Do you have knee pain when doing deep squats? These are just a couple of signs of PFPS or "runner's knee." โ  โ  ๐Ÿฆต๐ŸฝThe most effective treatments for PFPS are a progressive hip and quad strength program and a knee-specific return-to-run program. Since the structures around your knee have become weak from overuse, you want to start with easy exercises and runs that are well tolerated by your injured tissues and then work your way up to difficult exercises and runs so that your body becomes strong enough to handle normal training again.โ  โ  ๐Ÿ—“๏ธ A return-to-run program needs to be started as quickly as possible to maintain a running-specific load on the knee tissues. Run:walk intervals are key and then you gradually work in running that puts more stress on the knee, such as downhill running. The basics of movement are the same. Returning from an ACL reconstruction is a long road. The physical challenges are obvious, but often the psychological hurdles can be overlooked. Many athletes tie their identity to their activity level or sport. When their sport is taken away from them after an ACL injury, it can feel like their identity is being ripped away from them as well. As rehab progresses, we constantly assess ROM, strength and other objective physical measures to ensure a safe return to sport for our athletes. However, the athleteโ€™s confidence in their return is a key piece to the puzzle that cannot be overlooked. This study found that young ACLr athletes who scored <76 on their ACL-RSI were at a highly likelihood for a second ACL tear (see reference). From this study, we can infer that our athletes should be scoring AT LEAST a 76 on their ACL-RSI before returning them back to their sport. If you want to set realistic and achievable fitness goals, itโ€™s crucial to focus on what you CAN control. No sense in wasting your energy stressing over factors outside of your control, such as how busy the gym is or the weather. Instead, channel your efforts into things like your nutrition, exercise routine, and mindset. Remember, progress is progress, no matter how small. When it comes to setting goals for yourself (or others), we need to realize we canโ€™t control everything. The sooner we come to terms with that, we can begin to focus our attention on the things that we *can* control. Goal-setting: Control the controllables. First, identify the clear outcome for this particular goal. Define it. Be descriptive. Next, reverse engineer the process. Work backwards from the goal to where you are now. Create process-oriented goals (โ€œmini-goalsโ€) along the way. Identify how you plan to achieve them. This will finally lead to where you are now. Youโ€™re going to need to assess what youโ€™re currently doing and what needs to be modified in order to obtain the goal. My suggestion is to focus on building the fundamentals and the big rocks first. An example of this is sleep. Big Rock: You can get a very quick win just by increasing sleep quality and/or duration. This can help bleed into optimizing recovery, which then feeds into greater output and greater performance improvements in your training sessions. Little Rock: Trying to improve recovery or training by chasing things like ice baths, sauna sessions and massage guns. May is Brain Tumor Awareness Month ๐Ÿง  In 2014 I had a brain aneurysm that ironically saved my life in more ways than one. Because of the pain after surgery a CT was done which discovered a brain tumor. My medical team didnโ€™t provide any support and I was given an endless supply of pills. After 20+ year of coaching people at all levels while tracking intensity and recovery with heart rate training and HRV, I've observed the following: โžกMost people with full-time jobs and busy lives don't need, nor can they recover from, more than 1-2 high-intensity workouts/week โžกPeople with a high work capacity and lifestyles that support recovery can generally sustain 2, sometimes 3, high-intensity workouts per week โžกPeople doing 4+ true high-intensity workouts per week will either end up burned out and injured or are using PED's to make it possible It's easy to think that if a workout is hard or makes you tired, then it must be high intensity But the truth is that the way "high intensity" is defined and used in research is often very different than the way people talk about it in the gym A simple rule of thumb is that if you're training at or above 90% of 1RM for strength and/or above 90% of max HR for conditioning, then it's high intensity These intensities recruit the highest threshold muscle fibers, place the most stress on your CNS, and impose the greatest demands on your energy systems The cost is that this also leads to high levels of fatigue that accumulates if you don't allow for proper recovery between workouts It's incredibly important to understand that intensity is a tool that can build your fitness up or break your body down.
