After 13 months I woke up and I’m over it. The twins weaned themselves around 12 months. I’ve still been pump. I told myself, I’ll wake up one day and just know that it is time. It’s time. HAND EXPRESSION! My all time favorite way to boost milk supply. If you’ve worked with me before, you know my first thing is start hand expressing. In the hospital, just hand expressing 5 minutes after a breastfeeding session has shown to greatly increase milk supply. With hand expression you are also able to give baby those extra drops of goodness while he/she & you are learning how to latch. But the best part? According to a study in 2021, “Colostrum collected by hand expression had a higher lipid concentration and caloric content than that collected by an electric pump.” Get your hand expression on! Barriers to Breastfeeding 🌿 This one might ruffle some feathered. People often think “oh you just go ahead and put boob in baby’s mouth” While yes that is ONE step. But, leading up to the birth process to trigger that prolactin things happen. There are major medical interventions that can affect the delay of Lactogenesis stage 2. The biggest one being labor inducing medications or in this photo drugs and chemicals. These labor inducing medications can cause a cascade of issues whether it’s baby is sleepy, they’re not as aware. Cesareans can cause a delay due to separation and not getting that first golden hour. Yes, cesareans are very necessary and very needed when it arises. Mechanical and Physical forces, being forceps or vacuum can affect the breastfeeding journey. Limited mobility, such as swaddling. Swaddling restricts the movement and doesn’t ensue proper body position. Remember, nose to nips and tummy to mummy. Ears, hips, shoulders and knees should all be in a straight line. Swaddling prevents this. Fetal monitoring and IV fluids can also be a barrier to the initiation process of breastfeeding. Other barriers to breastfeeding such as separation (mentioned above), interventions (mentioned above), swaddling, pacifiers, supplementing and medical conditions are all barrier to breastfeeding. The Mastitis Protocol was recently updated in 2022. 🌿 Boyyyyyyyyyyy do I see some crazy things still being recommended for Mastitis and Engorgement. Only those things are damaging and can worsen the effects of Mastitis and engorgement. What’s the new protocol? 🌿 Find the root cause. Recurring engorgement and mastitis is NOT normal. There is physiological engorgement and pathological engorgement. We need to find out what is going on to have these come back. 🌿 Feed frequently and effectively. 🌿 Do not add in any extra pumping sessions. The “fullness” you are feeling is actually swelling in the surrounding tissues and breast. Adding that extra stimulation will just make that breast angry. 🌿 Ice packs and COOL compresses to help reduce the swelling. NO HEAT. Again, heat may exasperate the engorgement. Heat MAY improve the oxytocin uptake. Oxytocin is the hormone that triggers the prolactin. 🌿 Gentle breast massage from the areola upwards towards the breast. DO NOT USE ANY VIBRATION. Vibration will cause inflammation in the surround tissues and cause more breast “fullness.” It has the potential to damage the surrounding tissues. 🌿 Use an OTC anti-inflammatory nonsteroidal to help the pain. I’ve been through the abnormal part of the engorgement and it is not fun! The root cause for me were a couple things. With my first, it was due to oral restrictions and incorrect pump flange sizing. With my second, it was due to oral restrictions and not enough milk removal because of other motherly duties. April is Cesarean Awareness Month. In the US as of 2022, 32.1% of live births were cesarean. If you think a cesarean is the “easy way out” you need a huge reality check. There are 7 layers of tissue that need to be cut through to reach baby. It is a major abdominal surgery. You have a higher risk of dying on the OR table than you do a vaginal birth. Cesarean recovery is the most brutal recovery I have ever gone through and I did it 2x. The trauma is beyond what you can imagine. We have a 2w check up and then go back to 6w. We aren’t given a belly binder, we aren’t referred out to a pelvic floor therapist, a core therapist. All while being sent home to care for a child you aren’t even supposed to hold standing up, or can’t because it feels like everything is going to fall out. There are life long side effects. Mine? I have severe scarring on my bladder, kidney tubes, bladder ureter, dense adhesions. I have diastasis recti. I planned an HBAC and ended up in another surgery. What was supposed to be my most healing and redemptive birth, traumatized me into the fear of getting pregnant again. When you tell a mom “you took the easy way out” “you’re not a real mother” please, think of the trauma that family endured. Some mothers don’t come out of that OR with their baby. Some husbands and newborns walk out of that OR without a mom and wife. Research your hospitals cesarean rates, research your providers. RESEARCH.
