Domperidone (motilium) is a topic of great emotion, hope and controversy. It is not available for use in the US due to concerns about the side effects. I will do a further post eventually on Reglan which is what is used in the US. Reglan passes into the breast milk in higher doses and has a risk of extrapyramidal side effects whereas Domperidone has mood effects and it also has a risk of QT prolongation Motiilium is seen as a hope for many, many parents. And there are many cases where it can be extremely effective! But it is not a substitute or a work around for the frequent removal of milk that is required to establish a supply. It works really well in prematurity and nerve damage. And when you understand the science - it stands to reason that it works for these! In Part 2 I will go into the evidence a bit more and prolactin receptors and when it’s effective! Studies have shown it can increase milk by 90ml esepcallitbin prematurity. We’re not supposed to put Dosages on here. But it’s important that you slowly start it and slowly wean it down under the guidance of a doctor. The other day I got on my stories and did an open Q&A about all things SDI and PFL in California and one of the topics that people asked about the most was the mandatory 7-day waiting period with SDI. 🗓 Unfortunately it is unavoidable like most short term disability programs, so you won't start getting paid until the 8th day of your leave, whether that is before or after having your baby. So technically you receive four weeks off but only get paid for three. 🤷♀️ To help offset this, I recommend using your PTO, sick days, or vacation days through your employer to make sure you get paid for the week that SDI doesn't cover.
