Barbie themed fun, including ROLLERSKATING 🛼 wow, did it bring back memories, and serve as a reminder that I am NOT in my twenties anymore 🫠 Sometimes when you are in your 20's and even early 30's you aren't thinking about pelvic health issues yet, but how about when you are in your mid 40's and want to do something to make you feel young again, like rollerskating? 📌SAVE + SHARE📌 Your pelvic floor loves when your glutes and hips are really strong. This workout can feel great as it works posture/ pelvis positioning, mobilizing the pelvic floor muscles, and strengthening of hamstrings and glutes in gravity neutral positions. It can feel very good working in a hips up or anti-gravity/ gravity neutral position, because it is easier to not bear down in these positions. But just be aware, it’s still possible to bear down here, so pay attention to how you’re creating intra abdominal pressure. To do: - wall bridge 2x 10 - internal rotation rock backs 2x 10 - hamstring bridge w ball under 1 foot 2x 10 ea side - hamstring bridge w hamstring shakes 🔥 2x 10 ea side - glute kick backs 2x 10 ea side - side donkey kicks 2x 10 ea side Note: Resolving Pelvic floor issues and prolapse symptoms is a whole plethora of things, including really good breathing, understanding your symptoms, learning good pressure management, release and/or engagement techniques, ALONG with needing to get really freaking strong. What is ⭐️ normal ⭐️ pelvic floor function? How can you know if you’re having dysfunction if we don’t know what to expect? . When you’re pelvic floor is functioning correctly: 🔴 you urinate every 2-4 hours during the day 🔴 you wake 0-2 times to urinate at night 🔴 you have a pain-free bowel movement every 1-2 days that looks like uncooked sausage 🔴 you never have urinary or bowel leakage 🔴 you have no pain in or around your pelvis 🔴 you have no pain with s3x, gynecological exams, or tampon usage 🔴 you have no feelings of heaviness or pressure in your pelvis . Postpartum Pelvic Floor PT ❤️ What should you expect? . Pelvic floor PT has better viability than ever. Maybe you’ve heard you should see a pelvic floor PT postpartum but have no clue what that actually means. Let’s clear it up! . Whether meeting in the clinic or in your home, we’ll start by talking. A thorough and frank discussion of your medical history, pregnancy, labor, birth, and postpartum period and goals is essential for guiding our exam and treatment. Literally nothing is TMI - be prepared to talk pee, poo, and s3x. . Next comes a physical assessment. We’ll look at posture, functional movements, extremity strength, mobility of your spine and extremities, assess abdominals, check on DR, and more specific tests and measures depending on your complaints and goals. We’ll also assess the cesarean scar if that applies. . After our thorough orthopedic assessment we want to assess the pelvic floor directly. Gold standard is an internal evaluation using the PTs hand to determine strength, coordination, endurance and tone. But if you don’t want to do an internal assessment we can get a lot of information externally, even while staying fully clothed! . 📌Save these! 🌶️Also, these are more beginner-intermediate. Your ability to keep a good ribcage/pelvis stack is going to be key! Deep core is a great stabilizer and it acts like a corset between the ribs and pelvis. Through these moves think of maintaining control between ribs and pelvis as your guideline to having deep core working well. Check for any signs of low belly pouching or bread loading and back of the intensity or amount of reps until you feel like you’ve got control to keep firming and flattening. Through all of these exercises start with a great inhale into your side and back ribs, as you exhale think of a good ‘zip’ from the bottom of your abdominals and up. Once you feel you’ve engaged your abs, then start moving. 1. Plié bridge 12 x 2-3 sets 2. Kneeling lean backs 8-12 x 2-3 sets 3. Kneeling lean backs w weighted reach 8-12 x 2-3 sets 4.
