🙋🏻‍♀️“The Doctor told me to keep the pit on when there was tachysystole. What can I chart to protect myself in court?” If you’ve been around, you might remember when I was writing "Shifting the Pitocin Paradigm" I asked you all here, what big burning questions you had. What tripped them you up or made work hard when it came to Pitocin? And this was one of the top responses. Whew....deep breath. The hard truth is, if your involved in a lawsuit, a patient was hurt. And there's nothing you can document that will help if you negligentl continue pit ...even when a doc is pressuring you. The standard of care doesn't let us as nurses to knowingly continue down an unsafe path. But let’s delve into what the question is really expressing: "I know this is unsafe and I'm worried my patient is going to get hurt". That's a really scary thought. It feels more comfortable to be on the defensive, to think we need to protect ourselves. Just chart a "per MD order" & call it good. But the standard of care & our licensure, demands we embrace the role of pt advocate & lean into this feeling. Start with a conversation with the provider & express your concerns. If it doesn’t work, ultimately this is what the chain-of-command is for. Not comfortable, I know, but there's a reason we have it. And guess what? You’re already in an uncomfortable position… It’s a lot. But we’ll get into it, plus documentation, Pitocin, legal systems, fetal monitoring, all the uncomfortable & difficult stuff we run into on the floor in Reasonable & Prudent: Lawsuit- (and Worry!)Proofing Your Practice. 40 years of nursing... I retired Wednesday after working just shy of 40 years at the same institution and here are some of my take aways... 1.Never forget why you're there, to take care of the sick and injured people in your community. 2.Never let a doctor make you cry. A good share of them were bullied as children and those who weren't probably should have been. 3.No one ever died from charting something twice. 4.It's ok to push Lasix fast (ain't time for that) 5.Don't be afraid to ask a cop if his mom knows he's out this late (keeps it real) 6.No one dies alone, EVER. 7.Undress your patient for F sake, you'll be surprised what you find. It's your job to "find" these things. It may be a remote control under a boob, or a stab wound to the flank, you can't see either if they're dressed. 8.You'll make lifelong ride or die friends that you will cherish just like family. They are a gift. 9.You'll also develop relationships that are mistaken for the same, as hurtful as recognizing the difference is, you also will learn from these. 10.When patients lie to you (and they will), it's not personal. It's not about you. 11.For God's sake, if you don't know the arm drop test, learn it. (and use it so you don't say something you shouldn't because you think they're unconscious) 12. I regret nothing, loved (almost) every minute of my nursing career. It's all I ever wanted to be since I was in 3rd grade. I feel so lucky, to have been mentored by Kathy Ryckeghem and Sue Beswick, 2 absolute treasures. 13. A messy house, the laundry, the errands… Can all wait. But these moments Can’t. I am beyond grateful for the opportunity to stay home with my girls. The time truly goes SO fast. I honestly feel like all the time in the world still wouldn’t be enough! But being becoming a SAHM meant giving up my career as a nurse. After awhile I began to feel like I lost myself + needed more! Thankfully I am able to have both…moments like these while being able to make additional income (while helping others). 🙌🏼 In fact, I even hit a promotion this past month despite being sick for 2 weeks and having sick littles at home. Remember when I said you didn’t need another documentation course?! Funny enough-he-he- I made a documentation course😳 “Okay, Jen, then why did you make it when you said you never would?”, well here’s the thing… Is documentation the #1 thing that keeps patients safe and you from being involved in a malpractice suit? No. Nope. It’s not even listed as a cause of pt injury by the Joint Commission. It’s tempting to use documentation as a scapegoat when we have fears around pt harm and lawsuits (“It must be the documentation!”). Know what? It makes sense that it’s confusing! Not because documentation is inherently difficult, its because we get mixed messages all. the. time. Along with the mixed messages, we also get scary messages all. the. Time (“it’s all you have to defend yourself!!” You’re powerful! You’re the #1 thing that keeps pts safe in the hospital! I do still stand by the belief most nurses know how to chart just fine, but its still the #1 thing I get questions about. Just last week in an EFM class, a student told a story of a very intense nurses station debate on her unit, every one with strooong opinions. Its taking up a lot of mental space. So here we are…I made a class, with