We received the most lovely email testimony today. High-quality, comprehensive care is one of the most important missions at our clinic. We not only strive to provide thorough, compassionate, individualized care, but to listen to patients and not just look at lab results. This is when magic happens. “ Dr. Fotino caught my pancreatic cancer before it became cancer. I had no symptoms. I’d been seeing her for years to address extreme bloating & hormone imbalance. She identified my thyroid issues & helped me get off of several medications that were throwing my body off, and my body slowly became more balanced and at ease. But, over the years, my liver enzymes were elevated & nothing we tried (supplements, diet changes, etc.) changed them. Out of curiosity, she ordered an ultrasound, and we found a 6cm MCPN (mucinous cystic pancreatic neoplasm) with cancer markers on my pancreas & spleen. I was able to have it removed before it became fully cancerous and spread. Known Side Effects of PPIs The general side effects of PPIs (found on the inserted prescription package or medicine website) include, but are not limited to: Headaches Migraines Stomach and GI upset Fatigue Muscle spasm/cramps Interactions with other medicines A person may not associate these symptoms with the PPI they are taking and may seek out additional prescription or over-the-counter medicines to treat the new symptoms. This inevitably leads to taking more medicines which creates additional stress on the body. Health Problems Associated with Long-Term Use of PPIs Data shows that long-term use of PPIs increases the risk for serious health conditions including: Kidney disease Lupus Migraines Neurological conditions Hearing and vision impairment Osteoporosis Bacterial infection, particularly Clostridium difficile (C. diff) Pneumonia Cardiovascular disease (heart disease, heart attack, stroke) Dysbiosis (imbalance of gut microbiota) Vitamin and mineral deficiencies New Guidance for PPIs: Lifestyle Changes Before Medication The strong association between the long-term use of PPIs and adverse health risks has raised concern from professional medical associations and the FDA, resulting in new prescribing guidelines for PPIs. Doctors are now advised to prescribe "the lowest possible dose" of a PPI "for the shortest duration of use" (two weeks with rare exceptions) and only when other approaches have failed. These "other approaches" focus on lifestyle changes to reduce or eliminate the need for PPIs altogether. What Causes Reflux? People can experience reflux symptoms for several different reasons: Sphincter Muscle Dysfunction. At the base of the esophagus is a flap-like muscle called the lower esophageal sphincter (LES). If this opens too frequently or does not close properly, stomach acid backs up into the esophagus. In many cases, improper closure of this muscle is related to a person's diet. Caffeine, sugar and a diet high in processed food, as well as food allergies or sensitivities, can affect the LES. Other things that can affect the function of this muscle include smoking, medications, and chronic stress. Obesity, pregnancy and having a hiatal hernia may also lead to LES dysfunction. Insufficient Production of Stomach Acid. One of the signals for the LES to close is a rise in stomach acid. If the body's acid is suppressed or you are not producing enough stomach acid in response to the food you eat, that food travels back and you regurgitate. Other times, there may be cramping or burning sensations (known as heartburn). Rare Genetic Conditions. A very small percentage of people are born with a condition in which they do not produce enough acid. The condition creates an acid-bicarbonate imbalance causing their LES to not properly tighten. By not producing sufficient stomach acids for digestion, the body isn't absorbing the nutrients it needs which consequently leads to other chronic health issues. This genetic condition has led researchers to question what happens when you take a medicine, such as a PPI, that causes artificially-reduced stomach acid. They found that people taking medicines to reduce stomach acid experience the same types of symptoms and health issues that are seen with people who have genetically low acid production. What's the big deal? Over the last 15 years, studies have shown two things: People have been using PPIs longer than 60 days. In the digestive process, the body requires these acids for a relatively short period of time—long enough to break down foods into nutrients that can be absorbed as they move through the intestinal tract. This is how nutrition (vitamins, minerals, proteins, carbohydrates, amino acids, proteins, and fats) is provided to all cells, tissues, and organ systems in the body. The body can then access what it needs to heal wounds, recover from injury, bounce back from stress, and for the immune system to function properly. In a healthy digestive system, once the stomach acids have done their job a signal alerts to turn off causing the release of bicarbonate to buffer the acid production. Let's talk about what reflux is! Acid Reflux (aka GERD) Reflux symptoms can range from mild to aggravating and quite painful. In addition to the burning sensation most often experienced, other symptoms can include coughing, congestion, and difficulty swallowing. When reflux symptoms become persistent and intense, it may be labeled as gastroesophageal reflux disease (GERD). Proton Pump Inhibitors (PPIs) are the most widely prescribed class of drugs in the world and are used solely for GERD. They include the drugs omeprazole (Prilosec, Prilosec OTC, Zegerid), lansoprazole (Prevacid), and pantoprazole (Protonix). Evidence shows that PPIs present serious health risks for people who use these drugs and yet 113 million PPI prescriptions are filled annually, and over 13 billion dollars in sales are generated. What is a Proton Pump Inhibitor? Why is it so bad? Proton Pump Inhibitors are used to treat the symptoms of gastric disease. They were intended for short-term use to suppress stomach acid production by irreversibly inhibiting acid production within specialized gut cells called parietal cells. PPIs shut down the production of nearly all the acid in your stomach. You may wonder what could be wrong with this approach if it makes a person feel better. To get a clear understanding, we have to take a brief look at the important role stomach acid plays in the digestion of food and the absorption of nutrients. Stomach Acid: Essential for Healthy Digestion & Gut Health The stomach is the most acidic organ in the human body, producing several different types of acid and enzymes ("gastric juices") to help break down the various foods that we eat. Okay so I just tried matcha for the first time and don’t hate me, but I didn’t love it. 