Yes, your doula can support you in the OR! As we close Cesarean Awareness Month, this is an important reminder that doulas can & do support c-sections, both planned and unplanned. As a doula who supports primarily hospital births, my favorite in our area is always Morristown Memorial @atlantichealth because they respect and value a client’s rights to have their doula support them, regardless of the type of birth they’re having. I have always been allowed in the OR there and very often go in before a spouse/partner is able to. I’m there to literally and figuratively hold your hand until you’re ready to let go. Before the month is over. April is C-Section awareness month. I never mentally prepared myself that this could be a possibility. My Story: Started contractions Thursday night (11pm). Headed to the hospital at 5:30am Saturday only to find out I had barely dilated and had to be induced. Long story short, my contractions were about a minute apart while I was only dilated 2-3 cm so the pain was unbearable. My water broke on its own sometime Saturday. Finally around 6pm Sunday the doctors told me they would recommend a C-Section as I hadn’t progressed further from 7cm in the last 6 hrs and since Siena was trying to push out since 2cm her head was getting swollen. I asked for extra time to talk to Eddie & my mom without the doctors and within 15 min I had made my decision. I didn’t get the video I always dreamed of. I didn’t even get the photo at the operating table with the baby because Siena had to be suctioned immediately (we did hear her loud cry!). On this last day of #cesareanawarenessmonth, I want to amplify that, with advocacy and the right birth team: ✨ you CAN have a beautiful, healing, gentle, family-centered c-section. ✨you can be the FIRST person to hold your baby, instead of the last. ✨you can come away feeling proud and whole, even if a c-section wasn’t your first choice. I want to celebrate the OB and anesthesia providers who humble and decenter themselves to make these kinds of births possible. And shine a light on the birth givers who are brave enough to voluntarily walk back to a place that is filled with past trauma and complications for them. These moving images were graciously shared by repeat client Mariah, coincidentally on the 2 year anniversary of an extremely traumatic primary cesarean under general anesthesia. When I asked how she’d feel about doing this for CAM, she said, “It’s kind of fitting on his second birthday. I can finally talk about his birth story without crying.” With a unique history, including a vaginal birth prior to her cesarean, in her next pregnancy, she planned for a VBAC with a supportive provider. At 41 weeks, considering her matrix of risk factors, she knew that the balance of risks & benefits tipped in favor of a planned repeat cesarean. We made sure that the charge nurse, anesthesia team and OB providers knew: ✅ she had a history of severe trauma. ✅ she had traveled a LONG way to birth at this hospital on their reputation for patient-centered care. ✅ she would need her doula with her to go into the OR. ✅ we may need to pause for a timeout during the spinal if things became too overwhelming. There were tears & flashbacks. But she was brave. And she overcame. And as soon as baby was out, the OB handed him straight over the curtain to client himself ‼️ No stop at the warmer. No handoff to nurses. All assessments done on her chest behind the curtain. 👏I know every cesarean mom on my page will feel the magnitude of that in their soul👏 This is one of the MOST moving births I’ve ever attended. Our little Juliet Hart Cook was born this morning at 5:52am 🌸 She is an absolute dream, and we are so in love with her! We are both doing well, and I can’t wait to take her home tomorrow! This was by far the easiest delivery I’ve had. My water broke around 11:30pm last night. By the time we made it to the hospital, I was dialated to a 5. The anesthesiologist came with the epidural quickly, and I wasn’t in pain for very long which I’m very grateful for. It didn’t take long to push, and bam! She was here! •Testimony Tuesday• This was my 3rd natural birth completed at WCH women's pavilion with Bloomington Women's Care Dr. MarcAnthony delivered my first 2 boys--I delivered quickly after arriving to the hospital-- so this was my first birth "experience" at the hospital due to the extra time with my water breaking... At 41 weeks on a Sunday, my water broke at home around 3:30 in the afternoon (a very minimal amount so I wasn't sure if this was "it") , I called and spoke to CNM Kelly who wanted me to come in and confirm that the water had broken even though contractions had not started due to the quickness of my previous births. Nurses Abbey and Carmen (and student nurses) checked me in and did an amazing job of setting up a home birth atmosphere within the hospital room. Candles, projecting lights, a birth pool and other cushions/balls were all set up. I additionally had brought a TENS unit, essential oils and bluetooth speaker along. As with my previous labors, my preference was for intermittent doppler monitoring and saline lock to be the only interventions. With permission from my birth team I did start my TENS unit to alleviate a low constant backache I had been having all day. Around 9 Dr. Marcanthony broke the rest of my waters but it was lightly tinted with meconium. After discussing my birth preferences and reviewing policies Nurse Adrianna and Dr. MarcAnthony agreed I could continue with the plan for the birth pool but I would need to be on a continuous fetal monitor. Thankfully there was a bluetooth option that allowed me full freedom of movement. Dr. MarcAnthony prayed with us before leaving. Kelly then arrived and we discussed my desire to not use pitocin and ways to get contractions going naturally, thankfully they started on their own not long after our conversation……. About one in three U.S. births is a cesarean delivery. However, 10-19% of births actually require a cesarean delivery for the health of the birthing person, the baby, or both. This means that many C-sections are unnecessary. As the procedure involves more risks for both the birthing person and baby than vaginal births, it should be avoided when possible. The quality of maternal health care in states with severe abortion restrictions will go down as OB/GYNs and medical students leave those states because they are not able to offer comprehensive reproductive care without fear of legal repercussions. This will lead to more unnecessary c-sections, worse maternal and fetal outcomes, and worse birthing experiences overall. As such, it is important that we are educated and prepared to advocate for ourselves. Tips that may lower the chances of having a C-section: – If the pregnancy is healthy with no complications, consider giving birth at home, in a birth center, or with a midwife. – Ask about your medical provider’s birth philosophy. – Don’t induce labor unless there is a clear medical need. – Don’t go to the hospital until in active labor. – Have continuous labor support from a companion/advocate, such as a friend or doula. I think I have finally found a way to capture what I want to do, all encompassed in one, for our community of women & mothers, to elevate the care we are able to give to our clients. One of the largest areas of need in our patient population is the need for education and support. At Integrative Pregnancy + Birth Services, as a pregnancy and birth counselor/educator. I plan to serve our community of women and mothers by providing resources, support and education for your health and well-being. This is particularly geared toward hospital birth settings, but can be tailored to individual needs for more holistic settings. I would like to build a team to assist with needs futuristically. As you know, I am passionate about empowerment through education, trauma-informed care, informed care and consent. If you don’t like the way something is done, you have questions, or you just don’t feel like you are being treated fairly, SPEAK UP! It’s OKAY to be assertive, and it’s OKAY to advocate for yourself, in fact, you SHOULD advocate for yourself! Providers, nurses, or any member of your healthcare team DON’T always 100 percent of the time know exactly what’s best for you, and YOU are an important part of the healthcare team too! It’s IMPORTANT to educate yourself about birth & know what you do and don’t want, and it’s also IMPORTANT to LISTEN to your provider if he/she is suggesting something you are wary of. Listen, understand the pros and cons, and then BOTH come to a decision that you both feel comfortable on! And, trust your gut! 🥰 Remember - finding a provider you trust and one that listens to your concerns is so important! But, at the end of the day, all decisions about your prenatal/birth/postpartum care are up to YOU!