how to chart according to the SOC, when to chart, how to chart to ↑pt outcomes & prevent trauma, as well as a compilation of all the weird, random situations that seem tricky (like a patient declining something for example). But, the name isn't set… I even noodled on it with some friends and this is the best we came up with. The closer I’m getting to the BIG 4 0… The more and more I realize… the importance of finding balance in life. the importance of being grateful for those “small” things and “simple” moments. the importance of letting go of the idea of perfectionism. the importance of acknowledging how far you’ve come. the importance of meeting my needs. the importance of finding YOU. Se cree que las mujeres “naturalmente” son buenas para el servicio y cuidado a l@s demás. In February 1982, I graduated from San Joaquin Delta College with an Associate degree in Nursing. Later, while working, I graduated from Saint Mary's College of California with a BA in Health Services Administration, in 1995. I was privileged to practice nursing as an RN for 38 years. I first worked in an Oncology clinic and then in an OB/GYN clinic for a couple of years. Then, I enjoyed working in geriatrics and long-term care for several years. Primarily, I worked in our community hospital. I enjoyed working in Med/Surg, Pediatrics, Pain Management, Nursing Management and Case Management. The Unit Meeting is back on April 7th! April we are looking at Fetal Monitoring research and have we picked two super important, yet commonly overlooked research articles. If you're into simultaneously having your mind blown, but also feel like "wow! everything makes soooo much more sense" when it comes to the much misunderstood (and misutilized) fetal heart monitor...then this is the month for you!! We'll answer such questions like: wtf do they mean when people say "the fetal reserve" and wtf do we have that nonsense of a Category 2? "The Fetal Reserve Index Significantly Outperforms ACOG Category System" 2019 by Evans et. al. This is such a crazy thing, but it happens all the time. New Nurses👩🏻‍⚕️come out of school bright-eyedand ready to take on the world. Then they hit the floor🏃🏻‍♀️and us experienced nurse, disillusioned by our experiences😠, feel we need to make them realize what it’s really like. So, we grab our 🍴 and we get ready for dinner. We need to break this habit. The old that’s how I was trained and now I know what I’m doing so it must work idea! Well it doesn’t work, so let’s put down the cutlery and step away from the new grads!! Let’s envelop them in our protective nurse arms (not literally) and let’s make them feel safe with us. Let them know they have back up. The patients🥪 and hospital admin🍕 will make sure they get the idea that healthcare is hard. As an experienced nurse💉, let me tell you that I have learned so much from every member of my team!! I have learn numerous skills that help me in my practice daily from my previous CNAs! As a profession, healthcare puts us in tiers based on our education levels. This causes a divide at times where people in higher tiers think they can’t learn anything or are in someway superior to those people in the the tiers below them. This is not the case! We need to break down those tiers and work together as members of the same team! When we build a good relationship with out support staff, it improves the team’s dynamics and makes for a better working environment!! Thank you to all the CNAs and Techs in my career!!💩🏋️‍♀️ Who have taught me how to turn a patient when you don’t have many people to help you, how to change a brief with the patient on the toilet without taking off their pants, and other important nurse life skills..

✨on my own✨ A few thoughts on going through orientation, again… -a lot of people will think you are a new nurse and try to explain things. This can be good and bad. -be patient with yourself when you don’t know things right away. -take charge of your own learning. No one knows what you know and don’t know. -some preceptors suck. Ask for the ones you like. -your workflow will change and change again when your preceptors change. Have patience with these changes and expectations. -you don’t have to be excited to work on a new floor. You’re burnt out. Give yourself grace. At least you showed up. -you will have to break old habits that you’ve built. -Find people you can trust and count on. Especially on a floor where most people are new nurses (no offense but new nurses don’t know anything) -trust your gut -you will learn a lot of new things and you will have to spend some time off work debriefing. -work culture on a new floor can be completely different and you may not like it but you are here now for at least the next year so just accept that it’s your new reality. I need to get my hair done… If you read all this, thanks!! I’m nervous and surprisingly excited to be on my own.