😀I love green tea, the stronger the better, so I figured I would love matcha. I think it was the tea combined with a nut milk and collagen powder confusing my taste buds. I usually just drink tea steeped in hot water with nothing added. Not only are fermented foods tasty, they are great weapons against viral infections, including influenza. Research has found that Lactobacillus plantarum, a bacteria commonly found in fermented foods, specifically Kimchi, could hinder the growth of the flu virus. It is suspected that the lactic acid produced by these bacteria acts as an antiviral agent disrupting the viral membrane and activating immune cell mobilization. An animal study out of Korea found that daily administration of lactobacillus plantarum fortified yogurt enhanced host immunity and helped to prevent influenza type A viral infection. Daily administration of the L. plantarum fortified yogurt before influenza type A infection also increased natural killer cell activity, lung inflammatory cytokine responses, and survival rate of those who were infected with influenza type A. Many still believe that fish oil can increase the risk of bleeding and many doctors still warn of this. There have been many studies debunking this myth. Meet Dr. Fotino: Dr. Fotino received her medical doctorate from Albert Einstein College of Medicine of Yeshiva University in Bronx, NY, completed her internship and residency in internal medicine at Tulane University Health Sciences Center and is now Board Certified. Dr. Fotino has also obtained a Master of Public Health degree at Tulane School of Public Health with an emphasis on epidemiology. She graduated cum laude from Smith College in Northampton, MA, where she received a Bachelor of Arts degree in political science and a minor in Italian language and literature and concentration in music history. Dr. Fotino has a special interest in incorporating nutrition into medical care. She spends time with each patient to understand the underlying cause of disease in order to get to the root cause. For many who have recurrent pregnancy loss the answer of why is not always clear. It is estimated that recurrent pregnancy loss affects 5% of families who try to conceive and so far, the known pathogenesis is found in only 50% of cases. Although more research is needed, one factor involved in recurrent loss is immune response. Due to paternal antigens expressed on the surface of fetal tissue, immune tolerance and even immune suppression is necessary to sustain a normal pregnancy. A study published in Human Reproduction found that vitamin D deficiency increased the risk of recurrent miscarriage by promoting cellular immunity and autoimmunity. The study found that the prevalence of antiphospholipid antibodies, antinuclear antigen antibodies, thyroperoxidase antibodies, and anti-ssDNA antibodies were significantly higher in those with low vitamin D compared to those with normal vitamin D levels. Assessment of vitamin D levels is recommended in women with recurrent miscarriage and we suggest this level being checked in all women during preconception. Aim for an optimal level of vitamin D between 50 – 80 mg/mL. A new exciting study from the University of Utah School of Medicine has found that the microbiome may protect us from viral induced neurological damage. This research could change the way we look at neurological disorders such as multiple sclerosis, Alzheimer’s, Parkinson’s and more. Dysfunction of the gut bacteria might be key in allowing harmful viruses to trigger neurological diseases. In the study, mice with disrupted microbiota showed less active brain immunity through decreased activity of the microglial cells (immune cells in the brain) and that germ free mice receiving antibiotics were unable to control viral replication within the brain leading to increased paralysis after being infected with a neurotropic virus. They also found that the microglia depletion led to worsening demyelination. The study found that signals from microbes in the gut are essential to quickly clearing viruses in the nervous system and to prevent damage. The microbes do this through something called TLR4 signaling. They are unsure as to which bacteria in the gut is responsible for this signaling, but I am sure there is more information to come. This research might also give us further insight into conditions such as PANS/PANDAS and other childhood disorders. With the microbiomes role in so many conditions, it’s worth checking on your flora with GI Mapping. If you’ve never had GI mapping, it’s a great way to check out your microbiome and to see if there are any unhealthy guys hanging around. Preparing for a healthy pregnancy is one of the most important things you can do for your future baby and their future baby. There is overwhelming research to support proper preconception care. One reason for this is to ensure DNA is being properly read and that damaged genes are turned off to prevent disease. One way to turn off damaged genes is through methylation, or one carbon metabolism. Evidence shows that deficiencies in one carbon metabolism contribute to subfertility, early miscarriage and a compromise in babies health. Nutrients that support one carbon metabolism include Vitamin B12, choline and folate. Research shows that those taking these nutrients or consuming foods high in these nutrients in the preconception and periconception period had improved outcomes and showed positive affects on the health and longevity of the offspring. Fall is one of my favorite times of the year. Mostly because it's the perfect time for cinnamon nutmeg smoothies. For this smoothie add a handful of frozen or fresh kale, 2 scoops protein powder (I use metagenics ultra meal advanced protein vanilla), 1 tbl chia seeds, 1 tbl almond butter, 1 tsp vanilla extract, handful of ice, dash cinnamon and dash nutmeg, pour in an alternative milk such as almond milk, enough to just cover everything. Sole! When Himalayan salt is dissolved in water it creates a solution called Sole. Sole is chock full of minerals and can be very helpful for fluid balance in the body. It can also be especially helpful for those with adrenal fatigue, as with adrenal fatigue the body tends to dump minerals, including sodium which is vital to our health. To make Sole just place natural Himalayan crystal salt rocks in a jar and cover with water. Let sit for 24 hours then you can add 1 tsp of Sole to an 8 oz glass of water per day or as needed. In case you are concerned about sodium, one tsp of Sole has roughly 490 mg of sodium whereas 1 tsp of table salt has roughly 2292 mg of sodium, depending on the salt used. Sole is not just sodium.
